Book SynopsisThe Committee of Neurotraumatology of the World Federation of Neurosurgical Societies decided to elaborate a Glossary of Neuro traumatology, and for this purpose appointed a subgroup of members of this Committee, headed by Dr. E. S. Gurdjian, whose report follows this preface. The main reason for the Committee's decision was the hope that the diffusion of the Glossary through "Acta Neurochirurgica" may bring about a better understanding of neurotraumatological terminology on a worldwide scale. The Glossary should facilitate the classification and the comparison of traumatic lesions of the Nervous System. It will consequently be easier to compile statistics and correctly interpret the informations thus obtained. The very commendable initiative of the Congress of Neurological Surgeons in revising the terminology of head injuries and in publishing this work in Vol. 12 (1966) of "Clinical Neurosurgery" constitutes an important contribution. In 1977 the Glossary now presented endeav ours to complete and bring up to date the definitions of anatomical or physiological modifications in the neurotraumatological field. Book SynopsisThe Second World War gave rise to a previously inconceivable number of peripheral nerve injuries. Only later on did these injuries occasion renewed intensive research in this field of neurosurgery. Among the factors which have promoted this development is the operating microscope with more refined surgical techniques. Of course, surgery of peripheral nerve injuries in peacetime is not to be compared with that in wartime. Only rapid wound healing enables a secondary suture to be performed at a favorable time, after about four weeks. Besides in most cases the defect of substance is not as great as in war injuries, in which the so-called "critical resection length" (Seddon) was the cause of the greatest difficulties and consti tuted the crucial obstacle to good success. The experienced surgeons of the Second World War always knew that the specified dimensions were far too great for the critical resection length. They could only resort to attempting a suture even when there was a great gap be tween the ends of the nerves. Precise information was not available on direct damage to the axis cylinder or alteration due to vascular factors when stretching the various nerves, above all during the later mobilization of joint decompressions. In my opinion, the experimental investigations of the author can make a contribution here. The precisely executed and afterwards carefully analyzed experiments appear very likely to be applicable to human conditions as regards their percentages of total nerve length.Table of ContentsA. Book Synopsishas further broadened controversial though this subject and indeed the results of amygdaloidotomy may be. lt was Jinnai in 1963 who first published his work on the control of intractable epilepsy by interruption of conduction pathways of the epileptic discharge using stereotactic lesions in the field of Forel. This was followed by lesions in the thalamus by Mullen in 1967, and by capsular lesions by Bertrand in 1970 and myself in 1971. In the macroscopic form this was carried out by section of the inter hemispheric cerebral connections by open operation by Vogel in 1969. This has been an important contribution to knowledge of the basic mechanisms of the propagation of the epileptic discharge and to our understanding of brain function but I would look to stereotactic techniques for the greater development potential. There are, of course, limitations. Bilateral lesions of effective size are difficult to achieve without side effects, particularly in respect of speech (notably dysarthria) and yet are essential if intractable epilepsy is to be con trolled in severity and frequency. Increased accuracy of target siting and control of the size of lesion are not the whole answer for inevi tably there are areas where important neuronal circuits are very crowded. But we should not underestimate the contribution of surgery.
