This compilation discusses how while non-operative treatment of appendicitis has been proposed as an alternative to surgery, appendicectomy remains the mainstay of treatment in most cases and techniques have shifted from open to laparoscopic methods. The current knowledge in pathogenesis, diagnosis, and clinical management of acute appendicitis is also discussed. Next, the authors present an updated review chapter of appendicitis regarding its prevalence and risk factors, particularly how the diagnosis of appendicitis is based on a history of diffuse abdominal pain, which usually starts in the central region of the abdomen and migrates to the lower right quadrant of the abdomen. The residual remnant or "stump" of the appendix after an initial appendectomy may become inflamed causing what is commonly referred to as stump appendicitis. Treatment with antibiotics may be attempted as an alternative to surgery; however, the vast majority of cases require definitive surgical excision of the remnant appendix. The authors propose that keeping this disease in the differential is important in reducing complications associated with delayed diagnosis.

    Book SynopsisGastric cancer is the fourth most common cancer in the world. Most gastric cancers are diagnosed in advanced stages, and the prognosis remains poor. The 5-year survival rate is 40-60% in eastern countries and around 25% in the west. Among advanced gastric cancer cases, peritoneal implantation is one of the most common forms of metastasis and around 14% of cases present peritoneal metastases at initial diagnosis, presenting a survival of 3-6 months. Until the early 1990s it was considered a terminal disease. But today these patients have some opportunities to be treated with curative intent or to prolong their life. Although the prognosis remains poor, there are some treatment options including cytoreductive surgery, neoadjuvant therapy, and some forms of intraperitoneal chemotherapy with or without hyperthermia. These are aggressive treatments, so we must be careful when selecting patients. Imaging and preoperative staging are also crucial in selecting patients for appropriate treatment. The surgical technique is described and a detailed demonstration video is available in the e-book version. New forms of treatment such as PIPAC and new scientific advances and future perspectives are discussed. Three editions of the ASCRS Textbook of Colon and Rectal Surgery have been published and have proved to be extremely valuable for their wealth of general information and knowledge, providing not only background information, but also specifics regarding the more complex situations that surgeons who treat patients with colorectal disease experience on a regular basis. An ASCRS manual was produced in in 2009 and 2014, each accompanying their original textbooks.
