Book SynopsisIn the past two decades the radiological investigation and imaging of paranasal sinus disease has been revolutionised by the introduction of two new techniques: magnetic resonance and computerised tomography. These have made the diagnosis and localisation of neoplastic and non-neoplastic disease a far more exact process and they have completely replaced con ventional pluridirectional tomography as a means of augmenting the initial plain radiographic examination of the paranasal sinuses. The recent introduction into clinical practice of the paramagnetic contrast agent gadolinium DTPA has extended further the range of magnetic resonance in the demonstration of naso-sinus disease. The account which follows is an attempt to summarise present knowledge in this division of otorhinolaryngology imaging. • Pediatric Tonsillectomy: Intracapsular Versus

Nova Science Publishers Inc Pediatric Tonsillectomy: Intracapsular Versus

    1 in stock

Book SynopsisTonsillectomy is one of the most common procedures that otolaryngologists perform. Early in residency, most trainees are heavily influenced by their instructors in tonsillectomy technique and rarely revisit these procedures later in training. In practice, their surgical technique reflects these early experiences. Opinions regarding tonsillectomy are often difficult to change, possibly because of this early exposure. In 2000, Dr Peter Kotai described a radical new approach to tonsil surgery: intracapsular tonsillectomy. He questioned why otolaryngologists perform traditional extracapsular tonsillectomy (which was designed for infectious diseases) for children with obstructive symptomatology. As 19th century German philsopher Arthur Schopenhauer wrote, All truth passes through three stages. First, it is ridiculed. Second, it is violently opposed. Third, it is accepted as being self-evident. Intracapsular tonsillectomy is a prime example of this acceptance process. When Kotai first described it, there were very few otolaryngologists who embraced this procedure. Over time, much data has been collected, and this procedure has gained increasing acceptance. This textbook was undertaken with several ideas in mind. First, it reviews the current understanding of tonsil and adenoid surgery in children, along with when to consider intracapsular tonsillectomy (IT) and when to consider extracapsular tonsillectomy (ET). It reviews the last 16 years of information regarding differences in the surgical techniques. Concepts and data supporting the use of the procedures are presented. Secondly, this text will serve as a surgical atlas for those in the beginning of training, those that have mastered many techniques but may have not had exposure to all options, and those that are looking for even more advanced surgical maneuvers that may enhance their results. This will be a new type of surgical atlas, one using different colors to depict basic and advanced techniques. Each photograph demonstrates specific points of the procedures, with basic portions designated in green and more advanced techniques depicted in blue. Full length videos of each technique are also included. The European Experience of Tonsillotomy is not included in this text because there are no centers in the United States experienced with that procedure. Tonsillotomy is described as removing the tonsil tissue to the level of the anterior tonsillar pillar with a CO2 laser. It does not approach the depth of dissection of intracapsular tonsillectomy and therefore provides difficulty with comparisons. The text begins with Dr. Peter Koltai''s description of how he developed the concept of intracapsular tonsillectomy. Chapter One, Powered Adenoidectomy, introduces many concepts that are also common to tonsillectomy. Chapters Two - Four describe the authors technique of intracapsular tonsillectomy (IT), as well as those of Dr. Robert Ward (New York University), and Dr. John Bent (Albert Einstein Medical College). In Chapter Five, Drs. Goldstein and Gitman from SUNY Downstate review extracapsular tonsillectomy (ET) and include their interpretation of IT. In Chapter Six, Dr. Nina Shapiro, with Sophie Shay from University of California, Los Angeles, reviews her extensive experience with coblation assisted tonsillectomy, both for extracapsular and intracapsular techniques. In Chapter Seven, Drs. Rubinstein and Derkay from Eastern Virginia University discuss the results of both IT and ET from the perspective of treatment of sleep disturbances and compare the complication rates published over the past 16 years. In Chaper Eight, the recent controversy of post-tonsillectomy pain treatment is presented with recommendations from Drs. David Tunkel and Grace Tan of Johns Hopkins University School of Medicine. Finally, in the last chapter, conclusions are drawn and questions for the future are presented.

Book SynopsisThis compilation focuses on Ototoxicity, the adverse pharmacological reaction affecting the inner ear or auditory nerve, characterised by cochlear or vestibular dysfunction. Although ototoxic medications play an essential role in modern medicine, they can cause harm and lead to significant morbidity. The authors discuss aminoglycoside antibiotics, loop diuretics, platinum-based chemotherapeutic agents, macrolide antibiotics and antimalarial medications which are among the 600 frequently used ototoxic drugs. Next, an overview of cisplatin-associated ototoxicity, namely its clinical features, incidence rates, molecular and cellular mechanisms and risk factors is provided for health care practitioners managing patients with cancer. Ototoxicity and neurotoxicity are defined from the perspective of chemical agents, nicotine and cotinine, that individuals experience through cigarette smoking and environmental tobacco smoke exposure. Mechanisms for toxicity are described by review of animal and human studies revealing changes in peripheral and central auditory structures after exposure to nicotine. The mechanisms of ototoxicity induced by aminoglycoside are also discussed, as well as the usage of these drugs for treatment of several disorders such as Meniere disease, and possible protective agents. In closing, the authors highlight the serious issue of aminoglycoside mediated hearing loss in cystic fibrosis patients, and also discuss alternative approaches for avoiding aminoglycoside-induced cochleotoxicity.

Book SynopsisK.J. Lee is a world-renowned otolaryngologist and head and neck surgeon. Born in Malaya in 1940 during World War II, through his indefatigable spirit, talent and hard work he was able to change his life. Traveling to America in 1958 to study at Harvard on a full-tuition scholarship, he went on to make an indelible contribution to his chosen medical specialty. Among his many achievements, his drive and determination to support the next generation of ENT surgeons and to improve the healthcare experience for patients stand out. These vocations led to his publications of over 30 books and the seminal textbook, K.J. Lee's Essential Otolaryngology. It is considered the most widely read text in the field in the world, translated into several languages. This memoir covers his work as a government adviser for healthcare reform, and his founding the nonprofit foundation to encourage caregivers to treat patients as if caregivers are the patients, 'TPIU' (The Patient Is U). In this memoir, he shares his inspiring tale. In his usual spirit of encouraging and supporting the development of junior colleagues, Dr. Lee has chosen to tell his story through three Columbia University writers -- Sally Jee, Clementine Li, and Coco Ruan. Their own education and background have enabled them to fully understand and appreciate his story and communicate it in the words and style of K.J. himself, capturing his very essence and his unmistakable voice. Leeway is a tale of hard work and perseverance, reaching beyond expectations, positivity and forward-thinking, love and family, and above all, success against the odds. A memoir of surgeon K. J. Lee. Book SynopsisDeafness: Current Perspectives and Research Developments presents the theoretical foundations for miscue analysis, its utilization with diverse and bilingual students, and prior research conducted with deaf and hard-of-hearing individuals. The authors focus on a significant challenge to improving the reading achievement of deaf and hard-of-hearing individuals: finding non-biased assessments and compiling test pools of sufficient size in order to identify meaningful patterns. The viability of vibrotactile stimulation as an alternative to complement and foster linguistic development in the profoundly deaf, particularly those with a prelingual age of onset, is examined. The challenges, opportunities, and goals of researchers and clinicians working in the field of deafness and autism spectrum disorder are explored, emphasizing the need for training more culturally and linguistically competent deaf and hearing adults to conduct research and intervention with deaf children. In an effort to improve validity, the authors re-design a previous cross-sectional study to measure resilience when deaf/hard of hearing or blind/low vision students entered high school as well as when they exited, usually four years later, and compare the two data points. The first known project conducted within the United States to evaluate the use of assistive technology for healthcare interactions between student pharmacists and simulated deaf/hard of hearing patients is also discussed.

Book SynopsisMasterful 2D and 3D head, neck, and brain dissections provide unsurpassed insights into head, neck, and brain anatomy An internationally renowned and beloved author, educator, brain anatomist, and neurosurgeon, Professor Albert Rhoton has a special place in medical history. He was revered by students and colleagues and is regarded as one of the fathers of modern microscopic neurosurgery. A driving principle in his anatomy lab was the simple phrase, "Every Second." This was embraced in his philosophy that every second of every day, a patient's life was improved by a surgeon assisted by the anatomic knowledge his lab helped elucidate and distribute. Rhoton's Atlas of Head, Neck, and Brain is the visually exquisite crowning achievement of Dr. Rhoton's brilliant career and unwavering dedication to the intertwined pursuits of surgical anatomy and neurosurgery. The atlas reflects the unparalleled contributions Dr. Rhoton made to the contemporary understanding of neurosurgical anatomy. Dr. Peris-Celda, with the collaboration of an impressive cadre of international multidisciplinary experts, worked closely under Dr. Rhoton's tutelage on this project. This book is the culmination of 5 years of work and experience gleaned from more than 40 years of surgical anatomy research and exquisite dissection techniques performed in Dr. Rhoton's laboratory. Special Features Each anatomic dissection meticulously labeled with English and Latin descriptors for easy cross referencing with other resources. Multiple views of the most complex regions of the head, neck, and brain provide a deeper understanding of anatomy. More than 600 anatomical images systematically organized in four major sections: Osteology of the Head and Neck; Face and Neck; Ear, Nose, Pharynx, Larynx, and Orbit; and Neuroanatomy and Cranial Base. Superb 2D images presented in a large printed format to optimize the viewing experience. 3D digital images fully realize the beauty of the dissections and enhance the learning process. Specimens injected with colored silicone provide better visualization of arteries and veins. Book SynopsisHearing loss is a chronic and often lifelong disability that can cause profound damage to the development of speech, language, and cognitive skills in children, especially when commencing before the critical period of language development in infancy. That damage, in turn, affects the child''s progress in school and, later, his or her ability to obtain, keep, and perform an occupation. In addition to its effects on individuals, data from several countries indicate that hearing loss has huge economic consequences. Thus, prevention of hearing loss, as well as being socially and morally justifiable, makes substantial economic sense as well.
