Springer London Ltd The Tourniquet Manual Principles and Practice
Book SynopsisWhy write a book on the tourniquet? The tourniquet is used routinely in op- ating theatres throughout the world, but as far as I know there is no single book that surveys the considerable literature that has accumulated.Trade ReviewFrom the reviews of the first edition: "The use of tourniquet in orthopaedic surgery is a universal practice that is accepted with little question. Leslie Klenerman has had a life-long interest in the tourniquet and has conducted a considerable amount of research on its use and misuse. … He has produced a succinct summary of the physiology and application of the tourniquet … . a very readable account that must be read by all who may require use of this instrument." (Frank Horan, Journal of Bone and Joint Surgery, Vol. 86-B, 2004) "The tourniquet is used daily in orthopaedic practice, and a lot of questions do not have a clear answer … . This little book covers the history, science and practical use of the tourniquet. … Complications, which often have medicolegal implication, are well described. This excellent book finds a place in all orthopaedic departments." (M. De Smedt, Acta Orthopaedica Belgica, Vol. 70 (6), 2004) "This is the first manual entirely devoted to the effects and potential complications of tourniquet use in surgery and anesthesia. It summarizes the scientific work published on the subject … . This original and very clearly presented little manual of 100 pages is of major interest for all tourniquet users, orthopaedic surgeons, anesthetists, and operating-theatre staff. It may also help experts, lawyers and attorneys, since medicolegal procedures can follow the onset of complications related to the use of tourniquets." Brewin Books The "Acci": How the Birmingham Accident Hospital Led the World in the Management of the Severely Injured
Birmingham Accident Hospital, fondly known as The Acci, was established to manage the victims of the Birmingham Blitz, enemy action overseas and those injured in the Midland's factories. This book tells the story of its role during the War and how, in the years that followed, it became world-renowned for excellence in the management of the victims of trauma. Staff at The Acci played a major role in almost every clinical advance from the introduction of antibiotics to the establishment of trauma critical care. Although the hospital closed in 1993, relocated first to Selly Oak Hospital and then to the Queen Elizabeth Hospital, its staff went on to play a key role in the management of the victims of trauma during the wars in Iraq and Afghanistan, once again at the cutting edge of clinical practice. The legacy and the legend of the Birmingham Accident Hospital live on.

Springer Nature Switzerland AG Critical Care of COVID-19 in the Emergency
Book SynopsisThis book functions as a comprehensive manual for emergency care providers who are treating critically ill COVID-19 patients. Beginning with an overview of the COVID-19 infection and an examination of the patients who received critical care from hospital emergency departments around the country, this book provides clinicians with a guide to best-practices in everything from personal protection while deploying critical care to COVID-19 patients to evaluation and diagnosis of the illness. Subsequent chapters break down care of various comorbidities seen in COVID-19 patients and the care offered to patients suffering from the illness in a multitude of ways. Closing chapters examine how emergency departments can best prepare for potential surges in COVID-19 patients as the illness continues to develop, providing important information on how to best protect hospital employees and best care for future patients in a constantly shifting landscape.Due to the rapid onset of the illness and strong-hold the pandemic had on the lives and careers of healthcare professionals, Critical Care of COVID-19 in the Emergency Department fills a critical gap in the literature available on this topic.Table of Contents1. Springer Paediatric Emergencies: A Practical Guide to Acute Paediatrics
Book SynopsisThe aim of this book is to provide a practical guide to help junior doctors to manage the important acute paediatric problems they are likely to encounter. The emphasis has been placed on the diagnostic problems and management when the child first presents. The approach taken is largely pragmatic, in contrast with the more theoretical approach of undergraduate teaching. As many doctors in general paediatrics are also required to perform neonatal resuscitation, a chapter on this topic has been included, but no attempt has been made to cover the specialized field of neonatal intensive care. Several of the chapters have been published in a series of articles in Hospital Update. They have been thoroughly revised and many new chapters added. It would have been impossible for me to have written this book without the help and encouragement of my wife, Dr Ann Goldman. She has read the book at each stage of its gestation and made many constructive suggestions and improvements. I am also grateful to Dr Paul Hutchins who has helped me considerably. Dr Doug Jones has provided helpful advice on the anaesthetic aspects and practical procedures and contributed the section on the insertion of central venous catheters. Many other colleagues have read sections of the book and I should like to thank Drs Ruby Schwartz, Terry Stacey, Andy Whitelaw, Rodney Rivers, John Warner, Sue Rigden, Susan nah Hart, Mike Liberman and Bernard Valman.Table of Contents1. Book SynopsisPaul Weinberg spent 30 years in the Emergency Room (or ER) as a medical doctor and has seen everything, Described as “a strange career” by the author, entry into the field is unrestricted and open to all who are brave (or foolish) enough to start into the stream without the knowledge of the tsunami ahead. The strangeness of the practice is apparent from the very first visit to a busy urban ER. The swarm of commotion and great vividness of the scene can be dizzying. The relentlessness of the torrent and its strange day and night rhythms can enthral and repel like no other practice or job. In turns shocking, sad and funny, this book contains remarkable tales, inside stories and the experiences of a doctor’s career in ER. Emergency medicine in America is a critical asset to its healthcare system. The ER doctor is located at the interface of the public and the first point of healthcare. If a doctor is needed outside of office hours, nights, or holidays, if the patient is uninsured or has inadequate insurance, or is of such a social state that they might be unpleasant to be around, no one is turned away at the ER. In short, the life of the ER doc is one where no situation is off limits.

Oxford University Press Cognitive Autopsy A Root Cause Analysis of Medical Decision Making
Book SynopsisBehind heart disease and cancer, medical error is now listed as one of the leading causes of death. Of the many medical errors that may lead to injury and death, diagnostic failure is regarded as the most significant. Generally, the majority of diagnostic failures are attributed to the clinicians directly involved with the patient, and to a lesser extent, the system in which they work. In turn, the majority of errors made by clinicians are due to decision making failures manifested by various departures from rationality. Of all the medical environments in which patients are seen and diagnosed, the emergency department is the most challenging. It has been described as a wicked environment where illness and disease may range from minor ailments and complaints to severe, life-threatening disorders. The Cognitive Autopsy is a novel strategy towards understanding medical error and diagnostic failure in 42 clinical cases with which the author was directly involved or became aware of at the time. Essentially, it describes a cognitive approach towards root cause analysis of medical adverse events or near misses. Whereas root cause analysis typically focuses on the observable and measurable aspects of adverse events, the cognitive autopsy attempts to identify covert cognitive processes that may have contributed to outcomes. In this clinical setting, no cognitive process is directly observable but must be inferred from the behavior of the individual clinician. The book illustrates unequivocally that chief among these cognitive processes are cognitive biases and other flaws in decision making, rather than knowledge deficits.Trade ReviewThis is the first book I have read that uses actual patient care cases to investigate medical decisions that lead to errors and eventually unexpected outcomes. The depth of the discussion in these cases leaves readers with a good understanding of the processes that lead to the outcomes. * Sheffey N. Chicago Review Press Wild Rescues: A Paramedic's Extreme Adventures in
Book SynopsisWild Rescues is a fast-paced, firsthand glimpse into the exciting lives of paramedics who work with the National Park Service: a unique brand of park rangers who respond to medical and traumatic emergencies in some of the most isolated and rugged parts of America. In 2014, Kevin Grange left his job as a paramedic in Los Angeles to work in a response area with 2.2 million acres: Yellowstone National Park. Seeking a break from city life and urban EMS, he wanted to experience pure nature, fulfill his dream of working for the National Park Service, and take a crash-course in wilderness medicine. Between calls, Grange reflects upon the democratic ideal of the National Park mission, the beauty of the land, and the many threats facing it. With visitation rising, budgets shrinking, and people loving our parks to death, he realized that—along with the health of his patients—he was also fighting for the life of “America’s Best Idea.”Trade Review"There are thousands of ways to die in the Great Outdoors, but an elite group of park rangers and paramedics mind the gap. One of them has finally brought us their riveting stories of rescue in this quick and interesting book full of drama and real-life heroism." Ben Montgomery, author of Grandma Gatewood's Walk" Wild Rescues is a book you want to read, not be featured in. But if you did have the bad luck to be the subject of one of these entertaining and occasionally scary stories, you'd be eternally grateful for Kevin Grange and his fellow Rangers. A paramedic with more than a decade of experience rescuing people, Grange talks of tumbles, heart attacks, freak storms, wild animals, and suicides in the National Parks with the steady voice of one who's seen it all, and then some. Whatever the emergency, specially trained National Park Rangers respond again and again in his pages, risking their necks to save lives. Written with a deep appreciation of the Wild and the awesome beauty of our National Parks, Wild Rescues lives up to its title. Armchair travelers will thrill to the hair-raising rescues, and seasoned trekkers will be reminded of the inherent dangers of their pastime. All will be grateful that people like Grange are on the job." Jim DeFelice, bestselling author of American Sniper"Kevin Grange details nearly everything that possibly could go wrong in a national park and yet still manages to make you more excited than ever to hit the trail." Conor Knighton, New York Times bestselling author of Leave Only Footprints: My Acadia-to-Zion Journey Through Every National Park"Writing at the intersection of medicine and adventure, Grange gives us a fascinating glimpse into rescues that would scare the pants off most of us. He writes with empathy, intelligence, humor and humanity. A captivating memoir. I couldn't put it down." Kate Siber, correspondent for Outside magazine and author of National Parks of the USA"National parks bring together visitors accustomed to more controlled circumstances with heat, cold, cliffs, whitewater, wild animals, and thin air that challenges aging hearts. Anyone who suffers misfortune under these conditions can only hope that Kevin Grange is on duty. This man loves his job." Jordan Fisher Smith, author of Engineering Eden and Nature Noir"This is one helluva book. Grange somehow manages to combine the madness of life on an ambulance with the serenity and awe of America's most beautiful places. Rangers, medics, tourists, adventurers (Alex Honnold makes a brief appearance), the fauna of Yellowstoneit's all here. And you should be too. Wild Rescues is unlike anything you've read." Kevin Hazzard, author of A Thousand Naked Strangers"This book shares experiences, insights, learning from an experienced paramedic and wilderness rescue provider who tells it like it is about the challenges of patient care in remote environments, and the sometimes complex decisions that need to be made when 911 is not at your beck and call. There is honesty in the descriptions of team dynamics and the lives of those dedicated to serving others. And those who appreciate our national parks will take renewed pride at the dedication of those who care for those lands and those who visit them, the NPS rangers, who wear many hatsrescuer, resource caretaker, firefighter, visitor welcome, law enforcerall to preserve this great American idea, our national parks." Springer International Publishing AG Managing Emergencies in the Outpatient Setting: Pearls for Primary Care
Book SynopsisMany primary care clinicians are busy to the point of being overwhelmed and there is not time to do thorough evaluations on every patient. The outpatient clinician must be able to quickly identify and manage the overtly ill but also those with potential or even hidden issues that require a referral to the emergency department. This book provides a concise yet comprehensive summary of the various conditions the primary care practitioner could encounter in his/her office that require emergency department referral. Organized system by system, this manual provides short, readable yet detailed descriptions of situations where the clinician must make the quick decision to escalate the level of care. The book is comprised of thirteen main sections, each detailing a medical specialty and is further broken down into specific conditions within these specialties. The specialties that are featured are cardiology, dermatology, endocrinology, gastroenterology, HEENT, hematology and oncology, infectious disease, nephrology, neurology, gynecology, ophthalmology, psychiatry, pulmonology and urology. The chapters are broken down into five key aspects. First, a brief synopsis of the problem is identified covering the incidence, the risk factors and general information to give the clinician the insight to manage the situation. Next, there is a review of key elements of the history that can elucidate whether the patient has or does not have the emergent condition. Physical exam findings that the clinician can look for during the evaluation are then discussed followed by diagnostics that the clinician can get quickly the same day to help evaluate the situation. Lastly, treatments that can be provided while waiting for the urgent or emergent condition to be evaluated are identified. Cambridge University Press Paediatric Anaesthetic Emergency Data Manual
Book SynopsisThis compact book contains colour coded sections of checklists and flow charts for the immediate management of a wide range of paediatric situations including resuscitation, trauma, anaesthetic and medical emergencies. The ''age-by-page'' section consists of tables of commonly used anaesthetic and resuscitation drugs, pre-calculated for a specific weight within the WHO weight range, for that specific age. Each page includes medication doses, airway equipment including size of endotracheal tubes, laryngeal masks and airway adjuncts, resuscitation fluids including blood products and mannitol and stabilisation infusions such as sedatives and inotropes for ongoing management of the patient. In addition to the emergency guideline sections, the anaesthetic management of 50 common syndromes and congenital conditions are included and the drug formulary provides a convenient reference section of doses and indications of common paediatric medications. An essential quick-reference handbook for anaesthetists, intensive care and emergency department physicians, and trainees.Trade Review'I believe that this is a useful manual that can be of great help not only to the trainee or occasional paediatric anaesthetist but also within the dedicated paediatric hospital … I congratulate the authors on a job well done.'
