Book SynopsisDeconditioning is an integrated physiological response of the body to a reduction in metabolic rate, that is, to a reduction in energy use or in exercise level. Deconditioning and Reconditioning presents selected background information on the many aspects of ground-based and in-flight physiological research and applications. This multi-author volume emphasizes human research programs on prolonged bed rest, immersion and confinement studies simulating conditions of space flight, as well as data from humans in space. Consolidating this research, chapters cover the psycho-sociological aspects of confinement deconditioning, the effects of deconditioning and reconditioning on aerobic power, thermoregulation during deconditioning, and practical applications.Trade Review"This book is a valuable contribution to the field. Each contributing author presents a comprehensive summary of the current state of knowledge regarding their specific topic. … This book contains much useful information about the physiological deconditioning response associated with bed rest, prolonged water immersion, and prolonged space flight and weightlessness. The book is a good starting point for all students interested in understanding where good health begins, i.e., being physically active." - Doody's Review "[T]he volume offers a global experience in the understanding of maintaining optimal physiology. Dr. Greenleaf and his collaborators have created a useful primer and reference text on the physiology of conditioning and deconditioning … The editor and the various authors have made an admirable effort in keeping the reading both engaging and understandable for an audience not necessarily limited to medical personnel. … [T]his work offers a wealth of information for a wide audience or readers: aerospace physiologists, physicians, nurses, and technicians … . ." - Aviation, Space, and Environmental Medicine "The individual chapters are informative… .The text will provide useful graduate-level reading…."-Canadian Journal of Applied Physiology, Roy J. 1 in stock

Book SynopsisWorking in the Dark focuses on the authorsâ understanding of an individualâs pre-suicide state of mind, based on their work with many suicidal individuals, with special attention to those who attempted suicide while in treatment. The book explores how to listen to a suicidal individualâs history, the nature of their primary relationships and their conscious and unconscious communications.Campbell and Hale address the searing emotional impact on relatives, friends and those involved with a person who tries to kill themself, by offering advice on the management of a suicide attempt and how to follow up in the aftermath. Establishing key concepts such as suicide fantasy and pre-suicidal states in adolescents, the book illustrates the pre-suicide state of mind through clinical vignettes, case studies, reflections from those in recovery and discussions with professionals. Working in the Dark will be of interest to social workers, probation officers, nurses, psycholTrade Review‘Its subject notwithstanding – what a brilliant, much-needed, and ultimately hopeful-helpful book this is! The experience, if not the facts, of suicide remain taboo. The authors, based on nearly a century of combined clinical experience, expound its psychic meaning, developmental origins, and therapeutic implications in ways relevant and accessible to all mental health practitioners. Robustly yet flexibly psychoanalytic, they demonstrate the paradox of suicide as ‘an attempt to live at the cost of life itself’; outline the ‘10 danger signals’ of suicide for professionals; and illuminate the underlying phantasies inherent in the suicidal act. Heeded, this book will be a life-saver!’ – Professor Jeremy Holmes, MD, FRCPych, University of Exeter, UK'The authors’ unrivalled knowledge and experience garnered over many years is distilled to present a rich and compelling approach to suicide and therapeutic practice with suicidal people. The exploration of the influential concept of the pre-suicide state makes sense of suicidal motivations, explains the strong, and often seemingly strange counter-transference responses encountered in this work, and why working with suicidal people can draw negative reactions from professionals. Challenging conventional wisdom, Working in the Dark demonstrates that understanding unconscious suicide fantasies underpinning suicidal behaviour is essential to prevent the dangers of repeated suicide attempts.' - Professor Stephen Briggs, University of East London‘Its subject notwithstanding – what a brilliant, much-needed, and ultimately hopeful-helpful book this is! The reason that you really do need to read this book from beginning to end , is because of the help it brings to all of us who in our daily professional lives need to survive the trauma of working with people in the extremes of crises of mind and relationships. This is not just a book for the young inexperienced staff who are usually, and often inappropriately, left to deal with the emotional complexities of those who have tried to harm themselves. • Synthesizing Qualitative Research

    Wiley Synthesizing Qualitative Research

Book SynopsisA considerable number of journal publications using a range of qualitative synthesis approaches has been published. Mary Dixon-Woods and colleagues (Mary Dixon-Woods, Booth, & Sutton, 2007) identified 42 qualitative evidence synthesis papers published in health care literature between 1990 and 2004. An ongoing update by Hannes and Macaitis (2010)identified around 100 additional qualitative or mixed methods syntheses. Yet these generally lack a clear, detailed description of what was done and why (Greenhalgh et al, 2007; McInnes & Wimpenny, 2008). Choices are most commonly influenced by what others have successfully used in the past or by a particular school of thought (Atkins et al, 2008; Britten et al, 2002). This is a substantive limitation. This book brings balance to the options available to researchers, including approaches that have not had a substantial uptake among researchers. It provides arguments for when and why researchers or other parties of interest should opt for a cTable of ContentsPreface. Acknowledgements. List of Contributors. 1 “It looks great but how do I know if it fits?”: an introduction to meta-synthesis research (Barbara L. Paterson). 2 Obstacles to the implementation of Evidence-Based Practice in Belgium: a worked example of meta-aggregation (Karin Hannes and Alan Pearson). 3 Medicine taking for asthma: a worked example of meta-ethnography (Nicky Britten and Catherine Pope). 4 The use of Morphine to treat cancer related pain: a worked example of Critical Interpretive Synthesis (Kate Flemming and Elizabeth McInnes) 5 The Internet in medical education: a worked example of a realist review (Geoff Wong). 6 Mixed methods synthesis: a worked example (Josephine Kavanagh, Fiona Cambell, Angela Harden and James Thomas). 7 Bayesian approaches to the synthesis of qualitative and quantitative research findings (Jamie L. Crandell, Corrine I. Voils and Margarete Sandelowski). 8 Conclusion (Nathan Manning). Index.

Book SynopsisMedicines are the core of treatment in biomedicine, as in many other medical traditions. As material things, they have social as well as pharmacological lives, with people and between people. They are tokens of healing and hope, as well as valuable commodities. Each chapter of this book shows drugs in the hands of particular actors: mothers in Manila, villagers in Burkina Faso, women in the Netherlands, consumers in London, market traders in Cameroon, pharmacists in Mexico, injectionists in Uganda, doctors in Sri Lanka, industrialists in India, and policymakers in Geneva. Each example is used to explore a different problem in the study of medicines, such as social efficacy, experiences of control, skepticism and cultural politics, commodification of health, the attraction of technology and the marketing of images and values. The book shows how anthropologists deal with the sociality of medicines, through their ethnography, their theorizing, and their uses of knowledge.Trade Review'… [this] recent volume in the Cambridge Studies in Medical Anthropology series [is an] important contribution to the study of medicines, not only for medical anthropologists, but for anybody who wants to understand what medicines do and how they do what they do … This book does a good job of presenting some of the research that has been done, and makes a persuasive plea for more anthropological and public health attention to this area.' Journal of Biosocial Science'It is difficult to do justice to a book that is full of so many different ethnographic studies and details. The plethora of ethnographic material is the book's big strength.' Journal of Social AnthropologyTable of ContentsPart I. Introduction: 1. An anthropology of materia medica; Part II. • Elsevier Health Sciences PACES for the MRCP

    1 in stock

Book SynopsisThe introduction of the 5-station PACES (Progressive Assessment of Clinical Examination Skills) exam inspired a great deal of apprehension among candidates. This edition provides complete coverage of all the most common medical cases that will be covered in PACES and similar exams.Trade ReviewComments from candidates' reviews: "The book is an excellently designed companion to PACES preparation. It is portable enough to be carried around with you (unlike some other texts!), and removes some of the fear factor that can hamper preparation. I certainly used this book as my main text whilst on the wards and have recommended it ever since. I feel it is the best PACES text." "The book's format follows the format of the exam and helps to focus and get an idea what one needs to prepare for. It focuses not only on signs related to certain diseases, but also how to recognise and interpret these. As a second step it includes treatment/management to discuss the disease/case in the exam." "One of the greatest assets of this text is the positioning of relevant (and often asked!) questions at the end of this case. Even if studying alone it is possible to see a real patient and then test oneself on the questions practicing for the real exam experience. The close proximity of the answers (directly following the questions) aids readability, as some texts require you to seek out other pages. The competition lacks the questions which help you get good pass marks." "The miscellaneous section of 100 tips is motivating and all-encompassing! I often flicked to the back of the book throughout my preparation. It is also excellent for the night before the exam." "Hall's PACES for MRCP has comprehensive and up-to-date information presented in a very easy-to-read format. It quotes up-to-date studies. The content appears to be more comprehensive than any of the other PACES books out in the market that I have seen." Book SynopsisTranspersonal Hypnosis presents a multidimensional, energy-based view of human awareness that integrates disparate biological, psychological, and spiritual therapeutic techniques. Each of the chapters - all from world-renowned contributors - includes both a historical overview and the theory behind the development of each technique. The authors emphasize experimental studies that examine the validity of using hypnotically accessed transpersonal states of consciousness to heal the body, mind, and spirit. Several clinical vignettes highlight the types of medical and psychological symptoms responsive to these approaches. The emerging field of spiritually-influenced treatments is transforming the practice of medicine.Trade Review"Transpersonal Hypnosis explores an ancient yet modern view of the mind-that it extends beyond the body and is free in space and time. Modern physicians, therapists, and scholars are at long last beginning to take seriously this view of consciousness. The implications of this perspective for our origins and destiny are full of hope and promise, as this book compellingly reveals."-Larry Dossey, M.D., Editor, Alternative Therapies in Health and Medicine"A wonderful collection of information and insights that are vital to our understanding of the nature of life, healing and consciousness."-Bernie Siegel, M.D."The new frontier is on these pages; this is where the intellectual excitement, the creativity, and the healing potential truly lies. • The Trajectory of Discovery

Cambridge University Press The Trajectory of Discovery

    1 in stock

Book SynopsisExploring the forces that determine the rate and direction of medical progress, this book brings together the worlds of scientific policy, economics, sociology, and innovation to describe the medical research landscape. Covers how issues, including incentive structures and lack of novelty in drug development, influence and impede progress.Trade Review'With engaging examples and a surprising breadth of research, The Trajectory of Discovery, brilliantly illuminates how both the rate and trajectory of medical research rests on the incentives built into the scientific system and the social context in which research takes place. Khurana deftly applies a host of classical and new findings from across scientometrics, sociology of science, and the economics of innovation to the medical area and highlight why we (don't) know what we (don't) know. Through examples, theory, and empirical research, the book argues that discovery rests crucially on the aggregated choices of many scientists, whose actions are shaped by the social logic of scientific system - a logic that is not necessarily optimized for this endeavor. It will be a great read for social scientists interested in the intricacies of medical science, or doctors in search for explanations of why science works the way it does.' Emil Bargmann Madsen, Postdoctoral Researcher, Danish Centre for Studies in Research and Research Policy, Aarhus University'The rate and direction of medical progress remain neglected topics for systematic study. Mark P. Khurana's The Trajectory of Discovery shines the spotlight on such issues and makes this what is likely to be the most important book on both biomedicine and science policy this year.' Tyler Cowen, Professor of Economics, George Mason University'A compelling route into the contemporary world of science. With plenty of illuminating examples from biomedical research, Khurana gives us an insider's view into how prizes, important discoveries, easily-accessible tools, funding bubbles, temporary emergencies and crises have long-term effect on what scientists study, and contribute to determine whether science will ultimately progress or sluggish. It shows how patients' groups, philanthropic institutions, corporate lobbies and governments can have a say into what gets prioritized and discusses how some of the current obsessions of science, such as having positive findings, publishing first and cumulate citations pose additional frictions. The final portrait is a world where little room, if any, is left to academic freedom.' Book SynopsisMammalian heme peroxidase enzymes play a critical role in innate immune responses and disease prevention. The formation of potent chemical oxidants is essential to this protective physiologic activity in immunity. Although highly beneficial in the context of immune defense, it is now well established that peroxidases and their overproduction of oxidants contribute to the initiation and persistence of many chronic inflammatory conditions in the cardiovascular, neurologic, respiratory, renal, and gastrointestinal systems. Peroxidasins, a protein family related to heme peroxidases, play a novel role in tissue biogenesis and matrix assembly, which are also attracting attention in different pathological contexts. Given the diverse roles of mammalian heme peroxidases and the breadth and incidence of pathologies associated with these enzymes, there has been significant interest in modulating peroxidase activity as a therapeutic strategy. Book SynopsisA 77-year-old man phones â999â after experiencing severe backache. The paramedics arrive to find him in severe pain, and cold and clammy. While at his house, they note that he smokes and also elicit a history of hypertension. He is tachycardic and hypotensive en route to hospital, and intravenous fluids are started. You have been assigned his initial assessment ...100 Cases in Clinical Pathology and Laboratory Medicine presents 100 clinical scenarios commonly seen by medical students and junior doctors in the emergency department, outpatient clinic, operating theatre or in general practice. A succinct summary of the patient's history, examination and initial investigations is followed by questions on each case, with particular emphasis on the interpretation of the results and in which an understanding of the underlying clinical pathology is central to arriving at the correct diagnosis. The answer includes a detailed discussion on each topic, providing an essential revision aid as well as a practical guide for students and junior doctors, especially those preparing for undergraduate and postgraduate examinations. Making speedy and appropriate clinical decisions, and choosing the best course of action to take as a result, is one of the most important and challenging parts of training to become a doctor. Fully revised and updated for this second edition, these true-to-life cases will teach students and junior doctors to recognize important clinical conditions, to request the appropriate pathological investigation and correctly interpret those results, and, as a result, to develop their diagnostic and management skills.Table of ContentsSection 1- LABORATORY MEDICINECase 1 POLYURIA AND POLYDIPSIACase 2 TRACES OF BLOOD FOUND ON URINE DIPSTICKCase 3 CONFUSION, LETHARGY AND SHORTNESS OF BREATHCase 4 A CHILD WITH FACIAL SWELLINGCase 5 JOINT PAINS AND FATIGUECase 6 ANXIETY, IRRITABILITY AND REDUCED MENSTRUAL FREQUENCYCase 7 SCREENING FOR PROSTATE CANCER?Case 8 A YOUNG WOMAN FOUND UNCONSCIOUS IN BEDCase 9 A MIDDLE-AGED WOMAN WITH JAUNDICECase 10 DRUG OVERDOSECase 11 FATIGUE, WEAKNESS AND CONSTIPATIONCase 12 EXCESSIVE HAIR GROWTH AND INFERTILITYCase 13 FATIGUE AND IRREGULAR PERIODSCase 14 STIFF AND PAINFUL HANDSCase 15 ABDOMINAL PAIN, NAUSEA AND VOMITINGCase 16 FREQUENT THROAT AND CHEST INFECTIONSCase 17 A ROUTINE ‘HEALTH CHECK’ FOR A 55-YEAR-OLD MANCase 18 COLLAPSE IN A PATIENT UNDERGOING CHEMOTHERAPYCase 19 A 10-DAY-OLD BABY WITH AMBIGUOUS GENITALIACase 20 FATIGUE, WEAKNESS AND WEIGHT GAINCase 21 HYPERTENSION IN A YOUNG WOMANCase 22 NAUSEA, VOMITING, WEAKNESS AND PARAETHESIACase 23 ERECTILE DYSFUNCTION IN A YOUNG MANCase 24 A YOUNG MAN WITH ‘HIGH CHOLESTEROL’Case 25 RASH AFTER A BONE MARROW TRANSPLANTCase 26 A FALL ON AN OUTSTRETCHED HANDCase 27 A PATIENT IN FOLLOW-UP AFTER A LIVER TRANSPLANTCase 28 A MAN WITH POORLY CONTROLLED BLOOD PRESSURECase 39 DIZZINESS ON STANDING UPRIGHTCase 30 SUDDEN-ONSET CHEST PAIN IN A TALL WOMANCase 31 CONCERNS ABOUT THE RISK OF BREAST CANCERCase 32 SEIZURE IN A NEONATECase 33 FLU -LIKE ILLNESS AFTER FOREIGN TRAVELSection 2- HISTOPATHOLOGY Case 34 A YOUNG WOMAN WITH ANKLE SWELLING AND SHORTNESS OF BREATHCase 35 CHEST PAIN AND SUDDEN DEATHCase 36 AN ELDERLY MAN WITH SEVERE BACK PAINCase 37 INCREASED BREATHLESSNESS AND SUDDEN DEATHCase 38 A WORSENING COUGH AND HAEMOPTYSISCase 39 A YOUNG MAN WITH SHORTNESS OF BREATH AND HAEMOPTYSISCase 40 SUDDEN ONSET WEAKNESS AND DYSPHASIACase 41 NUMBNESS, WEAKNESS AND CLUMSINESSCase 42 FORGETFULNESS AND DISORIENTATIONCase 43 COFFEE-GROUND VOMITINGCase 44 EPISODIC RIGHT UPPER QUADRANT ABDOMINAL PAINCase 45 CHRONIC DIARRHOEA AND FLATULENCECase 46 A YOUNG MAN WITH BLOODY DIARRHOEA Case 47 ABDOMINAL DISCOMFORT AND GENERALISED WEAKNESSCase 48 ABDOMINAL PAIN IN A PUBLICANCase 49 ABDOMINAL DISTENSION, LOSS OF APPETITE AND MALAISECase 50 BACK PAIN AND FEVER IN A DIABETICCase 51 URINARY PROBLEMS IN AN ELDERLY MANCase 52 A YOUNG MAN WITH A TESTICULAR LUMPCase 53 HEAVY AND PAINFUL MENSTRUAL BLEEDINGCase 54 ABNORXMAL VAGINAL BLEEDINGCase 55 A LUMP IN THE BREASTCase 56 LYMPH NODE ENLARGEMENT IN A YOUNG MANCase 57 AN ITCHY, BLEEDING MOLE ON THE BACKCase 58 BLOOD IN THE URINE IN AN ELDERLY MANCase 59 THYROID DISEASE IN A YOUNG WOMANSection 3 – HAEMATOLOGYCase 60 SHORTNESS OF BREATH AND HEARTBURNCase 61 RAISED WHITE CELL COUNT IN AN ASYMPTOMATIC INDIVIDUALCase 62 WORSENING FATIGUE AND BLEEDING GUMSCase 63 BURNING URINE AND AN ABNORMAL CLOTTING SCREENCase 64 HEADACHES, BLURRED VISION AND ITCHINGCase 65 A PAINFUL ARM AND A LARGE SPLEENCase 66 A RASH ON THE THIGHSCase 67 FATIGUE AND WEIGHT LOSSCase 68 PNEUMONIA WITH ABNORMAL LIVER FUNCTIONCase 69 RECURRENT EPISTAXISCase 70 BRUISING AFTER A PNEUMONIACase 71 HEADACHE AND A RASHCase 72 SHORTNESS OF BREATH AND WEIGHT LOSSCase 73 ANAEMIA RESISTANT TO A BLOOD TRANSFUSIONCase 74 ASYMPTOMATIC ANAEMIA IN THE ELDERLYCase 75 BACK PAIN AND FATIGUECase 76 SHORT OF BREATH AND UNDERWEIGHTCase 77 DETERIORATION AFTER A BLOOD TRANSFUSIONCase 78 HEADACHE AND BLURRED VISIONCase 79 LOWER BACK PAINSection 4 – MICROBIOLOGYCase 80 LEG PAIN AND FEVERSCase 81 FEVER AND COLLAPSECase 82 BURNING SENSATION WHEN PASSING URINECase 83 A BIT SHORT OF BREATHCase 84 UNHAPPY CAMPERCase 85 DIARRHOEA IN AN ELDERLY MANCase 86 A PAINFUL KNEECase 87 A PAIN IN THE EARCase 88 A RED EYECase 89 CHILD WITH A RASHCase 90 PELVIC PAINCase 91 A PAINFUL SWOLLEN JAWCase 92 TRAVELLER’S DIARRHOEACase 93 FEVER IN A RETURNING TRAVELLERCase 94 A SPOT OF CONFUSIONCase 95 CONGENITAL INFECTIONCase 96 A CHRONIC COUGHCase 97 A JAUNDICED VIEWCase 98 GRAM NEGATIVE BACTERAEMIACase 99 A GROIN SWELLINGCase 100 URETHRAL DISCHARGE

Book SynopsisThis innovative book uses a case-based approach to discuss mistakes made in the practice of clinical neuropsychology to form a helpful tool in the training of early career clinicians. By allowing readers space for critical reflection during clinical practice, the book teaches competency in clinical neuropsychology, through the examination of errors as a central part of the learning process. The core of this book is a diverse series of mistakes, each embedded as a patient narrative. Each chapter is based around an example error, typically one that was made, by the authors, as early career clinicians. Early chapters focus on mistakes in neuropsychological assessment, and the diagnostic process. Later chapters focus on errors in rehabilitation and management. Each chapter is framed to reflect the situational context, for example the role of history, what constitutes normal performance, the way that complex tasks rely on foundational skills, or the treatment of patients with dysexTrade Review"Case studies are making a comeback in neuropsychology, especially for teaching. They are memorable and come closer to teaching the skill of neuropsychology than abstract knowledge can. Mistakes are especially memorable, but also shameful and so they are understudied. I salute the courage and wisdom of these thoughtful and skilled authors in modeling for all of us how to learn from our mistakes in assessment and treatment. If, through their example, we can all learn to focus on the lessons of our mistakes, we may be able to improve and accelerateneuropsychological training."Ted Judd, Clinical Neuropsychologist and Cross-Cultural Specialist, Associate Professor Seattle Pacific University"A decidedly human and entertaining account of some of the mistakes we can make while learning and practicing our profession.
