I am about to end my chief year in general surgery residency my copy of the first edition shows the wear of half a dozen total read throughs and probably hundreds of referencings... Simply perfect. The best choice in surgery for trainees! It makes the more difficult surgery areas very easy to understand. I recommend it to all surgeons. One of the best books I have read in my life! Must read for all docs out there. But the moral of the book is that if scientific rigorousness (protocols, guidelines, evidence-based) and common sense are at odds, follow common sense.

Book SynopsisDespite the fact that the incidence of gastric cancer is declining in the Western world, it remains a significant problem with respect to accurate diagnosis and treatment since it has a high mortality rate. In June 1989 an International Conference was held at the University of Rome "La Sapienza" entitled "New Trends in Gastric Cancer: Background and videosurgery". During this meeting background information on the aetiopathogenesis of gastric cancer was presented together with talks and video presentations on the latest advances in the treatment of gastric carcinoma, both from the European and Japanese experience. Because of the poor prognosis of gastric carcinoma there is increasing pressure for early detection. Some of the problems in the early detection of gastric carcinoma are discussed together with methods of surveillance of high-risk subjects. It is generally accepted that the surgical approach to gastric carcinoma should take into account the site and extent of the lesion and there are chapters on new methods for pre and intraoperative staging of the disease which allow a more logical approach to surgery. A comparison between Japanese and Western rule and results was attempted and reasons for the differences were investigated. Next, the authors present an updated review chapter of appendicitis regarding its prevalence and risk factors, particularly how the diagnosis of appendicitis is based on a history of diffuse abdominal pain, which usually starts in the central region of the abdomen and migrates to the lower right quadrant of the abdomen. The residual remnant or "stump" of the appendix after an initial appendectomy may become inflamed causing what is commonly referred to as stump appendicitis. Treatment with antibiotics may be attempted as an alternative to surgery; however, the vast majority of cases require definitive surgical excision of the remnant appendix. The authors propose that keeping this disease in the differential is important in reducing complications associated with delayed diagnosis.

Book SynopsisGastric cancer is the fourth most common cancer in the world. Most gastric cancers are diagnosed in advanced stages, and the prognosis remains poor. The 5-year survival rate is 40-60% in eastern countries and around 25% in the west. Among advanced gastric cancer cases, peritoneal implantation is one of the most common forms of metastasis and around 14% of cases present peritoneal metastases at initial diagnosis, presenting a survival of 3-6 months. Until the early 1990s it was considered a terminal disease. But today these patients have some opportunities to be treated with curative intent or to prolong their life. Although the prognosis remains poor, there are some treatment options including cytoreductive surgery, neoadjuvant therapy, and some forms of intraperitoneal chemotherapy with or without hyperthermia. These are aggressive treatments, so we must be careful when selecting patients. Imaging and preoperative staging are also crucial in selecting patients for appropriate treatment.
