Book SynopsisThis book is concerned with a trend of reconstructive surgical orthopedics using reparative potential of the organism. It presents a detailed description of combined surgical treatment methods of orthopedic pathologies in children as well as experimental substantiation. The first part of the monograph presents experimental studies concerning the combination of external fixation (Ilizarov method) and flexible intramedullary nailing (inert and bioactive) in limb lengthening. A separate chapter deals with a study of peculiarities in the influence of intramedullary transphyseal nailing on the longitudinal growth in tibia. In the clinical part of the monograph, a detailed description of combined surgical treatments (external fixation and flexible intramedullary nailing) in children of length discrepancy and deformities of limbs is presented. One can find results and peculiarities of management of deformities and shortenings in various etiologies, including congenital malformation, osteogenesis imperfection, and orthopedic complications in calcium and phosphor metabolic imbalance. This catalog also contains a review of conventional bioactive implants used in orthopedic practice. Also, new data of flexible intramedullary nailing application with bioactive coating in different adolescent orthopedic pathologies is presented. Variants and advantages of combined techniques using external and internal fixation in spine surgery are depicted. According to nosological groups, indications and methods of correction are defined. Along with a detailed description of new methods of combined surgical treatment, vast materials illustrating short and long term results are demonstrated.

Book SynopsisThe future of persons with Type 1 Diabetes Mellitus is continuing to "look brighter" with the advent of biomedical technology. Nevertheless, the acute consequences of this disease remains a significant threat. Diabetic ketoacidosis [DKA] remains the most serious challenge for primary care physicians and subspecialists caring for children and young persons with Diabetes Mellitus. Its prevention is possible only among those already diagnosed. Awareness of the etiology and clinical presenting signs among children of all ages is requisite for primary care and emergency department staff [Chapter 1]. While more likely to occur among those with Type 1 diabetes, a significant proportion of children with Type 2 diabetes are also at risk [Chapter 3]. Once diagnosed, the physician team must understand not only what biochemical imbalances must be corrected, but also what life-threatening complications must be avoided [Chapter 1]. The path through that "maze" is not yet fully interpreted; many choices remain as options, but each must be carefully evaluated. Diagnosis among the very young is a challenge because of they lack self-awareness and clarity of signs and symptoms [Chapter 2]. Education of the primary care staff is critical and able to alter the incidence of DKA at the time of diagnosis. Institutions that treat children and adolescents with DKA need an informed Emergency Department as well as Pediatric Intensive Care Facilities. Access to appropriate consultations, such as pediatric endocrinology, cardiology, neurology and neurosurgery are crucial. The initiation of care is critical, and occurs invariably in the Emergency Department [Chapter 4]. Meticulous documentation of fluid composition and volume, insulin administration, vital signs and neurological state of well-being is essential. These will facilitate the transition to the appropriate in-patient service, whose task is to understand the biological impact of fluid rehydration and fluid flux, as well as hypocapnea, hyperglycemia and cranial perfusion [Chapter 5]. The current standards of care all but prevent most of the varied biochemical complications of "appropriate" care for persons with DKA. However, awareness of these possibilities is crucial [Chapter 6]. Yet, one complication remains unexplained and too often unpredicted, and it is devastating to the pediatric patient: cerebral edema. This complication, its prevention and treatment are discussed [Chapter 6]. Type 2 diabetes mellitus is continuing to increase in incidence and prevalence among all ages. Once thought to be "non-ketosis prone" diabetes, now persons undiagnosed or previously diagnosed do obtain DKA. They, however, have other unique medical requirements that must be applied to their treatment [Chapter 3]. Hyperglycemic hyperosmolar state is a complication no longer limited to adults with diabetes or children with Type 2 diabetes mellitus. While the incidence is fortunately lower than that of DKA, its outcome is potentially direr. Appreciation of the unique treatment requirements and guidelines are discussed [Chapter 7]. This monograph provides a comprehensive compendium to those caring for children of all ages with diabetic ketoacidosis, its related disorders and associated conditions.

On the one side, sexuality is embraced and highly visible, manifesting itself in the culture of the music, dance, and the popular Carnival. The other side presents a society that is conservative and inhibiting, one that is heavily influenced by religion. There is a lack of communication regarding sexuality, both within schools and homes, making it very challenging for parents to be open with their children on the subject matter. Let''s face it, many parents do not feel comfortable talking to their children about sexuality for a variety of reasons. In this respect, sexual education offered in schools helps open the discussion. However, it is not a panacea. Parents should not leave this important topic to the schools. Rather, parents should work together with the schools and the information that is disseminated to ensure the values and beliefs of the family, community and society are integrated. Book SynopsisUsing the experience from the United States on positive youth development, researchers in Hong Kong developed the Project P.A.T.H.S., trained the potential program implementers and evaluated the developed programs. Because of the overwhelming success of the project in Hong Kong, the project was piloted in mainland China for several years. This project (supported by the Tin Ka Ping Foundation) was piloted in four cities in East China (Shanghai, Suzhou, Changzhou and Yangzhou) from 2011 to 2014. In this book, we present the training program in mainland China and also a leadership project for university students at The Hong Kong Polytechnic University. The chapters underscore the importance of evaluation, and we hope that the findings will motivate university teachers and others to utilise positive youth development principles within and outside of Hong Kong.

Book SynopsisIn the past there has been much emphasis on child and adolescent developmental problems and while it is perfectly legitimate to look at child and adolescent problems, there are two problems with this approach. First, over-emphasis on problems can reinforce a pathological orientation. As such, children and adolescents may be seen as "problems" or "issues" to be fixed. Second, this approach may overlook talents, potentials and abilities in young people. Because of the limitations of the "deficiency" or "pathological" approach in understanding children and adolescents, there has been a greater interest in positive youth development programs in recent years with a shift from looking at the negative to turning the related programs and approach into something positive and highlighting the positive side of youth development. This has resulted in a focus on talents, strengths, interests and potential in order to facilitate a stronger and better youth development. Instead of viewing children and adolescents as "problems" or "issues" to be resolved, they are regarded as "resources" and "assets" to be developed. In this Yearbook 2015 we cover many aspects of child and adolescent development and health issues from newborn to adolescence and it is our humble wish that the chapters can help stimulate more interest and effort in developing and implementing programs for children and youth to ensure a good and positive development.

Book SynopsisThe goal of the Middle East HBSC study presented in this book was to examine the needs and experience of Jewish and Arab Israeli youth, as well as Arab youth in the Palestinian Authority of the West Bank and Gaza Strip. The study was based on the HBSC (Health Behaviour of School-Aged Children) study that investigates the health and well-being, behaviour and experiences of adolescents around the world from a broad ecological and holistic perspective. The collection and analysis of youth health data help inform and influence appropriate, culturally-relevant policies of health-promotion and health-education as well as intervention strategies both regionally and internationally. HBSC is a cross-national, school-based survey currently encompassing 43 countries in Europe and North America under the auspices of the World Health Organization (WHO) European Regional Office. It is conducted every four years in order to track multi-year trends. HBSC-ME is a collaborative, cross-cultural study similar to the international HBSC. The project was initiated in 1995 by researchers from Bar-Ilan and Al-Quds universities and the Myers-JDC-Brookdale Institute. Data collection was carried out in 2004, and data analysis and dissemination of findings have continued over the last decade.

Breast milk is the optimal form of infant nutrition for the first six to twelve months of life, providing both short-term and long-term advantages for infants and mothers. Among other benefits, breastfeeding provides the infant with protection from a variety of infections, including recurrent otitis media, gastroenteritis, and respiratory tract infections. Chapter One in this book evaluates the correlation between the nutritional status of breastfeeding women, and the cholesterol concentration in their milk. Chapter Two reviews maternal viral infections that can be transmitted to infants directly through breast milk or other mechanisms associated with breastfeeding. Chapter Three discusses the application of hydrostatic high pressure treatments for improving the quality of banked human milk. Imaging procedures have come to overshadow it completely in many cases. In view of the current literature on the EEG in epilepsy, this shift in diagnostic priorities may be partly justified, at least as far as adults are concerned. But on no account does this apply to childhood epilepsy. This volume provides a critical review of the pertinent literature and shows quite clearly that very little attention is paid to major topic in this field.

Book SynopsisIt is well known that the incidence of epilepsies is higher in this age period than in any other time in the life span. The frequent occurrence of seizures refractory to antiepileptic drug treatments is of great concern. On the opposite side of the spectrum, the group of benign epilepsy syndromes presented in this book is clearly more prevalent in childhood than the intractable epilepsies. Neurologists and paediatricians should be familiar with these conditions because accurate diagnosis, and its consequent good prognosis, may save much suffering to the family. Special emphasis, however, is given to the small proportion of children who, in spite of presenting a usually benign disease, show an atypical evolution which hampers their neuropsychological development. It is fundamental to be aware of the possible ill effects of some antiepileptic drugs in these cases. Finally, advancements in the recognition of new -- and not so rare -- epileptic syndromes are presented. All the subjects are supported by extensive experience of the authors based in large series of patients.

The present report is a state-of-the-art critical review, putting together numerous data concerning topics such as epidemiology, pharmacotherapy, the effects of antiepileptic drugs and epilepsy surgery, the relationships between epilepsy and many psychiatric disorders. It also points out the specific areas in need of controlled trials, such as early diagnosis and management.
