Book Synopsis This comprehensive, multi-authored book covers all aspects of surgery on obese patients in emergency conditions. Obesity is a metabolic disease affecting a high percentage of world population.. It involves marked anthropometric changes, affecting surgical practice and altering patients’ ability to react to surgical stress. The prevalent comorbidities also affect the rate of complications and mortality after surgery. The obesity paradox, the ability of obese patients to survive emergency operations in spite of an increased risk of complications, is an effect of the widespread development of “Obesity Science”. This volume discusses this science, examining the frailty of the obese patients and the main comorbidities that affect clinical practice, as well as the most frequent emergency situations after trauma, inflammatory diseases and the complications of bariatric surgery. With contributions from leading experts, it provides clinicians with detailed and updated information for better practice in this emerging field of surgery.Trade ReviewTable of ContentsForeword.- 1 The frailty of the obese patients and the effect of surgical stress.- 2 Emergency anaesthesia in the obese patient.- 3 Perioperative and intensive care management of the obese surgical patient.- 4 Postoperative complications in ICU.- 5 The ERAS protocol.- I CLINICAL SETTINGS IN OBESE PATIENTS.- 6 Trauma and burns in obese patients.- 7 Perforations of the upper gastrointestinal tract.- 8 Acute Diverticolitis.- 9 Acute appendicitis.- 10 Pancreatic and biliary emergencies.- 11 Bowel obstruction.- 12 Abdominal compartment syndrome.- II CLINICAL SETTING AFTER BARIATRIC SURGERY.- 13 The evolution and development of bariatric surgery.- 14 Metabolic complications after b.s.: the faulse acute Abdomen.- 15 Complications after bariatric surgery.: a general overview.- 16 Complications of BIB therapy.- 17 Complications of restrictive operation.- 18 Upper G-I bleeding after bariatric surgery.- 19 Peptic ulcer after bariatric surgery.- 20 Bowel obstruction after bariatric surgery.- 21 Acute peritonitis and abscess after bariatric surgery.- 22 Anastomotic leakage after bariatric Surgery: from prevention to treatment.- 23 Gallstones and related complications, cholecistitis and Cholangitis after bariatric surgery.- 24 Emergencies After bariatric surgery: the role of flexible endoscopy and interventional radiology.- 25 Extreme remedial surgery for b.s. acute complications.- 26 Accreditation of the surgeon to emergency bariatric surgery.- 27 Litigation after bariatric surgery.

Springer Nature Switzerland AG Social Emergency Medicine: Principles and

    15 in stock

Book SynopsisSocial Emergency Medicine incorporates consideration of patients’ social needs and larger structural context into the practice of emergency care and related research. In doing so, the field explores the interplay of social forces and the emergency care system as they influence the well-being of individual patients and the broader community. Social Emergency Medicine recognizes that in many cases typical fixes such as prescriptions and follow-up visits are not enough; the need for housing, a safe neighborhood in which to exercise or socialize, or access to healthy food must be identified and addressed before patients’ health can be restored. While interest in the subject is growing rapidly, the field of Social Emergency Medicine to date has lacked a foundational text – a gap this book seeks to fill. This book includes foundational chapters on the salience of racism, gender and gender identity, immigration, language and literacy, and neighborhood to emergency care. It provides readers with knowledge and resources to assess and assist emergency department patients with social needs including but not limited to housing, food, economic opportunity, and transportation. Core emergency medicine content areas including violence and substance use are covered uniquely through the lens of Social Emergency Medicine. Each chapter provides background and research, implications and recommendations for practice from the bedside to the hospital/healthcare system and beyond, and case studies for teaching. Book SynopsisThis is the second edition of the book originally published under the title Medical Emergencies Caused by Aquatic Animals: A Zoological and Clinical Guide. Including updated chapters, new content and additional references, it discusses follow-up with patients, describes diseases that are not emergencies and explains procedures that can take place at health stations and outpatient centers, focusing on clinical and biological aspects relevant to researchers and practitioners alike. The chapter on Invertebrate Aquatic Animals presents facts and advances that were left out of the first edition. In addition, it includes improved images. The subtopic on Cnidaria presents recent data on outbreaks and new species identification in unprecedented areas, with a timely discussion on first aid treatment. The chapter on Injuries by Vertebrate Aquatic Animals has been improved, based on continued work with bathers and fishermen. Featuring numerous images and representative clinical cases, it explores the most significant injuries caused by fish around the globe. The chapter on Ingestion of Aquatic Venomous Animals: Toxinology, Clinical Aspects, and Treatment discusses outbreaks of intoxication diseases from eating fish and seafood, drawing on reliable records of serial cases of Haff's syndrome, scombroidism and ciguatera. Further, it examines the indiscriminate consumption of aquatic animals, which the author argues present the same (or even greater) risks of poisoning and infections as terrestrial animals. Lastly, since there is a general lack of awareness of the risk of infections in aquatic environments, the chapter on Bacterial and Fungal Infections in Aquatic Environments describes the symptoms and treatments and highlights preventive measures.Table of Contents1. Book SynopsisThe concept for this book evolved over many years of intense clinical practice during which a wide variety of vascular complications were encountered as a result of other surgical or interventional procedures. In this book, the authors describe cohesive efforts by experts in the field as they commit to advancing health care quality by all professionals. It should be noted at the outset that the book does not discuss various non-vascular or systemic complications of patients undergoing vascular interventions such as myocardial infarction, renal or respiratory failure. Instead, the content is divided into four separate sections. The material begins with an introduction to vascular anatomy and function. There follows a section addressing specific complications of the vascular system that can occur in any patient, sometimes even without intervention. The third section addresses complications specific to interventions and surgical procedures. The final section is dedicated to the delayed and long-term sequelae attributable to the most commonly encountered vascular complications.Vascular Complications of Surgery and Intervention: A Practical Guide is targeted towards dedicated health care professionals managing patients affected by inadvertent but potentially disastrous vascular complications and for those practitioners performing interventions either directly upon the vascular system or procedures that have potential for collateral impact.Table of ContentsPart 1. Book SynopsisThis comprehensive book addresses the acute emergencies in neuromuscular disease from a novel perspective, focusing on clinical management and treatment of these disorders rather than the more traditional neuropathology, neurogenetics, and neurophysiology approaches. The book fills the gap in guidelines by providing an evidence-based guidance for the clinical adult or pediatric neurologist confronted by an acutely ill patient in a potentially life-threatening situation. The book is structured according to pathophysiological principles, but each chapter is strictly organised around case vignettes that emphasize clinical relevance allowing the non-specialist to access complex scientific backgrounds. The electronic supplementary material and multimedia are provided as an integral tool to facilitate accessibility and understanding of a notoriously difficult field of neurology. Finally, the role of patients, who are increasingly involved in many management decisions, is highlighted in a chapter on ethical issues discussing withholding or withdrawing life-sustaining therapy, patient end-of-life values, wishes and preferences, and on advanced directives. The publication is aimed equally at the non-specialist neurologist in frontline clinical practice confronted by a rare disorder needing immediate management decisions, at the clinical specialist referring for guidance in specific management aspects, and at the specialist researcher keen to maintain clinic relevance.Table of ContentsDiagnostic assessment in acute neuromuscular disease.- The differential diagnosis of the patient with acute neuromuscular disease and acute flaccid paralysis.- The clinical assessment of the neuromuscular emergency.- Clinical assessment in the child with an acute neuromuscular illness.- Antibody testing and immunological assessment in acute neuromuscular disorders.- Testing of the patient with exercise intolerance and acute rhabdomyolysis.- Electrophysiological assessment of acute neuromuscular disorders and EMG in the ICU.- Assessment of autonomic function.- Nerve and muscle biopsies in the acutely ill patient and histological assessment.- Imaging of the acutely weak patient.- Molecular and genetic testing algorithms in the acutely ill neuromuscular patient.- The neurology of acutely failing respiratory mechanics and its assessment.- Categories of acute neuromuscular disorders.- Poliomyelitis and acute infectious disorders of the motor neurone.- The emergency presentation of Amyotrophic lateral sclerosis and atypical motor neurone disorders (text; tables/flow charts).- Acute presentation of inherited and metabolic neuropathies.- Acute inflammatory neuropathy – Guillain Barre syndrome and its differential diagnoses.- Myasthenia Gravis and myasthenic emergencies.- Botulism, and the Lambert-Eaton myasthenic syndrome.- Congenital myasthenic syndromes and their emergency presentation- Channelopathies and disorders of skeletal muscle membrane excitability.- Rhabdomyolysis and myoglobinuria.- Acute emergencies in childhood muscular dystrophies.- Emergencies in advanced muscular dystrophy.- Acute cardiac failure, cardiomyopathy, and cardiac dysrhythmia in muscle disease.- Congenital myopathies and acute emergencies.- Metabolic myopathies and their acute presentation.- Mitochondrial myopathies: acute presentation and emergency management.- Endocrine and toxic myopathies.- Acute inflammatory and paraneoplastic myopathies.- Infectious and granulomatous myopathies.- C. Book SynopsisIt is well known that certain diseases and patient conditions are associated with increased perioperative risk. The aim of this book is to define and identify the clinical factors that warrant a broader and more detailed assessment of pre-operative surgical risk in difficult and unusual clinical settings. One of the sections is dedicated to the main pathway of peri- and post-surgical critical care based on the patient-specific deterioration risk and associated diseases; here, a panel of selected experts describes the correct patient-oriented pathways for complex or unscheduled surgical operations in order to reduce the operative risk. In addition, the book describes the latest trends in minimally invasive surgical techniques that are associated with peri- and post-operative risk reduction, and provides an overview of recent advances in surgical simulation, focusing on perspectives in surgical research to increase patient safety.Guidance is also provided on extracorporeal membrane oxygenation (ECMO), Left Ventricular Assist Devices (LVADs), and their management in patients requiring emergency surgery. The book will help surgical trainees recognize cases with the highest surgical risk and identify the most common complications at an early stage. Table of Contentssee attachment

Book SynopsisThis book aims to highlight the importance of the development of health conditions and demand for the application of noninvasive mechanical ventilation (NIMV) outside the intensive care units (ICUs); the diversification of possible scenarios outside the ICUs; the need to establish references that consolidate this phenomenon and the healthcare organizations models. In the last decades the extension of the use of NIMV outside of the ICUs has led to the generation of protocols and to the creation of new in-hospital care models. In this field, the main determining factors are a better knowledge of technique, technological advancement, better monitoring capacity, the creation of multidisciplinary teams adequately trained in their application, and social and health events that have overloaded ICUs. All these elements have promoted the creation of these NIMV units outside ICUs. This new reality entails the need for clarification of concepts, recommendations, and analysis of how to plan NIMV.Although the literature that clearly determine the indications and aids on the use of NIMV is considerable, this volume, pointing out the diversity of different healthcare models to define how to organize NIMV outside the ICUs, shed a light and bring a clear benefit to the scientific community involved. The book is structured in eleven main sections analyzing the epidemiology and trends for NIMV healthcare models and determining factors for these models outside ICUs.The originality of the work, its clear clinical-practical impact and the multidisciplinary approach given by all healthcare professionals involved (intensivists, pneumologists, internal medicine and emergency medicine specialists, geriatricians, chest respiratory therapists, etc.) is very relevant for the thoroughness of the book. • Modern Anesthetics

Springer-Verlag Berlin and Heidelberg GmbH & Co. KG Modern Anesthetics

    3 in stock

Book SynopsisSome important constraints of anesthesia must be taken into consideration when the pharmacological properties of modern anesthetics are discussed. The most imp- tant of these could be that the target effect be achieved preferably within seconds, at most within a few minutes. Similarly, offset of drug action should be achieved within minutes rather hours. The target effects, such as unconsciousness, are pot- tially life-threatening, as are the side effects of modern anesthetics, such as respi- tory and cardiovascular depression. Finally, the patient’s purposeful responses are not available to guide drug dosage, because, either the patient is unconscious, or more problematically, the patient is aware but unable to communicate pain because of neuromuscular blockade. These constraints were already recognised 35 years ago, when in 1972 Volume XXX entitled “Modern Inhalation Anesthetics” appeared in this Handbook Series. The present volume is meant as a follow up and extension of that volume. At the beginning of the 1970’s anesthesia was commonly delivered by inhalation, with only very few exceptions. The clinical understanding of that time considered anesthesia as a unique state achieved by any of the inhalation anesthetics, in- pendent of their specific molecular structure. “The very mechanism of anesthetic action at the biophase” was discussed within the theoretical framework of the “u- tary theory of narcosis”.Table of ContentsMolecular Mechanisms of Anesthetic Action.- The Site of Anesthetic Action.- Inhibitory Ligand-Gated Ion Channels as Substrates for General Anesthetic Actions.- Actions of Anesthetics on Excitatory Transmitter-Gated Channels.- Voltage-Gated Ion Channels.- G-Protein-Coupled Receptors.- Modern Inhalation Anesthetics.- Inhalation Anaesthesia: From Diethyl Ether to Xenon.- General Anesthetics and Long-Term Neurotoxicity.- Special Aspects of Pharmacokinetics of Inhalation Anesthesia.- Inhalational Anaesthetics and Cardioprotection.- Non-Immobilizing Inhalational Anesthetic-Like Compounds.- Modern Intravenous Anesthetics.- Propofol.- Pharmacokinetics and Pharmacodynamics of GPI 15715 or Fospropofol (Aquavan Injection) — A Water-Soluble Propofol Prodrug.- Etomidate and Other Non-Barbiturates.- Remifentanil and Other Opioids.- Ketamine.- Midazolam and Other Benzodiazepines.- Pharmacokinetics-Pharmacodynamics Based Administration of Anesthetics.- The Effect of Altered Physiological States on Intravenous Anesthetics.- Anesthetics Drug Pharmacodynamics.- Defining Depth of Anesthesia.- Target Controlled Anaesthetic Drug Dosing.- Advanced Technologies and Devices for Inhalational Anesthetic Drug Dosing.- Hypnotic and Opioid Anesthetic Drug Interactions on the CNS, Focus on Response Surface Modeling.

  • Notfallsonographie

Springer-Verlag Berlin and Heidelberg GmbH & Co. KG Notfallsonographie

    15 in stock
