For years the administrative of anesthesia was extremely dangerous and risky. Because of this the surgeon and anesthesiologist had to balance the risks and benefits for each patient before proceeding with surgery. In the last two decades the care of the surgical patient has changed dramatically. New equipment, monitors and pharmacologic agents have transformed surgical technique and improved outcomes. Patients once deemed “too sick” for the operating room are found frequently on operating room schedules nationwide. Today, anesthesiology for the healthy patient in most developed countries is extremely safe. However, perioperative complications still occur. These events can be catastrophic for patients and may have serious implications for residents, surgical and anesthesiology staff and nurses. Prompt recognition and management of these incidents may reduce or negate complications. This is based on a fundamental base of knowledge acquired through several avenues and practiced with other team members to maximize outcomes. Engagement of all caregivers impacts outcomes. Many organizations do not have the structural components or education to recognize or manage these catastrophic events. This textbook will provide educational material for the many students, as well as nurses, residents or attending physicians who participate in perioperative medicine. It will focus on the most serious perioperative complications and include a discussion of the pathophysiologic and pharmacologic implications unique to each. If environmental protection is costly, how much should we spend on pollution control? Is it worth reducing pollution to zero, or should we accept some level of pollution because of the economic benefits associated with it? How can we assess the benefits that people get from a less-polluted atmosphere? In broad terms, environmental economics looks at how economic activity and policy affect the environment in which we live. Some production generates pollution, such as power station emissions causing acid rain and contributing to global warming, but household consumption decisions also affect the environment, where more consumption can mean more waste sent to polluting incinerators. However, pollution is not an inevitable consequence of economic activity - environmental policies can require polluting firms to clean up their emissions, and can encourage people to change their behaviour, through environmental taxes on polluting goods, for example. Generally, though, these measures will involve some costs, such as installing pollution control equipment. So there's a trade-off: a cleaner environment, but economic costs. In recent years, many economists have argued for greater use of incentive in the form of pollution charges and emissions trading rather than more traditional direct regulation of polluters. But in fact, more effective policing is key to the success of any comprehensive effort to reduce community gun violence. The stakes are high--gun violence is concentrated in low-income Black communities, and consequently these communities bear the brunt of the associated economic, social, and psychological burdens. Any successful strategy must overcome the current impasse where the residents of high-violence neighborhoods do not trust the police, having experienced both abuse and neglect in their dealings with officers. How can police departments find the right balance between over- and under-policing of high-violence areas? What are the best practices for police to preempt and deter gun violence, while engendering support and cooperation from the public? Peter Levi (1931-2000) was one of the most romantic and complicated of twentieth-century Oxford characters. Although descended on his father's side from Jewish carpet-merchants in Constantinople, he was brought up a Catholic and was a Jesuit from late adolescence until he left to marry at the age of 45. Part-way through his training for the priesthood, Levi joined the small Jesuit intellectual elite as an undergraduate at Campion Hall, Oxford. Already a compulsive poet, he made literary friends and experimented with hard, bright lyrics on a variety of themes. His first collection, From the Gravel Ponds, was Poetry Book Society choice for spring 1960. From then onwards he maintained an uneasy balance as a Jesuit, tolerated for his literary activities but always subject to disciplinary correction. When, in 1963, his seniors declared that he had broken so many rules that he could not be ordained that year, he persuaded them to let him visit Greece. That summer he fell in love with the country and formed lasting friendships with several Greek poets. As classics tutor at Campion Hall from 1965 to 1977, he was an intriguing figure to many undergraduates.Intolerant of Oxford damp, he was allowed to spend winters in Greece. From 1967 onwards he passionately supported the resistance to the junta; twice banned from Greece, he later made a television documentary about the evils of the regime. After leaving the Jesuits he wrote autobiographical works, translations, thrillers and elegiac verse, supplementing his literary earnings with casual tuition. Elected Oxford Professor of Poetry in 1984, he used his lectures to convey his enthusiasm for the work of other poets, from Shakespeare to Pasternak, Larkin, Lowell and his defeated electoral rival and former pupil James Fenton. This deeply researched, sensitively written biography explores Levi's many friendships, with figures such as Cyril Connolly, George Seferis, David Jones, Iris Murdoch, Patrick Leigh Fermor and Bruce Chatwin. In the same way that women who reduce employment hours when raising their young children experience reductions in salary, savings, and public and private pensions, the mothers of those same women, as grandmothers, are rearranging hours to take care of their grandchildren, experiencing additional loss of salary and reduced old age pension accumulation. Madonna Harrington Meyer’s Grandmothers at Work, based primarily on 48 in-depth interviews conducted in 2009-2012 with grandmothers who juggle working and minding their grandchildren, explores the strategies of, and impacts on, working grandmothers. While all of the grandmothers in Harrington Meyer’s book are pleased to spend time with their grandchildren, many are readjusting work schedules, using vacation and sick leave time, gutting retirement accounts, and postponing retirement to care for grandchildren. Some simply want to do this; others do it in part because they have more security and flexibility on the job than their daughters do at their relatively new jobs. Many are sequential grandmothers, caring for one grandchild after the other as they are born, in very intensive forms of grandmothering. Some also report that they are putting off retirement out of economic necessity, in part due to the amount of financial help they are providing their grandchildren. Finally, some are also caring for their frail older parents or ailing spouses just as intensively. Most expect to continue feeling the pinch of paid and unpaid work for many years before their retirement. In this famous essay, first published in 1793, Kant considers the alleged conflict between theory and practice in the conduct of human affairs in three widening contexts: those of the common person faced with a moral decision, of the politician and the citizen concerned with the extent and limits of political obligation, and, finally, of the citizen of the world whose actions have a bearing on war and peace among nations. Unlike other animals, Kant reminds us, people must decide how they will live their lives. They therefore ask for a guide to action, a set of principles—a theory. From the outset, Kant rejects the ancient claim that the practical possibilities of action cannot always be reconciled with moral demands. He offers his own moral theory, a theory starting out from the principle of the right as an unequivocal guide to action. In partial disagreement with the rival theories of Hobbes and Locke, he proposes that the only condition under which the individual can achieve true destiny as a person and a member of the human race is the civil state. Such a state can be secured only by law. Although "from such crooked wood as man is made of, nothing perfectly straight can be built," only the rule of law can bring about a stable society. Last, Kant turns to the relation between theory and practice in international relations. "Nowhere," he writes, "does human nature appear less lovable than in the relation of whole nations to each other." But to hope for world peace on the basis of "the so-called balance of power is a mere chimera." There is no other remedy to international lawlessness and war than an international coercive law, and such law can grow only out of sound theory. "I put my trust in theory. At the same time, I trust in the nature of things, and also take account of human nature, which I cannot, or will not, consider so steeped in evil that in the end reason should not triumph."
