Back pain is the commonest form of rheumatism and it is estimated to affect between 65% and 80% of populations sampled. The costs of back pain-related medical care and disability compensation make it one of society's most significant non-lethal medical conditions. Whilst mortality is low, its morbidity is high, inconvenience great, and economic burden massive. The actual symptom of back pain is one of the commonest and most perplexing encountered in medical practice. A proper understanding of what it means is crucial for further medical advance. However, the field of back pain research probably has more experts than any other. Anybody who has back pain will, as often as not, expound views on all matters concerning causation and expectations. Indeed, it is not for a lack of ideas that this field of investigation has failed to make progress. A respected senior investigator advised me early on that patients could not even tell which side their pain was on. It was only many years later that I discovered why. Though the initial incident or accident would seem important to the patient and to the researcher, it is remarkable how little it helps distinguish patterns of back pain. Tests of inappropriate behaviour in the past have been widely disseminated and yet even the originators have said that they cannot be sure of the implications of positive non-organic test findings. This book will show that these 'indicators' are not what they seem to be. In many cases, it is difficult to tell which level in the lumbar spine the pain comes from, and thus treatments such as injections or operation, may be given at the wrong level, let alone the wrong side. Though the various forms of back pain treatment seem to help some patients, when these same treatments are subjected to randomised controlled trials, the evidence for efficacy seems to all but vanish. With all these difficulties and uncertainties, it can be seen why low back pain presents one of the most fallible fields of healthcare. This book indicates that there are ways of dealing with the majority of these problems. Many of the answers from the research conducted were most unexpected. The reader is also advised that the scientific approach cannot deliver hard and fast answers to all contemporary questions in such a wide field and much has depended on the choice of problems to study. Whilst the tests used to distinguish the patterns in this study have their limitations, and whilst there is, as yet, only limited evidence as to the underlying pathology for the patterns recognised, it is felt that some of the other patterns identified are descriptive of diagnoses that are generally accepted. If the answers that are put forward appear to change the direction of understanding, it is hoped that you, the reader, will be able to carry the enquiry further when you see patients, advise treatment or even experience back pain yourself!

Americans have always been a hard-drinking people, but from 1920 to 1933 the country went dry. After decades of pressure from rural Protestants such as the hatchet-wielding Carry A. Nation and organizations such as the Women's Christian Temperance Union and Anti-Saloon League, the states ratified the Eighteenth Amendment to the Constitution. Bolstered by the Volstead Act, this amendment made Prohibition law: alcohol could no longer be produced, imported, transported, or sold. This bizarre episode is often humorously recalled, frequently satirized, and usually condemned. The more interesting questions, however, are how and why Prohibition came about, how Prohibition worked (and failed to work), and how Prohibition gave way to strict governmental regulation of alcohol. This book answers these questions, presenting a brief and elegant overview of the Prohibition era and its legacy. During the 1920s alcohol prices rose, quality declined, and consumption dropped. The black market thrived, filling the pockets of mobsters and bootleggers. Since beer was too bulky to hide and largely disappeared, drinkers sipped cocktails made with moonshine or poor-grade imported liquor. The all-male saloon gave way to the speakeasy, where together men and women drank, smoked, and danced to jazz. After the onset of the Great Depression, support for Prohibition collapsed because of the rise in gangster violence and the need for revenue at local, state, and federal levels. As public opinion turned, Franklin Delano Roosevelt promised to repeal Prohibition in 1932. The legalization of beer came in April 1933, followed by the Twenty-first Amendment's repeal of the Eighteenth that December. State alcohol control boards soon adopted strong regulations, and their legacies continue to influence American drinking habits. Soon after, Bill Wilson and Dr. Bob Smith founded Alcoholics Anonymous (AA). The alcohol problem had shifted from being a moral issue during the century to a social, cultural, and political one during the campaign for Prohibition, and finally, to a therapeutic one involving individuals. As drinking returned to pre-Prohibition levels, a Neo-Prohibition emerged, led by groups such as Mothers against Drunk Driving, and ultimately resulted in a higher legal drinking age and other legislative measures. With his unparalleled expertise regarding American drinking patterns, W. J. Rorabaugh provides an accessible synthesis of one of the most important topics in US history, a topic that remains relevant today amidst rising concerns over binge-drinking and alcohol culture on college campuses.

In Suburbs under Siege Charles Haar argues passionately that all people--rich or poor, black or white--have a constitutional right to live in the suburbs and that a socially responsible judiciary should vigorously uphold that right. For various reasons, American courts have generally failed to question local zoning regulations that trap the urban poor in the squalor of inner cities, away from decent housing and jobs in the suburbs. No U.S. Supreme Court case, for instance, has confronted exclusionary zoning rules, as Brown v. Board of Education once attacked school segregation. Instead, judges at all levels have most often reinforced the residential segregation that may well destroy American society. In this provocative book on the landmark Mount Laurel cases, Haar shows how the N.J. state judiciary broke out of this pattern of judicial behavior. These courageous, innovative judges attracted nationwide attention by challenging the forces of affluence that ruled the suburbs (and the legislature) of their state. Furthermore, they based their reasoning on the N.J. state constitution in order to protect their rulings from invalidation by the U.S. Supreme Court. In the early 1970s, when the cases began, the plaintiffs, Ethel Lawrence and her daughter Thomasene, were barely making ends meet in the Philadelphia suburb of Mount Laurel, a town where their African-American ancestors had lived for seven generations. The Lawrences' dream was to live in a Mount Laurel garden apartment planned by a grassroots reform group as affordable housing: in their way stood a typical minimum acreage zoning ordinance. The eventual court victory of the Lawrences and their young public interest attorneys inspired other N.J. suits and a process of remediation that continues to this day, as judges, experts (special masters), the state legislature, and other citizens work to carry out the Mount Laurel principles. Haar's book is a bold attack on conventional doctrines of the separation of powers limitations on the judicial branch and a plea that judges across the country assume their proper responsibilities for fair housing before it is too late. Since the appearance of Darwin's book, "The Origin of Species," adaptation is one of the processes that explains the diversity of species in ecosystems. Adaptive phenomena in the 19th century and until the mid-20th century have been analyzed in macroscopic biological systems, however since the second half of the 20th century and to date the development of disciplines such as Molecular Biology, has allowed us to delve into the mechanisms that regulate cell physiology. The molecular bases that allow explaining the adaptation processes of microorganisms to their environment have special relevance, because through their analysis it is possible to size the complexity of these mechanisms that involve receptors of a protein nature associated with transduction chains that transport the information flow to genomic DNA, and which subsequently involves the emission of a response through the expression of specific genes. From the point of view of the adaptive phenomenon analysis, the approach through the molecular bases makes it possible to understand the enormous diversity of the microbial world. Mainly for two reasons, on the one hand the presence of micro gradients in the bacterial ecological niches that are continuously fluctuating, which forces the microorganisms to a rapid adaptation phenomenon. And on the other hand, horizontal gene transfer phenomena, which allow bacteria the information exchange. These two elements carry great intensity in establishing new relationships. This phenomenon is especially relevant if it is related to a concept that Darwin cites in the Origin of Species, "The tangled riverbank", where it is emphasized that the new interactions establishment is the basic driving force for the new species generation. This mechanism is explained by the positive feedback loop generation, whereby ecosystems with high levels of biological diversity generate new interactions that lead to new species, which in turn tends to make the ecosystem network more complex. This complexity analyzed in its molecular bases allows to generate new research questions that can be applied to other knowledge areas, such as Biotechnology. The analysis of the molecular bases of the microorganisms adequacy, makes it possible to identify and characterize mechanisms that implemented in different pharmaceutical areas and agricultural industry has led to the product generation with high added value, a clear example of this economic development is the enzyme industry and even recombinant protein production. Finally, it is convenient to emphasize the need to incorporate the analysis of the molecular bases of adaptation from the perspective of omics techniques. Techniques that allow the study of processes and mechanisms to be approached from a global perspective. This book summarizes some topics of special relevance referring to adaptive processes of different microorganisms of special relevance both in basic and applied research.

The formation of the Pathfinder Force in August 1942 produced a steady but certain change in the fortunes of Bomber Command. Its effectiveness against targets during the early years of the war had been very difficult to gauge. When examined in detail afterwards, aerial photographs showed that only one third of the aircraft were successfully reaching their target area and less than this were actually placing their bombs with target accuracy. It was known during the large-scale bombing of Coventry in the autumn of 1940 that the Germans had used an elite force of pathfinder aircraft, armed with incendiaries, who had acted as target finders for the main force of German bombers. What was now needed for the RAF were some similar specialist squadrons, with crews handpicked for their discipline, courage, high morale and, in particular, skills in a wider than normal range of flying jobs. Sidney Bufton, Deputy Director of Bomber Operations, developed the concept of a new Target Finding Force, and his tenacity in putting pressure upon senior Air Ministry staff to implement his ideas and bring Bomber Command out of the doldrums paid off. The new force was finally accepted and the choice of Don Bennett as its first Commander was inspired. Bennett was restless, imaginative, and receptive to change. He never accepted second best and he became a legend to all who served under him. Pathfinder Squadrons were equipped with the best available aircraft, which included the famous Lancaster bomber and later, increasingly, the Mosquito which was a hugely versatile and successful fighter bomber. Would be aircrew for the RAF came as volunteers from all parts of the world including Canada, America, Australia and New Zealand. To join a Pathfinder Squadron was a rare privilege but with it went a huge leap in the likelihood of being shot down. Pathfinder aircrew and aircraft had to lead the way for their following Bomber Force in hazardous raid after raid. They flew at night but it took a full 25 minutes to run the gauntlet of the Berlin defences from end to end at full stretch. They were highly vulnerable to the wall of flak thrown up by German city defenders, as well as to attacks by night-fighters. By the end of the war some 56,000 crewmen of Bomber Command had lost their lives. Martyn Chorlton has written a gripping account of the RAF's Pathfinder Squadrons, recalling the challenges faced in the smoke-filled skies over occupied Europe. It is also a tribute to the brave young men whose exploits, lives and, in all too many cases, deaths have left a powerful torch to bear for all who care about freedom. Is there such a thing as Latin American Art? Does it need this label in a global art world? It is, of course, clear that there is no such thing, just as there is no African nor European art. It would be interesting though to include Caribbean art (which of course doesn't exist either) and talk about specific forms of creolization in the way that Edouard Glissant means it from a global perspective that is not just Western. If we look at different artistic expressions in different local contexts within a geographic and linguistic area called Latin America, the insights we get are very interesting. It might remind us of a kaleidoscope, an ever changing, glittering flux of perspectives on a fluid carrier medium. Through extensive field research involving travel in Argentina, Brazil, Colombia, Honduras, Mexico and Peru, I have been seeking to answer these questions and go into the Looking Glass. Those journeys have included connecting as much as possible with locals and investigating the subject of visual culture through them. In order to learn about local traditions, I have explored a small portion of the Amazonas on the border of Tres Fronteras, and stayed with an indigenous family on one of the Islands of Lake Titicaca. Additionally, in Peru I visited the art school in Cusco which is famous for the Cuzco School paintings. Whether in rural settings or the urban landscapes of Latin America, my approach has always been to be humble, to listen and learn, aware that my perspective is that of a Western woman. The discovery of immense cultural diversity intensely broadened my horizons. In order to develop an understanding of Latin American art, I have realized that attempting to describe it requires an open mind that stretches beyond any pre-conceived Eurocentric notions. Only then it is possible to appreciate the enormous variety of artistic expressions and contexts throughout multiple Latin American regions. The projects described here reflect fifteen collaborations that were carried out, over a period of six years involving thirty-four artists. In some cases, the exchanges included galleries in various Latin American countries. The themes explored were diverse and varied. They ranged from the abstract and highly conceptual to the more tangible and material. Together, the culminating exhibitions have each contributed to exploring essential questions about Latin American Art. The answers, however, are far from simple or complete. Beyond the collection gathered here, the process of discovery continues. So far, I have only witnessed the seedlings of my understanding beginning to emerge. During the process of trying to better understand Latin American Art and through producing exhibitions in Switzerland, as well as curating content in Mexico City and Brazil, it has become clear that cultural bridges can be built through the universal interests that inform artistic expression. In July 1841, Ralph Waldo Emerson wrote to Thomas Carlyle: “My whole philosophy…teaches acquiescence and optimism.” The journals in this volume, beginning in the summer of 1841, record the spiritual history of two years that can be viewed as the most critical test in Emerson’s life of his ability to maintain the two aspects of that philosophy.Early in 1842 his son Waldo died, and the man who only months before had described himself as “professor of the joyous Science” found himself once again confronting the full implications of grief. The Crocker Art Museum has one of the finest and earliest German drawings collections in the United States. Featuring artists such as Johann Wolfgang Baumgartner, Anton Raphael Mengs and Johann Heinrich Wilhelm Tischbein, The Splendor of Germany examines the major developments in German draughtsmanship over the course of the eighteenth century. Published to coincide with the collection’s 150th anniversary. In the 21st century, the collecting and study of 18th-century German drawings has become a major focus for American museums. One of the finest collections of them, however, has been in California for 150 years. The superb drawings at the Crocker Art Museum, from a Baroque altarpiece design by Johann Georg Bergmüller to a Neoclassical mythology by Johann Heinrich Wilhelm Tischbein, provide a panorama of German draughtsmen and draughtsmanship throughout the century. Many of the drawings are remarkable for their modernity. A self-portrait by Johann Gottlieb Prestel bypasses convention to achieve a direct, unmediated likeness. Well-placed slashes with brush and black ink define the features below his peruke outlined in black chalk. Other drawings encapsulate specific developments and styles, such as Johann Wolfgang Baumgartner’s Lazarus and the Rich Man, which shows the florid dynamism of the Augsburg Rococo. A full range of eighteenth-century German artists are represented here, from the satirizing moralists Johann Elias Ridinger and Daniel Chodowiecki to the Classicist and friend of the art theorist Johann Joachim Winkelmann, Anton Raphael Mengs. Landscape artists are especially well represented, such as the key figure Johann Georg Wille, printmaker to the French king Louis XV, and generations of artists he taught and influenced all the way to the early Romantic landscapists. The exhibition and catalogue gather together a variety of dynamic and sensitive portraits, charming scenes of daily life, and often humorous moralizing subjects, as well as narratives, both religious and mythological, from the late Baroque to Neoclassicism. In the realm of landscape, the depth of the collection allows the exhibition to trace schools and influences—in addition to Wille’s mentioned above—even in families such as that of Prestel, whose wife and daughter were both landscapists. It also allows it to demonstrate the great variety of works by single artists such as Christoph Nathe, represented by four landscapes in four different genres including a splendid scene near Görlitz. Some artists, in fact, work in several genres as in the case of Johann Christian Klengel, whose works include the scene of a family by candlelight, a farmstead landscape, and a sketchbook that he carried through the countryside to record picturesque views. This is a rare opportunity for the public and for drawings enthusiasts. Two-thirds of the drawings in the exhibition have not been shown before; most of the exceptions have not been seen since 1989. Because of the drawings’ 150-year history of limited exposure, the state of preservation of the collection is exceptional, as is the condition of the new acquisitions included in the exhibition.

The Schuylkill River flows more than 100 miles from the mountains of the Pennsylvania Coal Region to the Delaware River. It passes through five counties - Schuylkill, Berks, Chester, Montgomery, and Philadelphia - and its valley is home to more than three million people, yet few are aware of the hidden ruins and traces left by a pioneering 200-year-old inland waterway: the Schuylkill Navigation. Some of it is literally buried in their own backyards.Often called the Schuylkill Canal, this complex Navigation system actually boasted twenty-seven canals. The first of the anthracite-carrying routes in America, the 108-mile Navigation shadowed the Schuylkill River for nearly all its length. It once had more than thirty dams and slackwater pools, more than 100 stone locks, numerous aqueducts, and the first transportation tunnel in the nation. They were all built by hand starting in 1816.In the 1940s, as part of a massive environmental cleanup of the river, this important and influential infrastructure was largely dismantled - but not entirely. Two short sections of the watered canal get plenty of attention: the Oakes Reach at Schuylkill Canal Park near Phoenixville and the Manayunk Canal in Philadelphia. Both are popular recreational destinations. What happened to the rest of it?Photographer Sandy Sorlien resolved to find out. Over the course of seven years, she traveled upriver repeatedly to bushwhack along the riverbanks and to row and paddle in the river itself. Armed with camera and binoculars, loppers and trekking poles, nineteenth-century maps and modern satellite imagery, and abetted by local historians and an archaeologist, she found all sixty-one lock sites and explored most of the canal beds. Her photographs reveal a mysterious remnant landscape, evidence of a bold industrial innovation that spelled its own demise.
