Springer-Verlag New York Inc. Vitreous
Book SynopsisThe vitreous body long has been the most mysterious of all ocular structures, owing perhaps to its seeming simplicity. There have been few concerted, sustained efforts to unravel the mysteries of how vitreous is composed and what role it plays in normal physiology as well as pathology. Over the years, however, many studies have produced important findings concerning vitreous biochemistry, structure, physiology, and pathobiology. Published on the 25th anniversary of Sebag''s first book on Vitreous, this second installment is nearly five times longer than its predecessor, reflecting how much has been learned in the past quarter century. This well-constructed compendium not only addresses the most current scientific knowledge, but also reviews clinical perspectives in a manner that lends richness to the scope of the book. Written by 90 authors, this book has 56 chapters organized into 7 sections: Biochemistry; Anatomy, Development, and Aging; Pathology/Pathobiology; PhysiTrade Review“A striking feature of this textbook is the stunningly vivid color illustrations that have remarkable resolution, in spite of considerably large size. Graphs and tables are featured in both grayscale and color, adding significantly to the ease of comprehension when reading this book. … this book is not only useful to skilled clinicians and expert scientists, but also students, as the book has no competition anywhere in the world.” (Taiji Sakamoto, Graefe's Archive for Clinical and Experimental Ophthalmology, Vol. 257, 2019) Table of ContentsPREFACE: J. John Wiley & Sons Inc Bioceramics: Materials and Applications IV
Book SynopsisThe content from these proceedings comes from a symposium honoring Larry Hench, a pioneer in the field of bioceramics. Prof. Hench has condensed his Sosman Lecture into the keynote paper of this volume. In addition, this proceedings draws together research in the different aspects of bioceramics and illustrates its unifying themes. Apatites and active bone substitute materials are well represented, with extended analyses of processing effects and variations in making these materials more functional. Included in this volume are a series of papers on interactions between ceramics and biological environments with some much needed analysis of why ceramics succeed or don't in vivo. Temple University Press,U.S. Across The Red Line: Stories From The Surgical
Book SynopsisRichard Karl, a doctor and teacher, takes the reader closer than any writer before into the corridors of the hospital, on the surgical table, and into the world of medicine. In these pages we see the tragedies and triumphs of modern medicine: the beauty of surgery done well, and the aftermath of operations that fail to deliver on the hopes of the doctor and patient. We witness the "M&M"-the morbidity and mortality meeting-where doctors scrutinize their own work and mistakes, and the often inevitable outcomes of treatment. Suffused throughout are Karl’s keen observations on the workings of the human body and its immense capacity for healing. "...I celebrate the rich privilege accorded the practicing surgeon. The surgical life is really about bearing witness to the human condition and about respecting the many almost whimsical variations of biology and about the intersection of the two. It is remarkable, really, the way I get to know people so intimately so quickly, and to observe the brave and often noble behavior in them, while I witness the relentless push of biology, the aging and decay, the growth and development, but most especially the healing, both physical and emotional. It is this natural drive of our bodies to repair themselves from all injuries (including the surgeon's wounds) that is the centerpiece of medicine. Without it no surgeon could cut." Written with economy and subtlety, Across the Red Line offers a vivid picture of disease and the miracle of life. It will interest anyone who's ever been on either side of the surgical table.Trade Review"Richard Karl appears to be one of those rare surgeons who cares about as well as for his patients, who speaks to them in English, rather than confusing them with complex medical terms, and who in these difficult days still loves surgery. I admire his ability to describe technical matters in simple language. His book is engagingly conversational, and, like many surgeons, he's a great storyteller."-Joan Cassell, Ph.D., Department of Surgery, Washington Medical School, St. Louis "[Karl is] a natural storyteller and writer... He writes short, pithy sentences and gives a description of an MRI that should speak for everyone who has survived that procedure. Dr. Karl has an acute sense of patients' rights, and members of Congress would do well to read his book before voting on the matter."-Mary McGrory, The Washington Post "From the introduction, readers will be struck with how well this book captures the human side of medicine... It's enlightening to read as Karl eloquently chronicles the sleepless nights spent worrying about patients, the outcomes that were more related to luck than his expertise and the very moving conversations held at the end of life."-Tribune & Times (Tampa, Florida) "In Across the Red Line the author describes much that is satisfying in being an academic surgeon. [Karl] emphasizes the pleasures of teaching, the satisfaction of performing complex surgery, and the great joy in participating in the course of patient recovery...he emphasizes the emotional rewards of surgical work and the privileged interaction with the secrets of bodies and patients."-Dr. Sanford E. Feldman, The Journal of the American Medical Association "...[A] collection of vignettes about the training, practice, and personal life of a busy surgeon...The chapter about the M&M conference is especially revealing and familiar...I recommend this easy-to-read, heartwarming, and familiar book. It will cause you to reflect, remember, and smile."-James O. Menzoian, The Journal of Vascular Surgery "14 tough and gritty essays... If you want to know what it's like to chat with someone one minute and cut into him in the next... this book won't disappoint."-Albert Howard Carter III, St. Petersburg Times "Dr. Karl is a good storyteller. No, make that a very good storyteller... This book will appeal to laypeople as well as to health care practitioners. The book contains no revelations for the surgeon-nor need it contain any. Across the Red Line provides enjoyable reading about things infrequently discussed, and that's reason enough to pick it up."-The Permanente Journal "...spellbinding...The book is entertaining in some parts, but very intense in others, making it difficult to put down. It has value for anyone interested in medical matters or simply a good read, and will be helpful for prospective medical students to decide whether surgery interests them."-KliattTable of ContentsAcknowledgmentsIntroduction: Across the Red Line1. Thieme Medical Publishers Inc Aesthetic Breast Surgery: Concepts & Techniques
Book SynopsisFilled with take-away messages and valuable insights, Aesthetic Breast Surgery by Elizabeth Hall-Findlay, MD, is a masterful text that you will refer to time and again. It presents a personal and time-proven approach to all forms of aesthetic breast surgery. This book is a distillation of the author's extensive experience and provides her unique perspective on problem-solving and patient analysis. Dr. Hall-Findlay has spent years measuring and studying patient results to help other surgeons use "science" to improve the "art" of aesthetic breast surgery. She has developed an approach to analysis and surgery which is simple and consistent-with results that can be achieved by all surgeons. Prepare Yourself for a Master Class in Aesthetic Breast Surgery With personal insights and wisdom pervading every page, this book provides the reader with a master class in aesthetic breast surgery by one of the world's leading experts. Dr. Hall-Findlay is widely known for her popularization of the medial pedicle approach to vertical breast reduction and this is covered in depth. However, her accomplishments in breast surgery extend far beyond this technique. Her ongoing innovations continue to advance the field. In this semi-atlas work, she brings her considerable expertise to bear on the range of aesthetic breast surgery procedures as she describes her individual approach to patient evaluation, operative planning, clinical decision-making, and operative technique. Comprehensive Coverage Divided into nine comprehensive chapters, the book begins with key chapters on analysis, concepts, applied anatomy, and patient selection that lay the foundation for the technical descriptions that follow. These introductory chapters are not to be missed. They offer a unique approach to patient assessment and alert the reader to critical anatomic landmarks that strongly influence the success of operative procedures. Concepts such as the breast footprint (high-breasted and low-breasted patients) are examined to help readers understand the realistic possibilities for what can and cannot be achieved when planning breast surgery. The emphasis is on the decision-making process involved in choosing the right approach for each patient and then planning the procedure with these landmarks in mind. The next seven chapters are devoted to operative technique. A consistent format is used in each chapter with detailed information on patient evaluation, planning, technique, revisions, complications, and outcomes. These pages are liberally illustrated with step-by-step descriptions depicting breast augmentation, vertical breast reduction with the medial pedicle, liposuction-only breast reduction, mastopexy, mastopexy/augmentation, and surgery for breast asymmetries. Information on avoidance and treatment of problems and complications is also highlighted throughout the book. Of particular note is the comprehensive information on nipple necrosis in the reduction chapter; this provides essential information to help surgeons who are faced with decreased blood supply to the nipple. Beautifully Illustrated Beautiful and dramatic color illustrations and numerous preoperative, intraoperative, and postoperative images help the reader visualize the steps of each procedure and appreciate the excellent long-term results that can be achieved. Unique Features Throughout the technique chapters, artwork icons are paired with photographic images to better elucidate the surgical technique being described. Key points are highlighted, and a summary box outlining central issues concludes each chapter. Two DVDs contain operative video to complement the text.Table of ContentsContents PART I Concepts and Principles 1 Breast Analysis and Clinical Concepts 2 Applied Anatomy: Key Concepts for Modern Breast Surgery 3 Surgical Principles 4 Patient Selection, Education, and Surgical Decision Making PART II Technique 5 Breast Augmentation 6 Vertical Breast Reduction 7 Liposuction-Only Breast Reduction 8 Breast Re-reduction 9 Mastopexy 10 Mastopexy-Augmentation 11 Nipple Necrosis 12 Breast Asymmetry Index Video Contents Breast Augmentation Video A-1: Preoperative markings for subglandular breast augmentation Video A-2: Intraoperative review of markings for subglandular breastaugmentation Video A-3: Infiltration for subglandular breast augmentation Video A-4: Pocket dissection for subglandular breast augmentation Video A-5: Implant insertion for subglandular breast augmentation Video A-6: Incision closure for subglandular breast augmentation Video A-7: Wrap-up for subglandular breast augmentation Video B-1: Preoperative markings for subpectoral breast augmentation Video B-2: Intraoperative review of markings for subpectoral breast augmentation Video B-3: Infiltration for subpectoral breast augmentation Video B-4: Pocket dissection for subpectoral breast augmentation Video B-5: Implant insertion for subpectoral breast augmentation Video B-6: Incision closure for subpectoral breast augmentation Video B-7: Wrap-up for subpectoral breast augmentation Vertical Breast Reduction Preoperative markings Intraoperative review of markings Infiltration Pedicle creation Parenchymal resection Pillar closure Closure of dermis Liposuction for contouring Skin closure Wrap-up Breast Re-reduction Video A: Re-reduction of previous vertical breast reduction Video B: Re-reduction of previous inverted-T breast reduction Mastopexy Preoperative markings Intraoperative review of markings Infiltration Deepithelialization Creation of inferior wedge Elevation of pectoralis strip Closure of pillars Closure of dermis Skin closure Wrap-up Mastopexy-Augmentation Preoperative markings Intraoperative review of markings Infiltration Pedicle creation Inferior wedge excision Subpectoral pocket dissection Pillar closure Skin closure Wrap-up

John Wiley & Sons Inc Anxiety Management in Adult Day Surgery: A
Book SynopsisThe book is the first of its kind to specifically outline the psycho-educational nursing interventions required by the anxious, adult patient undergoing elective, ambulatory surgery. Anxiety management is a considerable issue for the majority of surgical patients and has been recognised as such for many decades. However, no formal nursing intervention currently exists to support patients during this acute phase. This book is one of the first to provide strong evidence for the way in which patients can be assisted in the management of their anxiety. Moreover, it provides future direction for surgical nursing intervention in this new era of minimal invasive surgery where patients undergoing elective procedures increasingly require less physical nursing intervention and spend very little time within the acute hospital setting.Table of Contents21st Century: A Time for Change. American Registry of Pathology Benign and Reactive Conditions of Lymph Node and
Book SynopsisOne of the main reasons that this edition was undertaken is that for some years there has been no single, up-to-date book containing extensive information on benign and reactive lymphoid conditions. Although malignant processes garner the majority of attention, it is the benign processes that have an underlying complexity that is often an illustration of the delicate interplay of many components of the active immune system. In diagnosing the pathologic specimen, a basic approach is: 1. To determine if the process is benign or malignant. 2. If malignant, to characterise the origin of the malignancy. 3. To subclassify or identify the process. In some cases, the first part—the question of benign versus malignant—cannot be resolved by histomorphologic evaluation alone. The second part of the approach has become considerably easier with the advent of immunophenotypic analysis. The final issue, that of identification of the pathologic process, may be of greater or lesser importance, depending on the clinical situation. If an exact diagnosis can be made it can have several benefits. In the case of neoplastic processes, it may indicate the type of treatment and the prognosis. In benign processes, there are other important benefits as well. Although some therapies may be instituted in benign conditions, often simply naming a thing can have great benefit to the patient. It is important to realise that even in the case of a purely reactive process, finding a name, a category, a classification, can provide real peace of mind to the patient. As the American Journal of Clinical Pathology declares, “The hundreds of images are the greatest asset of this book. There are extraordinary renditions of such exquisite classic (yet rarely illustrated) entities: `sago’ and `lardaceous’ spleens, `lollipop’ and `explosive’ lymphoid follicles, multinucleated measles cells and hyperplastic mesothelial inclusions, and sundry mystifying diseases, including those known eponymously: Kawasaki, Kikuchi, and Kimura.”For pathologists and hematopathologists specifically, the authors have created a book that will serve for years as a useful guide to the multifaceted world of benign and reactive lesions of the lymphoid system.

Springer Nature Switzerland AG Plastic and Cosmetic Surgery of the Male Breast
Book SynopsisThe thorax has a fundamental role in the aesthetic of the male body: congenital or acquired alterations are experienced as extremely frustrating clinical situations. Most breast surgery texts briefly mention gynecomastia, but no text comprehensively addresses the male breast and thorax, including reconstructive and cosmetic surgery and surgery in transgender patients. This gap is now thoroughly filled by Plastic and Cosmetic Surgery of the Male Breast: after introducing principles of anatomy and aesthetics for the male thorax, the first part focusses on the main congenital malformations and their treatment; the main benign acquired pathologies and their treatment (including an ample section dedicated to gynecomastia), the carcinoma of the male breast, with reference to familiar forms; the treatment of carcinoma and reconstruction of the thorax. Springer Nature Switzerland AG Drug Delivery in Dermatology: Fundamental and
Book SynopsisSkin accounts for approximately 15% of an adult’s total body weight, with a surface of about 2m². It provides an entry and exit barrier to various substances, offers effective protection against harmful substances, microorganisms and ultraviolet radiation, and is also paramount in immunology and body homeostasis, preventing dehydration and loss of essential minerals. The topical route has a number of advantages over oral and parenteral routes: no first-pass metabolism, greater convenience resulting in better treatment compliance, lower side-effect frequency, and prompt dosing cessation if required. However, the stratum corneum allows penetration of only 1 to 5% of substances applied topically, making this application route much less efficient. Lipophilic and small molecules (up to 500 Da) can cross the stratum corneum, since keratinocytes are covered by a lipid matrix, while it is virtually impossible for hydrophilic and large drugs to cross intact, normal skin, which means that passive topical delivery is restricted. To address this, techniques have been developed aimed at optimizing drug skin penetration by means of chemical, mechanical and physical methods, such as adding permeating active agents and using occlusion, iontophoresis, microneedling, lasers and intense pulsed light, and it is now possible to deliver medication to different skin layers or transdermally to the systemic circulation. Drug delivery is a hot topic in dermatology, with a Pubmed search generating 203431 articles. Offering a detailed review of this emerging therapeutic option, including the various methods available, this unique book guides physicians and doctors in selecting the correct technologies, technique and products for each patient. Springer International Publishing AG The Acute Management of Surgical Disease
Book SynopsisEmergency General Surgery has become a cornerstone of Acute Care Surgery over the past decade. Once the scope of community general surgeons, the growing complexity of patients with acute surgical diseases has increasingly driven their care to tertiary referral centers. The aging population confounds this problem further as these patients present with more complex comorbidities, life-threatening physiology, and progressively severe anatomic disease severity. To ensure better outcomes, the practice has evolved to focus on evidence-based practice management guidelines in order to standardize care and optimize outcomes. The purpose of this text is to be a comprehensive volume for patients with acute surgical diseases. There will be three sections, each comprised of multiple chapters. The first section will be a comprehensive analysis of the background of Emergency General Surgery. The goal of this section will be to provide the reader background into the acute surgical practice and to introduce the reader to the important concepts discussed in the remainder of the textbook. The second section will be the largest and will discuss specific acute surgical diseases. Individual chapters will outline the diagnostic approach, current treatment standards, operative approaches, and expected outcomes with an emphasis on practice management guideline implementation into practice. As acute surgical diseases and surgical critical care are integral to each other, the last section will be Critical Care for the Acute Care Surgeon. These topics will focus on the critical care concepts essential to surgeons who care for acute diseases. This comprehensive text is targeted to any surgeon who takes emergency call for any of the listed diseases including community and referral general and vascular surgeons. In addition, this would be an excellent resource for all general surgery residents. Lastly, this would also serve as a resource to intensivists who care for acute surgical patients.Table of ContentsPart 1: Introduction to Fundamental Emergency General Surgery Concepts 1. Springer International Publishing AG Communication Skills for Surgeons: A Contemporary
Book SynopsisHistorically, communication was described as a secondary, or ‘soft skill’ for surgeons. Now, astute communication, both with patients and with colleagues, forms a fundamental element of holistic surgical practice and comprises a core component of the ‘Non-Technical Skills for Surgeons’ that are increasingly recognised in modern surgical practice. Good communication is required during each patient interaction: history taking, explanation, consent, breaking bad news, and managing difficult encounters such as the demanding or angry patient. Good communication with patients improves patient trust, compliance and overall satisfaction, reduces complaints and malpractice claims. High quality communication is also fundamental when interacting with colleagues: in theatre, on the ward, whilst making referrals and organising special tests. In the busy schedules of medical professionals, such communication must be succinct and relevant. Team structures must empower all members to speak up, so as to prevent harm being done. Suboptimal communication is a root cause for the majority of serious adverse events. Furthermore, good communication reduces job stress and enhances satisfaction for the surgeon. Good communication is not an inborn behaviour; it is a learned skill that is based on key principles. Studies have clearly demonstrated that education in communication improves patient outcomes and satisfaction. Several frameworks have been described, to facilitate good communication in certain scenarios: SPIKES for breaking bad news, SBAR for handover, surgical briefs and de-briefs, to name a few. This textbook will be aimed towards medical students, surgical trainees and surgical consultants internationally. It is relevant to every-day practice, examinations and OSCEs, such as medical finals, MRCS, FRCS and international equivalents, and interviews where role play is often featured. Springer International Publishing AG The High-risk Surgical Patient
Book SynopsisIt is well known that certain diseases and patient conditions are associated with increased perioperative risk. The aim of this book is to define and identify the clinical factors that warrant a broader and more detailed assessment of pre-operative surgical risk in difficult and unusual clinical settings. One of the sections is dedicated to the main pathway of peri- and post-surgical critical care based on the patient-specific deterioration risk and associated diseases; here, a panel of selected experts describes the correct patient-oriented pathways for complex or unscheduled surgical operations in order to reduce the operative risk. In addition, the book describes the latest trends in minimally invasive surgical techniques that are associated with peri- and post-operative risk reduction, and provides an overview of recent advances in surgical simulation, focusing on perspectives in surgical research to increase patient safety.Guidance is also provided on extracorporeal membrane oxygenation (ECMO), Left Ventricular Assist Devices (LVADs), and their management in patients requiring emergency surgery. The book will help surgical trainees recognize cases with the highest surgical risk and identify the most common complications at an early stage. Table of Contentssee attachment

Springer International Publishing AG The SAGES Manual Operating Through the Endoscope
Book SynopsisIn 2016 the first edition of the SAGES Manual Operating through the Endoscope was published. At the time, this represented a review of cutting-edge new technologies that were being introduced within the field of flexible endoscopy. Largely based on applications and data up to 2013, this served as a unique resource for clinicians and researchers to learn about this expanding field. Since then, there has been a significant expansion of new technologies and procedures within this discipline that have meaningfully changed this entire sub-specialty. In addition, large numbers of high-quality data have emerged that support more heartily the safety and efficacy of these techniques. Major international societies now routinely include hands-on training and scientific content of this field in meetings and educational efforts, and many trainees and practitioners are looking for resources to improve their knowledge and technical skills sets. For these reasons, an update on this rapidly advancing field seems appropriate at this time, given the enormous amount of new content available. Endoscopic surgical procedures allow for less invasive access for performing increasingly complex procedures. This results in maximal benefit for patients with fewer complications, faster recovery, and less pain. Endoscopy allows access to the gastrointestinal tract without transgressing the abdominal or chest walls, by means of a truly less invasive route. These technologies and skills are being sought out by practitioners and asked for by patients. The proposed textbook is designed to present a comprehensive and state-of the-art approach to the diverse applications of surgical techniques and procedures through the endoscopic platform. Written by experts and emerging specialists in that field, each of these sections will address patient selection, pre-operative considerations, technical conduct of the most common operations, and avoiding complications. A review of the existing literature addressing the particular topic will follow in each section. Extensive endoscopic images and illustrations will make this an interactive text. In addition to updates on most topics in the previous manual, this edition will include exciting new fields of advanced imaging, artificial intelligence and augmented imaging, applications of robotics to endoscopy, and more robust exposure of endoscopic applications for treatment of obesity, a worldwide epidemic. The subject of surgical endoscopy has changed meaningfully since publication of the first edition; now a substantial focus of major journals, scientific meetings, and online platforms. A timely update can capture this broad subject in an accessible manual focused on technologies and data supporting these interventions.Table of ContentsThe Interface Between Therapeutic Gastrointestinal Endoscopy and Endoscopic Gastrointestinal SurgeryEndoscopic Tools: Instruments Operating Platforms for Surgical EndoscopyEndolumenal Electrosurgical EnergyEndoscopic Training – Surgeon and GI ParadigmsAdvanced Training and Certifications in EndoscopyUpper Gastrointestinal Tract BleedingStricture Management: Interventional OptionsUpper Gastrointestinal Tract LeaksEndoscopic Treatment of Gastrointestinal LeaksLower Gastrointestinal Tract BleedingEndoscopic Stenting for Malignant Colorectal ObstructionInterventional Procedures for Inflammatory Bowel DiseaseEnteral Access: Percutaneuous Endoscopic Gastrostomy, Gastrostomy -Jejunostomy, and JejunostomyBarrett's Esophagus Surveillance - WATS, real-time endoscopic microscopyBarrett's Esophagus Treatment – Radiofrequency and other Ablation ModalitiesEndolumenal Gastroesophageal Reflux Disease TherapiesEndomucosal Resection of the Upper GI TractEndoscopic Mucosal Resection Colon and RectumEndoscopic Submucosal Dissection: Upper Gastrointestinal Tract Endoscopic Submucosal Dissection in Colon and RectumPeroral Endoscopic Myotomy (POEM)Intramural Surgery Per-Oral Endoscopic Myotomy for Zenker’s Diverticulum (Z-POEM)Intramural Surgery: Per-Oral PyloromyotomyPer-oral Endoscopic Tunneling for Restoration of the Esophagus (POETRE)Operating through the Endoscope -Endoscopic Full-Thickness ResectionSubmucosal Tunneling Endoscopic Resection of GI Submucosal TumorsThoracic Applications Per Oral Plication of the Esophagus (POPE) Thoracic Applications: Endoscopic Approaches to Benign Esophagorespiratory Fistula ClosureEndoscopic Ultrasound (EUS) Guided Biliary DrainageEUS-directed Transgastric ERCP (EDGE Procedure) for Management of Choledocholithiasis in Post-Gastric Bypass AnatomyAdvanced EUS: Future ApplicationsCholangioscopy Role of Endoscopic Bariatric Therapies in a Comprehensive Multidisciplinary Metabolic and Bariatric ProgramEndoscopic Anatomy of the Bariatric PatientIntragastric Balloon TherapyEndoscopic Liners (Duodenojejunal Bypass Liner, Gastroduodenojejunal Bypass Sleeve)Endoscopic Sleeve GastroplastyNew technologies to treat obesity and related comorbiditiesEndoscopic Bariatric Revisional ProceduresERCP and the Bariatric Patient Hybrid Laparoscopic and Endoscopic Techniques: Upper Gastrointestinal TractHybrid Laparoscopic and Endoscopic Techniques: Colon and RectumNatural Orifice Translumeinal Endoscopic Surgery (NOTES ™)Artificial Intelligence in EndoscopyAdvanced Imaging Through The EndoscopeRobotics in Endoscopy/Tele-EndoscopyFuture Horizons in Flexible Endoscopy
