Nova Science Publishers Inc Medicaid Spending by States: Variations &
Book SynopsisMedicaid is a means-tested entitlement program that finances the delivery of primary and acute medical services as well as long-term care. Medicaid is jointly funded by the federal government and the states. The federal government''s share of a state''s expenditures is called the federal medical assistance percentage (FMAP) rate. The remainder is referred to as the nonfederal share, or state share. Estimates of Medicaid spending developed from Centers for Medicare & Medicaid Services (CMS) data sources suggest wide variation among states in Medicaid spending per enrollee, overall and for each of four main eligibility groupschildren, adults, disabled, and aged. This book examines Medicaid spending per enrollee by state; selected factors that influence Medicaid spending per enrollee, by state; and how states account for factors that influence expected per-enrollee spending when setting rates for Medicaid managed care plans.

Nova Science Publishers Inc Managerial Solutions to Hospitals' Challenges
Book SynopsisIn the attempt to provide a global vision of the challenges faced by hospitals, this book analyses the dense network of rules and expectations coming from institutions, patients and professionals in which healthcare organisations are embedded. The numerous changes that have occurred within a large part of the healthcare systems during the last 20 years, have determined important consequences, which have been promptly faced by managers. Uncertainty, rules, and innovations are just some of the main ingredients of the challenges to be tackled, whereas mimicry, legitimacy and diffusion of managerial fashions are some of the core strategies implemented to face them. Hospitals survival depends on the ability to intercept on time and to accomplish quickly the external expectations. The role of managers is to create a workplace in which changes are possible. Individual and organisational work value congruence can facilitate it by improving the quality of communication, the degree of trust in hospitals'' missions, and physicians'' and nurses'' knowledge of the processes. Hospital challenges reach up to the single physician, often asking him/her to play new and conflicting roles. The "new" physician needs to balance his/her time to accomplish ambiguous expectations, to fulfill heterogeneous tasks, and to compete for scarce resources in order to embrace the changes. In this vein, physicians'' negotiation skills assume a strategic role for the successful running of hospitals.

Nova Science Publishers Inc Driving Hospitals Towards Performance: Practical
Book SynopsisHospitals, just like orchestras, are made up of many "instruments" which play their notes on their own, yet within a broader symphony. If even one instrument fails in doing its part, or if the orchestra director leaves even one of them behind, the overall symphony will just not sound right and the final performance will be ruined. This book is aimed at providing practical guidance to hospital managers who are called to implement complex organizational transformations. In doing so, they must take into account a high number of managerial tools and dynamics which should not be overlooked. Failing to do so may negatively affect their long-term success in reaching strategic objectives. Hospitals do not have violins, drums, and trumpets, but they do have departments, clinical wards, clinical pathways, and functional units. Moreover, their professionals must carry out different activities such as assisting patients, reporting their actions, communicating and, more and more frequently, managing parts of the hospitals themselves. Not only physicians are in charge of these aspects. Nurses and other clinical and non-clinical professionals are co-responsible, facing frequent changes in the number and type of activities they must perform. In this context, hospital managers must design organizational charts and implement them by distributing resources and responsibilities, by setting and monitoring objectives, and by assessing the performance of units. They must motivate their staff and ensure an effective use of technology. Yet, although healthcare management is widely studied worldwide, it is rare to find contributions with such a broad and holistic point of view. Although each aspect and item of hospital management is key in reaching organizational success, the risk of overlooking one or more of them is high. Specifically, if management fails in directing all its tools towards its objectives, the chances of reaching outstanding performance are poor. Managerial tools must be used in an integrated and coherent way as multiple components of a single managerial strategy.Table of ContentsAcknowledgments; Introduction; Understanding the Role of Hospitals within Healthcare Systems; How are Hospitals Changing? TFM Publishing Ltd Getting to Grips with Clinical Governance
Book SynopsisThe launch of clinical governance in the late 1990s was accompanied by a measure of excitement although, at that stage, it was impossible to determine whether this was really to be the start of a quality-driven revolution in healthcare. Since its inception, much has been done to turn the concepts behind clinical governance into practical reality. Have these efforts been successful? Many would argue that the jury is still out. The authors have approached the topic from the diverse backgrounds offered by urological surgery, general practice and public health medicine. The result is a commentary on the evolution of clinical governance that has the firm anchors of practicality and realism. The book looks at clinical governance from the perspective of the practising clinician and explains what clinical governance has to offer them. Doctors, nurses, therapists and managers should all find something of interest and practical value within these pages.

John Libbey Eurotext Hospitals & the Nursing Profession: Lessons from
Book SynopsisHospital systems throughout the developed world are undergoing waves of reform which seek to address multiple challenges of intensifying acuity, such as population ageing, technological advance, heightened expectations on the part of increasingly informed patients, the reduction of public spending deficits and the specialisation of staff, especially nurses, as well as the difficulty in establishing appropriate incentives for change and improved performance. Within such a context, the purpose of this book is to analyse the interaction between the nursing professions and hospital institutions in France and Japan, taking as its starting point the conviction that comparative analysis of empirical reality in each of these countries will provide new insights into the transformations currently taking place. To that end, the material in this study has been contributed by an international, interdisciplinary team of experts, combining economic, sociological, political and historical perspectives, which are brought to bear upon evidence from original research carried out in both countries. The findings reveal that the relationship between the nursing profession and hospital structures in Japan is characterised by the predominance of a domestic logic, rooted in dependence upon the institution and the promotion of supposedly feminine qualities, in sharp contrast with the French situation, where industrial and professional logics prevail, entailing specialisation, independent initiative and increasing workloads. From this perspective, the future development of the nursing profession in Japan is inextricably linked to the forms taken by the process of women''s emancipation, whereas in France, it is the evolution of hospital structures, of the position of nurses in the healthcare system and of the division of labour within the world of medicine which emerge as the determining factors. In order to highlight French and Japanese particularities for the Anglophone reader, the book also features numerous socio-historical points of comparison with developments in the United Kingdom.

Oxford University Press Systems Science and Population Health
Book SynopsisPopulation health is complex and multileveled, encompassing dynamic interactions between cells, societies, and everything in between. Our typical approach to studying population health, however, remains oriented around a reductionist approach to conceptualizing, empirically analyzing, and intervening to improve population health. The trouble is that interventions founded on simplifying a complex world often do not work, sometimes yielding failure or, even worse, harm. The difficult truth is that silver bullet health science often fails, and understanding these failures can help us improve our approach to health science, and, ultimately, population health.SYSTEMS SCIENCE AND POPULATION HEALTH employs principles from across a range of sciences to refine the way we understand population health. By augmenting traditional analytic approaches with new tools like machine learning, microsimulation, and social network analysis, population health can be studied as a dynamic and complex system. This allows us to understand population health as a complex whole, offering new insights and perspectives that stand to improve the health of the public. This text offers the first educational and practical guide to this forward-thinking approach.Comprising 17 chapters from the vanguard of population health, epidemiology, computer science, and medicine, this book offers a three-part introduction to the subject:DT An intellectual and conceptual history of systems science as it intersects with population healthDT Concise, introductory overviews of important and emerging methodological tools in systems science, including systems dynamics, agent-based modeling, microsimulation, social network analysis, and machine-learning-all with relevant examples drawn from population health literatureDT An exploration of future implications for systems science and its applications to our understanding of population health issuesFor researchers, students, and practitioners, SYSTEMS SCIENCE AND POPULATION HEALTH redefines many of the foundational elements of how we understand population health. Nearly a decade after the great recession of 2008, homelessness rates are once again rising across the United States, with the number of persons experiencing homelessness surpassing the number of individuals suffering from opioid use disorders annually. Homelessness presents serious adverse consequences for physical and mental health, and ultimately worsens health disparities for already at-risk low-income and minority populations. While some state-level policies have been implemented to address homelessness, these services are often not designed to target chronic homelessness and subsequently fail in policy implementation by engendering barriers to local homeless policy solutions. A must read for public policy and public health scholars. * Patricia Strach, University of Albany *Charley Willison has written an urgently important book that explores and explains critical facets of the politics of homelessness policy. Marshalling an impressive range of qualitative and quantitative data, Ungoverned and Out of Sight highlights homelessness as a vital public health problem while advancing our understanding of how local policy processes shape homelessness governance. With remarkable clarity, devastating depth, and compelling breadth, Willison offers knowledge that is crucial for addressing homelessness as a structural political problem. This book is a must read for political scientists, public health scholars, policy makers, and everyone who is part of the decentralized complex of actors who play a role in the systems that structure homelessness. * Jamila Michener, Cornell Center for Health Equity *Homelessness is viscerally experienced at the local level. Local governments play a crucial role in shaping factors that affect homelessness, like housing development and police behavior, yet direct responses to homelessness exist largely outside of local government. Why would this be? Using novel quantitative data along with in-depth qualitative evidence from three divergent cases, Charley Willison identifies four influential actors in homelessness response and explains how this history of devolution, combined with a lack of coordination among powerful players, contributed to homelessness policy responses coming up short. * Jessica Trounstine, University of California, Merced *Table of ContentsTable of Contents Acknowledgements Chapter 1: America's Homelessness Crisis Chapter 2: Homeless Politics in the United States: Theories of the Ungoverned and Unwanted Chapter 3: Seeking Deeper Explanations: Case Selection and Qualitative Analysis Chapter 4: Measuring Municipal Participation in Homeless Governance: A National Perspective Chapter 5: The Integrated State: San Francisco and Challenges for Policy Implementation Chapter 6: The Fragmented State: Atlanta and the Challenges of Coordination Chapter 7: The Delegated State: Shreveport and Challenges of Authority and Capacity Chapter 8: A Way Forward Appendix A: Dataset Overview and Codebook Appendix B: fsQCA Deduction Procedure Appendix C: Interviewee Demographics by Cases Appendix D: Qualitative Interview Consent Protocol Appendix E: Qualitative Interview Questionnaire Appendix F: Codebook for Qualitative Interviews Appendix G: Process Tracing Analytic Approach and for Archival Documents Notes References Index
