  • Making Sense of Organizational Change and Innovation in Health Care An Everyday Ethnography

Taylor & Francis Ltd (Sales) Making Sense of Organizational Change and Innovation in Health Care An Everyday Ethnography

    1 in stock

Book SynopsisThis book explores the hospital via organisational ethnography (OE), an approach that involves a mix of fieldwork methods designed to analyse the hospital which also includes participatory observation, qualitative interviews and shadowing. One way to define a hospital is by its high level of formal organisation, resulting in written or digital communication as the main source of communication in patient journals, minutes and medical and quality guidelines. In contrast, in this book, the aspects of the informal organisation will be the focus. In spite of the many formal regulations of healthcare, hospitals are also chaotic organising places where many different groups of people interact in order to negotiate, to practice and to make sense of daily work tasks. The underlying argument is that, in the mundane everyday life of hospitals, frontline workers and their interactions with patients and local managers remain at the core of organising hospitals. The overall purpose of this book is to report stories back from the field of healthcare, demonstrating how people, spaces and work (as examples of events) become important elements of organising hospitals. The book will be of interest to students and scholars in and across healthcare management, organisation studies, ethnography, sociology, qualitative methods, anthropology, service management and cultural studies.Table of Contents1. Introduction. Making sense of organizational change and innovation in health care: An everyday ethnography 2. A theoretical narrative approach to organizational change studies: Towards everyday organizing 3. Making sense of everyday innovation at a clinical ward through individual narratives of visitation routines 4. A resistance and everyday view on health care professionals: Meeting encounters with patients and professionals 5. Designing and driving collaborative, everyday innovation using combined narratives of internal and external participation 6. • Neoliberalism Globalization and Inequalities

Baywood Publishing Company Inc Neoliberalism Globalization and Inequalities

    1 in stock

Book SynopsisSince U.S. President Reagan and U.K. Prime Minister Thatcher, a major ideology (under the name of economic science) has been expanded worldwide that claims that the best policies to stimulate human development are those that reduce the role of the state in economic and social lives: privatizing public services and public enterprises, deregulating the mobility of capital and labor, eliminating protectionism, and reducing public social protection. This ideology, called ''neoliberalism,'' has guided the globalization of economic activity and become the conventional wisdom in international agencies and institutions (such as the IMF, World Bank, World Trade Organization, and the technical agencies of the United Nations, including the WHO). Reproduced in the ''Washington consensus'' in the United States and the ''Brussels consensus'' in the European Union, this ideology has guided policies widely accepted as the only ones possible and advisable. This book assembles a series of articles that cTrade Review"An impressive and comprehensive set of insights into the current state of globalization. This is an essential book for anyone who wants to know how and why neoliberalism has failed to deliver on its promises." -Jeff Faux, Founder and Distinguished Fellow, Economic Policy Institute, Washington, DC, Author, The Global Class War "Vicente Navarro has assembled a comprehensive, tightly argued, and powerfully supported set of contributions that demonstrate the links between neoliberalism, economic inequality, and health outcomes in a convincing and very timely fashion." -Andrew Glyn, Professor of Economics, Oxford University "These incisive essays dissecting neoliberal globalization provide highly significant information along with careful and penetrating analysis, subjecting claims of advocates and critics to the test of judiciously assembled empirical evidence. Moreover, they develop neglected perspectives that should prove invaluable to those seeking to dismantle illusions and deceptive presentations, and to discover the real human consequences of the dominant intellectual and policy paradigms of the past several decades. They are a very welcome contribution to the understanding of some of the most critical issues of the contemporary era. It is an outstanding work and a remarkably important book." -Noam Chomsky, Institute Professor, Retired, MIT "This book provides essential reading for scholars, practitioners, and policymakers wanting to understand the rapid social, health, and economic changes in today's world. It offers an excellent base for reflection on forces driving these changes as well as critical analyses of now widespread interpretations of their causes and consequences." -Walter Korpi, Professor of Social Policy, Swedish Institute for Social Research, Stockholm University "Never in our history have human beings enjoyed such good health; but never have so many persons suffered or died prematurely because of hunger, poverty, and avoidable diseases, and never have inequities in health been so deep and so evident. Vicente Navarro, internationally known as a champion of social justice, has assembled the best contributions from all over the world. This book appears at a time when health for all is being recognized as a fundamental human right and a touchstone of the civilization of any country. All those who work in this field may enjoy reading these essays and find a new stimulus for their activity." -Giovanni Berlinguer, Professor Emeritus, Sciences Faculty, University of Rome Member of the WHO Commission on Social Determinants of Health..."This important book is packed full of evidence that challenges conventional economic wisdom surrounding the issue of globalization. The contributors succeed in their purpose of raising the alarm about the consequences of neoliberal economic policies for human development. The issues the book raises are legitimate concerns for public health: It would be a tragedy if nobody listens", -Margaret Whitehead, The Lancet Volume 371, April 5, 2008."Table of ContentsPART I What Is Neoliberalism? 1 in stock

Book SynopsisWith healthcare making the transition from volume-based reimbursement programs to value-based approaches, understanding performance measurement is vital to optimize payment and quality outcomes. Performance-Based Medicine: Creating the High Performance Network to Optimize Managed Care Relationships guides readers through the maze of definitions and discussions related to value-based purchasing, healthcare delivery, and pricing. It tackles the question of how hospitals, HMOs, physician groups, and employers can arrive at an optimized reimbursement cost and coverage access decision that is attractive to consumers yet fulfills the need for a working margin. The book begins by looking at HMOs and the three key factorsreimbursement, coordination, and performancethat have led toward performance-based contracting. Laying the foundation for clearer communication between physician hospitals and purchasers, the author defines important concepts in the discussion,Trade ReviewBill De Marco draws upon his extensive experience to provide a comprehensive and detailed discussion of performance-driven improvement in healthcare. There is much here of value to policy makers, healthcare system leaders and anyone who wishes to have an in-depth and practical understanding of how to improve healthcare performance and how to align payment to drive better performance. Healthcare continues to be in crisis and in need of further reform, and Bill helps leaders understand the steps they need to take to get that done.—George J. Isham, MD, Chief Health Officer, HealthPartnersReal health reform must solve deep systemic problems in the delivery of medical services, not just the way we pay for care. This latest book by Bill De Marco presents a realistic and practical blueprint for fixing the system at its base. Performance-Based Medicine: Creating the High Performance Network to Optimize Managed Care, not the Affordable Care Act, should be required reading for all who really want to build a new and better health system. —Jeff Bauer, PhD, author of Paradox and Imperatives in Health Care and Statistical Analysis for Decision-Makers in Health CareI feel fortunate to have been given the opportunity to review and endorse Mr. De Marco’s amazing new book, Performance-Based Medicine: Creating the High Performance Network to Optimize Managed Care. This book is particularly remarkable in that it focuses not just on pay-for-performance and performance-based medicine, but truly on the provider side of innovating managed care and embracing pay-for-performance. This provider focus emphasizes physician organizations, and transitioning the delivery of actual healthcare to one that rewards clinical excellence, benchmarked performance attainment, and improving purchaser value. This focus of the managed care enterprise from the physician’s perspective, especially in the contexts of performance benchmarking and Lean engineering, is truly unique just by itself. But Bill De Marco, goes a few steps further. As we’re now seeing with the rise of state healthcare exchanges, accountable care organizations, and attempts by purchasers to narrow their provider markets according to their own metrics, Mr. De Marco has hit a "home run" with his book: He has shown how to re-engineer clinical pathways and disease processes – even with free, public-domain tools – to add value in ways that most emerging ACOs and healthcare exchanges have not yet realized how limited their value will be without moving toward value-based care and performance-based contracting. I was particularly impressed with the findings shared by Bill De Marco in describing Episode Treatment Groups® (ETGs®) and their syntheses of complex and statistically-valid disease management datasets in ways that the healthcare market is just beginning to realize. For example, I can easily see Bill’s descriptions of ETGs as transforming pharmacy benefit managers (PBMs) and Third-Party Administrators (TPAs) away from micro-managing consumption to setting clinical pathways, specific to disease states and their co-morbidities, and therein rewarding benchmark attainments and disease adaptations of specific people. This model is truly the ultimate goal of Paying-for-Performance and transitioning to Performance-Based Metrics, designed to improve clinical care, adherence to healthier lifestyles, optimal disease adaptation, morbidity and mortality prevention, as well as assurance of improved clinical outcomes – all of which purchasers (including CMS) have always wanted to buy from health plans and managed care organizations but have never been able to do so. With Mr. De Marco’s new book, Performance-Based Medicine, these market transformations can finally occur and at the broadest levels within a newly re-emerging U.S. healthcare delivery system. Great job, Bill!—David I. Samuels, author of Managed Health Care in the New Millennium Bill De Marco draws upon his extensive experience to provide a comprehensive and detailed discussion of performance-driven improvement in healthcare. Performance-Based Medicine: Creating the High Performance Network to Optimize Managed Care, not the Affordable Care Act, should be required reading for all who really want to build a new and better health system.—Jeff Bauer, PhD, author of Paradox and Imperatives in Health Care and Statistical Analysis for Decision-Makers in Health CareI feel fortunate to have been given the opportunity to review and endorse Mr. Archelle Georgiou, MD, explains her CARES model, the formula she developed to help family, friends, and thousands of television viewers make smart healthcare decisions thatbalance the bestmedical optionswith individual preferences. Using more than 30 real-life stories and insider tips, she demonstrates how to use this step-by-step guide to access the medical information you need to evaluate your options and make well-informed choices. Whether you are addressing a life-threatening illness, self-managing a minor ailment, selecting a doctor, or buying insurance, Georgiou's roadmap shows you how to be an active participant in your care. Her go to approach describes how to: Identify all treatment options for an illness, including those not mentioned by your doctor.Make tTrade ReviewGeorgiou, an M.D. who served as chief medical officer of United HealthCare, knows her subject matter. To help Americans make good medical choices, she recommends following her own CARES model. She presents its five sensible, empowering steps: understand your condition, know your alternatives, respect your preferences, evaluate your options, and start taking action. A compelling storyteller, Georgiou, who also reports on health news for a Twin Cities television station, includes questions from the public about everything from bad breath to toenail fungus along with her responses. She also uses the power of numbers effectively. Noting, for example, that 40 websites provide 'report cards' on physicians, but only 23 percent of Americans have used them. All doctors are not created equal, Georgiou reminds us, so check them out (HealthGrades.com is a good start). She also points out that only half of patients takes medications as prescribed, and she offers interesting, relevant trivia. The most common reason for an office visit? A cough. What’s the leading cause of bankruptcy? Unpaid medical bills. Georgiou provides readers with sound tools to be better-informed patients. * Booklist *Archelle Georgiou, MD’s book is outstanding. . . . As a medical professional with 39 years of experience dealing with multiple family medical issues, I approached this review with a healthy dose of skepticism and doubt that any book intended for the public could actually help navigate the mess that our health care system has become. I was wrong. This book is an easy read for consumers and clinicians, offers excellent advice, and would make a wonderful textbook for an introductory course on effective health care decision making for a wide variety of audiences. * Family Medicine *You are the expert on you, especially when it comes to your healthcare! If you use Dr. Georgiou's simple process, you will empower yourself to pursue personalized healthcare choices, leading to better care, better cost, and better outcomes. We require this of all other industries...it is time for us to demand it of healthcare! -- Stephen Bonner, Entrepreneur in Residence at Harvard Business School; former CEO of Cancer Treatment Centers of AmericaIn the final analysis, good healthcare starts with the relationship between the patient and the individual care provider – and the knowledge and understanding gained through personal interactions between these two people. Our current system has unfortunately made this relationship more difficult, with the far too frequent result being misinformation, frustration, anxiety, waste and suboptimal outcomes. It is critically important for all of us to understand what is going on with our own health and care, and how to better ourselves through the questions we ask and the knowledge we gain from the caregivers in whom we trust our very lives. -- William W. Book SynopsisIf the furious debate around the state of healthcare in the US has led to any consensus, it's that the system should be delivering better quality for less cost than it does. The truth is that our healthcare system is a sprawling mix of competing interests in which those of the patient are valued least. Too much discussion has devolved to simplistic scapegoating, and too few comprehensive, constructive solutions have been offered. It's time for a fresh vision. In straightforward language, Healthcare at a Turning Point: A Roadmap for Change outlines a new market-based business model that aligns industry financing mechanisms with the goals of prevention, improved quality, and reduced costs. Drawing on more than 25 years of cross-industry consulting experience, the authors: Articulate a market-based vision of the industry Examine past efforts to reduce costs, their failures and their unanticipated consequences Spotlight perverse incentiTrade ReviewThis book is a compelling and thought provoking analysis of critical choices facing health care leaders. It offers a detailed prescription for stakeholders on how to proceed and how to implement genuine accountability at each stage of care management. ... Numerof and Abrams offer a road map for sustainable market reform with remarkable clarity and vision.—Book review by Peter Boland, PhD, appearing in The Journal of Ambulatory Care Management, Vol. 37, No. 1, pp. 38–41Everyone complains about the state of healthcare in this country—patients feel uninformed, mistrustful and lost; clinicians and hospitals are frustrated in their efforts to improve the quality of care while meeting demands for lower costs; employers are burdened with the rising expense of employee health insurance; payers who provide the coverage are pushing for greater evidence before approving reimbursement for new drugs, technologies, procedures and tests; the medical product industry is struggling under the weight of increasing costs, longer development timeframes, shifting regulatory requirements and resistance from the other stakeholders to pay for new products; and politicians and state and federal government agencies are mandating a patchwork of fixes that seem to only make things worse. What we have lacked are tangible solutions that can actually work—until now.Rita Numerof and Michael Abrams in their new book, Health Care at a Turning Point: A Roadmap for Change have offered a fresh look at the current state of healthcare in America, providing a detailed diagnostic assessment of what’s wrong, and importantly offer a comprehensive strategy to fix the problems. They begin with a vision of what a viable future state could look like in 10 years for providing affordable, accessible healthcare that promotes innovation in business models, products and services. Numerof and Abrams identify clearly and objectively the obstacles that must be overcome and gaps that must be closed by each of the important participants in the healthcare ecosystem, and then as the book's title implies, they deliver a roadmap, which is not only plausible, but if followed, will undoubtedly succeed in getting all of us there.—Harlan F. Weisman, M.D., Former Chief Scientific and Technology Officer, Medical Devices and Diagnostics, Johnson & JohnsonNumerof and Abrams have crafted a thoughtful, and often provocative, analysis of the challenges and opportunities facing the healthcare industry today. At its heart is a clarion call for the sector to become much more patient and consumer-centric -- before their existing business models become obsolete.—Paul Howard, Director and Senior Fellow, Center for Medical Progress, Manhattan Institute for Policy ResearchA pair of strategic management consultants intelligently scrutinize America’s healthcare industry. ... The authors’ blueprint for healthcare redesign is impressively conceived yet decidedly business-minded. Chapters defining their proposed remedies emphasize a market-based approach that objects to recent legislation and ideas of federally financed accountable care organizations (ACOs) while promoting models of bundled payment systems in which services are funded through a fixed-price schedule. ... they offer fresh, clear approaches to restructuring the healthcare system and place appropriate emphasis on the fostering of informed, accountable consumers. ... Numerof and Abrams’ ambitious study succeeds in illuminating problems within the multifaceted system ... . An elucidating, relevant discourse on the precarious future of medical care.—Kirkus Reviews, August 2012The authors present thought-provoking ideas for the 'disruptive innovation' needed to uncover pragmatic solutions for our country's healthcare delivery system. ... this suggested blueprint for market-based action is a must-read for all sides of the healthcare reform debate.—Marc Boutin, Executive Vice President & Chief Operating Officer, National Health CouncilEveryone agrees that the American healthcare system needs to change, but there is little agreement about how it should change. Abrams and Numerof paint a picture of how healthcare could look in an ideal future, and offer to manufacturers, payers, and providers a way to get there. Examining the healthcare system today, changes already taking place in attitudes towards innovation and medical education, ... they show how the key players in healthcare must change their thinking to survive. ... The main, and certainly provocative, message: if we move to a value-based system based on patient choice, rather than a cost-based system that assumes one size fits all, the consumer of health care is the ultimate beneficiary. The book will stimulate the thinking of industry across functions, and should be read not only by management but by R&D, Strategic Planning, and Sales. Sections on the need to form partnerships and alliances can change the way industry, payers, and providers think about each other. By examining the four forces they see as dominating our healthcare environment today, namely changes in the regulatory environment, a changing competitive landscape, shifting technology, and changing market expectations, Abrams and Numerof have indeed provided an insightful guide to how the industry needs to change its strategic thinking.—David L. Horwitz, MD, PhD, FACP, Chief Medical Officer, Johnson & Johnson Diabetes Institute Here it is in Healthcare at a Turning Point healthcare must restore control and accountability for intimate, personal decisions to each of us as our own consumer. No one else can define our quality-value-price equation, and discipline our providers ... Such is the economics, humanity, history, and even politic of western innovation…it is time for the Renaissance of those principles, just as articulated here in Healthcare at a Turning Point.—Steve Bonner, President and Chief Executive Officer, Cancer Treatment Centers of America® Numerof and Abrams have conducted a thorough analysis of the implications of market and non-market forces on all players in the health industry -- carriers, care providers, governments, consumers and employers. They have hit the nail on the head that it will take disruptive innovation and a collaborative approach across all these stakeholders to drive solutions to the fundamental problems of high cost and poor quality in health care.I appreciate that Numerof and Abrams focus in on three major principles: 1.) Rethink the customer: It’s really the consumer, the end-user of health care services. The consumer needs to be at the center of the work that we are undertaking. It is imperative that we provide them transparency to cost, quality and value. 2.) Watch out for non-traditional competitors: There are many other industries which have been historically very close to the consumer. The players in our industry are changing, and all of those who serve the consumer need to rapidly evolve ourselves in order to differentiate and succeed. 3.) Segment care providers: We need to apply the discipline of market segmentation to our relationships with care providers. That is, we cannot assume that a one-size-fits-all approach on paying for value or outcome will succeed with each and every physician or hospital. Rather, we must segment our networks and identify which care providers are most likely to succeed in a partnership to reform payment in a particular way, work with them closely and then allow that success to breed other success in the market place.—Elena McFann, VP Network Strategy and Implementation, UnitedHealthcareFor every professional engaged in the financing and delivery of medical care, this book is essential. Like it or not, sparked by the renewed debate over the controversial Affordable Care Act, we find ourselves in the midst of a major transformation in the health care sector of the economy. The convergence of biomedical breakthroughs, new medical technologies, new organizational structures is upon us. But the authors got it right: change will be frustrated, and energies will be diverted into unproductive channels, unless and until we change health care financing and put the patient in the center of the process as the key decision-maker.—Robert E. Moffit PhD, Senior Fellow, Center for Policy Innovation, The Heritage Foundation Congratulations on the publication of Healthcare at a Turning Point. …[You] really hit the ball out of the park with this book. First, you identified the real root problem in our healthcare system [payment], and then provided the key solution, a consumer-driven market model. This book should be the new reference text for healthcare reform and should be read by every healthcare provider, administrator, insurer, product developer, and lawmaker. Thank you for delivering such remarkable insight and strategic guidance to a very complex and polarizing issue in the U.S.—Kurt Weingand, D.V.M., Ph.D., Chief Science Officer, Central Garden & PetHave there ever been three not-so-little words that turned an industry upside down, virtually paralyzed the planning process and left providers and consumers alike adrift as much as "health care reform"? The authors bring a calm, dispassionate tone to the dialogue, underscoring the unsettling nature of transition (a much more palatable word than change) and offer reasonable strategies that override politics. I found the emphasis on collaboration especially comforting and doable.—Ellen Sherberg, Publisher, St. Louis Business Journal ...it is important to have a clear bridge between the issues of today and the possibility of a much brighter future. Without glossing over serious issues…this book describes a way forward for those inside and outside the Healthcare system.—Jeff Thompson, MD, Chief Executive Officer, Gundersen Lutheran Health System I appreciate that Rita and Michael call out the "ultimate objective" in the debate—to improve health outcomes at lower cost. It would seem that all sides can agree we save money when we decrease and change the demand for services. There are two approaches that allow us to do it. Approach 1 is to ration care. Approach 2 is that we incentivize and align consumers AND providers toward a plan for health which the authors rightly refer to as the consumer-driven market model.—Amanda L. Book SynopsisWinner of a 2014 Shingo Research and Professional Publication Award!Reaching America's true potential to deliver and receive exceptional health care will require not only an immense and concerted effort, but a fundamental change of perspective from medical providers, government officials, industry leaders, and patients alike. The Institute of Medicine set forth six primary aims to which every participant in the American healthcare system must contribute: health care must be safe, timely, effective, efficient, equitable, and patient-centered. Presented as the acronym STEEEP, the collective realization of these goals is to reduce the burden of illness, injury, and disability in our nation. Baylor Health Care System is committed to doing its part and has adopted these six aims as its own. Isaacson, MD, MBA, Senior Vice President/Partner, Phillips, DiPisa, & AssociatesAt the heart of quality improvement lies Deming’s idea of fundamental knowledge—the concept that there is a difference between theory and reality. Theory is always an abstraction. Reality is the devil lurking in the details, down in the mud and weeds. Said another way, some talk about it; a relatively few actually do it.You hold in your hands the detailed journey map of a group who has done it, with elegance and flair. It tracks the transformation as theory becomes functional reality. Those seeking to move to the safety of the high ground in an increasingly difficult health care delivery world will find it valuable beyond compare.—Brent James, MD, MStat, Chief Quality Officer, Executive Director, Institute for Health Care Delivery Research, Intermountain HealthcareAll too often, case studies of health care organizations that are actively involved in transforming their delivery of care focus on only a single element of that transformation, as if it were the "magic bullet" that could solve all problems. By contrast, Ballard and his colleagues have set forth a comprehensive look at the multiple initiatives—from simple to quite complex—that are enabling the Baylor Health Care System to play a leading role in transforming health care in the US. Every leader who aspires to achieve safe, timely, effective, efficient, equitable, patient-centered care should read and learn from this book.—William F. Jessee, MD, FACMPE, Senior Vice President and Senior Advisor, Integrated Healthcare StrategiesThis book provides a lens into one organization’s journey to safe, timely, effective, efficient, equitable and patient-centered care. Full of insights, strategies, lessons and resources, this is an important read for any health care organization striving for improvement.—Maulik Joshi, DrPH, President, Health Research & Educational Trust, Senior Vice President of Research, American Hospital AssociationThis book represents a tremendous contribution by a highly respected organization that is fully grounded in the challenges and opportunities of American medicine. Table I.1 alone is worth many times the price of any book—it is essentially a playbook for forward looking, progressive organizations, and one that meets them were they currently are, and takes them forward. David Ballard and Baylor Health Care System’s national leadership reach a new level with this book’s publication.—Thomas H. Lee, MD, Chief Medical Officer, Press Ganey Associates, Inc.In this monograph, Dr. David Ballard and colleagues describe how they have embedded the basic dimensions of the Institute of Medicine framework for quality (i.e., safe, timely, effective, efficient, equitable and patient-centered care), into a practical set of tools used by the Baylor Health Care System—but applicable to any other health care system in the country—into day-to-day operations designed to improve care. They describe the essential components that make their system work, including leadership commitment, use of standardized measurement and quality improvement tools, care coordination across the continuum and the creation of a safety culture. As a health care professional for 35 years, but also a stage IV cancer patient for the last 2 years, I can only hope all health care professionals read and take to heart the advice contained in this book.—Jerod M. Loeb, PhD, Executive Vice President, Division of Healthcare Quality Evaluation, The Joint CommissionThis book will find a permanent place within quick reach of anyone working to implement clinical quality and safety improvement in the real world. It is filled with in-the-trenches wisdom on topics ranging from effective leadership at the top to getting the details right in the front lines of care.—Michael L. Millenson, President, Health Quality Advisors LLC and author of Demanding Medical Excellence: Doctors and Accountability in the Information AgeAchieving STEEEP Health Care describes Baylor Health Care System’s bold experiment to align teams, evidence, systems, and a culture of caring in the unwavering pursuit of higher value health care. The result is a continuously learning health care system poised, along with new partners, to tackle one of the most urgent and significant challenges confronting our nation—optimizing the health of populations while maximizing on increasingly finite resources. The valuable lessons from Baylor Health Care System’s journey accelerate the capacity of health systems in all communities to assume a leadership role in addressing this challenge.—Mary D. Naylor, PhD, FAAN, RN, Marian S. Ware Professor in Gerontology, Director of New Courtland Center for Transitions and Health, University of Pennsylvania School of NursingIn today’s health care world of swirling, conflicting reform pressures and 24/7 information overload, a book has to exceed a very high bar to merit executive attention. David Ballard and colleague’s Achieving STEEEP Health Care clears that bar with truly exceptional room to spare. This is the book that will explain why physician leadership is essential and possible, and how it can effectively align complex organizations and the larger health system culture to the triple aim vision of better care, better health, and lower cost. It represents a major contribution to the science of performance improvement precisely because it explains how it was institutionalized in a large and diverse Dallas, Texas institution in the last 14 years. This is the story of how American health care can reform and sustain itself, if it but will.—Len M. Nichols, PhD, Director, Center for Health Policy Research and Ethics, Professor of Health Policy, College of Health and Human Services, George Mason UniversityAchieving STEEEP Health Care presents not only a roadmap but also a how-to manual for health systems striving to achieve the Triple Aim. Within these pages, those interested in improving America’s health care will learn how to transform the elements of the Institute of Medicine’s ideal state of American health care into action. This book should be read by all health care leaders on their journey to safe, quality health care.—Robert W. Pryor, MD, MBA, CPE, FAAP, FCCM, FCCP, President and Chief Executive Officer, Scott & White HealthcareBaylor Health Care System has been at the forefront of quality in health care for many years. Dr. Ballard and his colleagues, by describing the approach used at Baylor, have created a useful blueprint for others in health care that should speed their journey in quality improvement.—David B. Pryor, MD, Executive Vice President, Ascension Health, President and Chief Executive Officer, Ascension Clinical HoldingsLike a basic scientist who takes a discovery from the bench to the bedside, Ballard and his colleagues at the Baylor Health Care System (BHCS) have taken quality improvement from hypothesis-testing to application in a large delivery system. Their description of the historical context of quality improvement at BHCS and "lessons learned" is especially authentic for health care CEOs who attempt to walk the tightrope of performance improvement without a net. At the VCU Health System, we have used the STEEEP® acronym for years in our own journey toward quality improvement. The experience at BHCS, as described in [this book], is both illuminating and sobering – this is a race without a finish line. Continuous improvement is the only option.—Sheldon M. Retchin, MD, MSPH, Senior Vice President for Health Sciences, Virginia Commonwealth University, Chief Executive Officer, VCU Health SystemThis book, by the Baylor Health Care System enterprise, should be a bellwether mark for our industry, as Baylor is living where health care needs to be in the near future. We must find a way to engage the industry to focus on quality and safety as the way to control costs and to effectively lower hospital expenses; rather than working on quality as separate from costs. This is an example of one system's approach and other models will follow. A great body of work, from a strong national example of high quality care.—Dan Stultz, MD, FACP, FACHE, President and Chief Executive Officer, Texas Hospital AssociationThe Institute of Medicine’s six aims provide a sound framework for those dedicated to transforming the care we deliver to our patients and communities. David Ballard and his colleagues describe a remarkable journey operationalizing this framework across all six dimensions in a complex, growing health system. Their story is both instructive and inspirational. Book SynopsisKnown in health care circles for his ability to fix ailing hospitals, David Shulkin was originally brought into government by President Obama, in an attempt to save the broken Department of Veterans Affairs. When President Trump made him the first VA secretary without military experience-a fact Dr. Shulkin first learned from his television-he was as shocked as anyone.Yet this surprise was trivial compared to what Shulkin encountered as the VA secretary: a team of political appointees devoted to stopping anyone -- including the secretary himself -- who stood in the way of privatising the organisation. In this uninhibited memoir, Shulkin opens up about why the government has long struggled to get good medical care to military veterans and how the current government has stopped even trying. This is a book about the commitment we make to the people who risk their lives for our country, how and why we've failed to honour it and why the new administration is making things worse than they've ever been.
