The well-known Alzheimer's Disease Neuroimaging Initiative (ADNI) Center provides the most advanced, comprehensive, multiparametric and up-to-date biomarkers for mild cognitive impairment (MCI) and early Alzheimer's disease (AD) projects, including neuroimaging, clinical assessments, biospecimens and genetic data. Recent developments in imaging techniques, including new molecular tracers for imaging disease burden and systematic multi-modal integration, have emerged to overcome the limitations of each single modality and individual-dependent variability. The MRI-based high-resolution structural and morphological changes in the brain, such as atrophy, and the abnormal activity/connectivity patterns of the hippocampus subfields and default mode network (DMN) modulation, together with the amyloid and tau neuropathological quantification using PET molecular tracers, could be used to predict brain changes and cognitive performance declines in early AD, including transitional MCI. Finally, a generalized and integrative model with multiple biomarkers could be built to target disease progression and symptom prediction as well as to optimize patient management. Multiomics investigates metabolomic, lipidomic, genomic, transcriptomic and proteomic perspectives by presenting an accurate biochemical profile of the organism in health and disease. The Alzheimer's Disease Metabolomics Consortium (ADMC) in partnership with ADNI is creating a comprehensive biochemical database for patients in the ADNI1 cohort, consisting of eight metabolomics datasets. The vast majorities of biospecimen data provide rich biological information to the human brain at normal and dementia status. One of the purposes is to reveal the connections between disease and multiomics such as obesity, hypertension, cholesterol imbalance and inflammation risks that might lead to neurodegenerative disease. Multiomic biomarker developments in the dementia field have provided earlier clues to novel treatments that help correct metabolic dysfunction and delay disease progression. Furthermore, the assembling of multiomics-based biomarkers including metabolites and lipids, cholesterol biosynthesis, purine metabolism, lipoprotein, bile acids, and genetics as well as their relation to the pathological amyloid and tau network could improve disease diagnosis sensitivity and reveal more diverse and complementary molecular pathways to allow for the advancement of early AD diagnosis and therapeutic prevention. In this book, we report on the significant differences of multiple biomarkers from the ADNI database including neuroimaging, clinical assessments and multiomic biospecimen/genetic data in MCI and early probable AD (pAD), and elucidate the interconnections among different metrics at various domains. Classification results with high accuracies (0.95-1) for each early dementia subtype including early MCI (EMCI), late MCI (LMCI) and pAD, and better prediction of clinical symptoms is achieved with these comprehensive biomarkers. Further longitudinal changes of imaging and neuropsychological biomarkers, and inter-correlations with baseline parameters are examined for a better illustration of disease progression association. Additionally, an analysis of the post-traumatic stress disorder biomarkers is performed with high classification accuracy. With illustrative and rigorous data analyses and confirmative results, this book provides readers with a full spectrum of biomarker research for early dementia diagnosis and treatment, and helps convey the technical development and data evaluation perspectives in advanced medical imaging and various disease application fields.

While arthroplasty is the preferred treatment for most elderly patients with displaced femoral neck fractures, internal fixation is the treatment of choice in the majority of patients below the age of 65 as a joint-preserving procedure. The osteosynthesis of fractures of the femoral neck in the elderly has been partly abandoned during the last years due to the poor clinical outcomes following the conventional fixation with parallel screws or DHS. Based on clinical evidence and laboratory testing, the novel method of biplane double-supported screw fixation (BDSF) offers much better fixation stability, reflecting in excellent clinical outcomes. With its innovative biomechanic principle, the BDSF method provides supreme stability for cannulated screw fixation, achieving up to 44% higher axial fixation strength in vitro, and a rate of bone union reaching up to 96.6% in clinical practice, which is much higher than the conventional parallel screw fixation data. The method of BDSF provides supreme stability by buttressing two out of three medially diverging cannulated screws on the inferior femoral neck cortex and supporting the steeper inferior screw on the posterior femoral neck cortex. The two calcar screws are oriented in different coronal inclinations intended to provide constant fixation strength during different patient activities and load directions. Biomechanically, the most effective component is the inferior screw placed at an obtuse angle and supported on a large area along the inferior and posterior cortex of the femoral neck following its spiral anterior curve. Given the clinical outcomes, BDSF is the perfect technique for femoral neck fracture fixation, as the fracture healing rate is high at 96% with this approach. Therefore, BDSF is not only a treatment alternative to conventional fixation, but also a much better procedure. Thus, BDSF should be routinely applied, and conventional fixation gradually abandoned in clinical practice (this has been the approach in our institution over the last ten years). This book describes the full surgical technique of the method of BDSF for femoral neck fracture osteosynthesis; quality criteria and surgical recommendations for successful BDSF implementation, according to the vast clinical experience of ten years with this highly effective method. A novel surgical approach for hip arthroplasty is described in this book. The current trends aimed at decreasing operative trauma and blood loss have been not entirely satisfied with respect to most of the standard approaches for hip arthroplasty. These surgeries are often associated with considerable blood loss and the necessity for restricting patients' activities in the postoperative period due to impaired joint stability and risk of dislocations. This book describes the full surgical technique of the novel anatomical direct lateral approach for hip arthroplasty, aimed at decreasing blood loss, minimizing operative trauma, and optimizing joint stability. This technique is associated with minimal blood loss and high joint stability. Patients are allowed to perform activities within the normal range of motion in the early postoperative period. This book describes also the history of internal fixation in femoral neck fractures, as well as the biomechanics of femoral neck fracture osteosynthesis and the role of the implants.

The atheists Daniel Dennett in Breaking the Spell and Richard Dawkins in The God Delusion talk down to believers. Sam Harris in The End of Faith and Letter to a Christian Nation insults believers outright. All three assume that believers are not very bright. Their approach is not productive of much understanding. In The Philosopher’s Enigma, Richard Watson explains to believers in temperate and readable prose why he and many others are not believers. His discussion is based on strict Augustinianism, the foundation of seriously argued Christianity. God is hidden – that is, the concept of God is unintelligible – as discussed at length by Leszek Kolakowski in his Religion If There Is No God (St. Augustine’s Press) – in the sense that there are no known rational arguments for God’s existence. Moreover, Augustine argues that finite human beings cannot understand God’s infinite perfections. Augustine concludes that God has omniscient knowledge of every human being’s behavior, which after all, is predetermined by God prior to His creation of the world. Most difficult to accept, as Calvin later stresses, is the inference that because humans do not determine their own behavior, God predetermines who is saved and who is damned with no reference to this behavior. A foundation of Christianity is that because of the Fall of Man, we are all sinners, and thus there is no reason why God should pick this person for salvation and that one for damnation. But most Christians believe that faith, God’s grace, Jesus’ sacrifice, being born again, and in particular, good works, can earn one salvation. But Augustine and later Calvin see no evidence for these views. Even if, or perhaps even because, God gives a sinner the grace to be good – a person’s good works do not assure salvation. After all, even before God created the world, God predetermined the behavior of every human being. Thus because humans cannot determine their own behavior, they cannot be saved or damned with reference to this behavior.A major difficulty in understanding and accepting the story of the Creation, then, is that even though God determines Adam’s behavior, God punishes Adam for disobedience by decreeing that all Adam’s progeny will be born sinners. Watson begins his book with the steel-trap objections made by his daughter, when she was seven years old, as he read the Bible to her. To the story of the Garden, she objected: “But God made Adam! God made Adam sin! God is not fair!” She slid off his lap, and he had to bribe her to return.In The Philosopher’s Enigma, Watson also discusses in detail the concepts of the soul, angels, ghosts, mind, and body. He argues that the classic Cartesian mind/body problem of how an immaterial mind or soul and a material body can interact will eventually be superseded by a concept of a human being according to which, even though a person’s body/mind is bound by physical laws, it still makes its own considered decisions, and to that extent a human being is free. And because the mind/body is one entity, there is no problem about two different things – a mind and a body-interacting.Watson concludes that this means there is no such thing as a disembodied mind or soul, and so no such things as angels and ghosts that could help or harm you. Basing this discussion in the context of contemporary neurophilosophy, his conclusions about the relationships of mind/soul follow those of Kolakowski in being reminiscent of Spinoza.

What is the man who cannot be known apart from his socio-political environment? As Zbigniew Janowski asserts, one does not ask who this man is, for he does not even know himself. This man is suppressed and separated, and not by Fascism or Communism. In present-day America this has been accomplished by democracy. “Only someone shortsighted, or someone who values equality more than freedom, would deny that today’s citizens enjoy little or no freedom, particularly freedom of speech, and even less the ability to express openly or publicly the opinions that are not in conformity with what the majority considers acceptable at a given moment. It may sound paradoxical to contemporary ears, but a fight against totalitarianism must also mean a fight against the expansion of democracy.” Janowski all at once brazen and out of bounds states what he calls the obvious and unthinkable truth: In the United States, we are already living in a totalitarian reality. The American citizen, the Homo Americanus, is an ideological being who is no longer good or bad, reasonable or irrational, proper or improper except when measured against the objectives of the dominating egalitarian mentality that American democracy has successfully incubated. American democracy has done what other despotic regimes have likewise achieved––namely, taken hold of the individual and forced him to renounce (or forget) his greatness, pursuit of virtue and his orientation toward history and Tradition. Homo Americanus, Janowski argues, has no mind or soul and he cannot tolerate diversity and indeed he now censors himself. Democracy is not benign, and we should fear its principles come by and applied ad hoc. It is deeply troublesome that in the way democracy moves today it gives critics no real insight into any trajectory of reason behind its motion, which is erratic and unmappable. The Homo Americanus is an ideological entity whose thought and even morality are forbidden from universal abstraction. Janowski mounts the offensive against what the American holds most sacred, and he does so in order to save him. After exposing the danger and the damage done, Janowski makes another startling proposal. It is a “diseased collective mind” that is the source of this ideology, the liberal anti-perspective that presses man into the image of the Homo Americanus, and its grip can only be broken through the recovery of instinct. Homo Americanus cannot be free again until he is himself again. That is, until the shadow that belongs only to him is restored, and he is thereby no longer alienated from others. Despite the condemnation Janowski seems to be levying on the citizen of the United States, he betrays a great hope and confidence that the means to shake ourselves awake from the bad dream are nevertheless in hand. Janowski’s work is the next title in St. Augustine’s Press Dissident American Thought Today Series. It occupies a controversial overlapping terrain between the philosophical descriptions of liberalism as a tradition, psychology and the fundamentally influential critiques of democracy offered by Thucydides, Jefferson, Franklin, Tocqueville, Mill, Burke and more. More anecdotal than analytical, Janowski offers the contemporary proof that the reader is right to be scandalized by democracy and his or her own likeness of the Homo Americanus. Once upon a time it was the despicable Homo Sovieticus fruit of tyranny, but now we fear democratic society too might fall and all its citizens never be found again.

Kathryn Maple (b. 1989, Canterbury) is an artist specialising in drawing and painting. Her large-scale paintings feature urban, suburban and rural landscapes which are frequently populated by human figures. Her work is distinctive for its use of intensely layered mark making, lending the work both urgency and intimacy. The places and people depicted, rendered in a range of painting and drawing materials, are frequently afforded a sense of wildness or mystery by dint of their colour palette, collage-like compositions and recurring motifs such as wind-blown trees and winding pathways.This, her first monograph, features 379 images, many of which are reproduced for the first time. These include the presentation of her recent major series of oil pastel on paper works 'A Year of Drawings', alongside reproductions of her mixed media works on paper, as well as large oils on canvas.An essay by Kathryn Lloyd, writer, artist and Contemporary Art Editor at The Burlington Magazine, offers insight into Maple’s impulse to explore the world around her through her work. Large-scale paintings, replete with dense layers of marks, are constructed by means of personal encounter, memory and imagination. In the devastation that follows a major disaster, there is a need for multiple sectors to unite and devote new resources to support the rebuilding of infrastructure, the provision of health and social services, the restoration of care delivery systems, and other critical recovery needs. In some cases, billions of dollars from public, private and charitable sources are invested to help communities recover. National rhetoric often characterizes these efforts as a "return to normal." But for many American communities, pre-disaster conditions are far from optimal. Large segments of the U.S. population suffer from preventable health problems, experience inequitable access to services, and rely on overburdened health systems. A return to pre-event conditions in such cases may be short-sighted given the high costs - both economic and social - of poor health. Instead, it is important to understand that the disaster recovery process offers a series of unique and valuable opportunities to improve on the status quo. Capitalizing on these opportunities can advance the long-term health, resilience, and sustainability of communities - thereby better preparing them for future challenges. Healthy, Resilient, and Sustainable Communities After Disasters identifies and recommends recovery practices and novel programs most likely to impact overall community public health and contribute to resiliency for future incidents. More is known about Nikita Khrushchev than about many former Soviet leaders, partly because of his own efforts to communicate through speeches, interviews, and memoirs. (A partial version of his memoirs was published in three volumes in 1970, 1974, and 1990, and a complete version was published in Russia in 1999 and will appear in an English translation to be published by Penn State Press.) But even with the opening of party and state archives in 1991, as William Taubman points out in his Foreword, many questions remain unanswered. "How did Khrushchev manage not only to survive Stalin but to succeed him? What led him to denounce his former master [an event that some interpreters herald as the first act in the drama that led to the end of the USSR]? How could a man of minimal formal education direct the affairs of a vast intercontinental empire in the nuclear age? Why did Khrushchev’s attempt to ease East-West tensions result in two of the worst crises of the Cold War in Berlin and Cuba? To resolve these and other contradictions, we need more than policy documents from archives and memoirs from associates. He has received many awards and honors for his work in computer science, missile design, and space research. Besides his many technical publications, he has published widely on political and economic issues. In 1991 Little Brown published his memoir about his father’s last years, Khrushchev on Khrushchev. In that same year he received an appointment to the Center for Foreign Policy Development of the Thomas J. Watson Institute for International Studies at Brown University, where he is today. He and his wife, Valentina Nikolayevna, applied for U. S. citizenship in 1999, an event widely covered in the media.

The potential of tomb mosaics as an academic resource has often been underestimated and consequently they have only been partially analysed not only in Italy but also throughout the Western Mediterranean. This work is intended to shed a new light on these finds, which are often incomplete, lost, or little studied. The first part of the book presents the history of previous studies on the subject and briefly explains the structure of the corpus. The corpus, in turn, is organised according to current Italian administrative regions, specifically: Sardegna, Sicilia, Puglia, Campania, Lazio, Marche, and Friuli Venezia Giulia. Every region is then further divided following current provinces and municipalities. This work does not aim to present merely a compilation of data in a catalogue; thus the second part of the book focuses specifically on tomb mosaics found in the Italic peninsula and major islands, and provides information on their geographic distribution, dating, typology, place of discovery and iconography, and considers the potential identification of individual workshops. The purpose of the book is to bring tomb mosaics to greater consideration, since they have not survived in academic literature to the same extent as did their rich villa or domus counterparts. This work does not therefore aspire to be a complete analysis of the subject, but rather a starting point which can be both useful and a stimulus for future studies. Her multimedia practice, which incorporates sculpture, video, painting and sound, explores the processes and effects of perception, the complexities and contradictions of landscapes and visuality, and the politics of representation and territorialisation. On April 26, 1865, as Abraham Lincoln's funeral cortege paused in Union Square, New York, before being taken by rail to Springfield, Illinois, William Cullen Bryant listened as his own verse elegy for the slain president was read to a great concourse of mourners by the Reverend Samuel Osgood. Only five years earlier and a few blocks downtown, at Cooper Union, Bryant had introduced the prairie candidate to his first eastern audience. There his masterful appeal to the conscience of the nation prepared the way for his election to the presidency on the verge of the Civil War. Now, Bryant stood below Henry Kirke Brown's equestrian statue of George Washington, impressing Osgood as if he were "the 19tth Century itself thinking over the nation and the age in that presence." Bryant's staunch support of the Union cause throughout the war, and of Lincoln's war efforts, no less than his known influence with the president, led several prominent public figures to urge that he write Lincoln's biography. Oliver Wendell Holmes wrote him, "No man combines the qualities for his biographer so completely as yourself and the finished task would be a noble crown to a noble literary life." But Bryant declined, declaring his inability to record impartially critical events in which he had taken so central a part. Furthermore, while preoccupied with the editorial direction of the New York Evening Post, he was just then repossessing and enlarging his family's homestead at Cummington, Massachusetts, where he hoped his ailing wife might, during long summers in mountain air, regain her health. But in July 1866, Frances died of recurrent rheumatic fever, and, Bryant confessed to Richard Dana, he felt as "one cast out of Paradise." After France's death Bryant traveled with his daughter Julia for nearly a year through Great Britain and the Continent, where he met British statesman and novelist Edward Bulwer Lytton and French literary critic Hyppolyte Taine, renewed his friendship with Spanish poet Carolina Coronado, Italian liberator Giuseppe Garibaldi, and British and American artists, and visited the family of the young French journalist Georges Clemenceau, as well as the graves of earlier acquaintances Francis Lord Jeffrey and Elizabeth Barrett Browning. In his spare moments Bryant sought solace by beginning the translation of Homer, and Longfellow had found relief after his wife's tragic death by rendering into English Dante's Divine Comedy. Home again in New York, Bryant bought and settled in a house at 24 West 16th Street which would be his city home for the rest of his life. Here he completed major publications, including the Iliad and Odyssey of Homer and an exhaustive Library of Poetry and Song, and added to published tributes to earlier friends, such as Thomas Cole, Fenimore Cooper, and Washington Irving, memorial discourses on Fitz-Greene Halleck and Gulian Verplanck. In addition to his continued direction of the New York Homeopathic Medical college and the American Free Trade League, he was elected to the presidency of the Williams College Alumni Association, the International Copyright Association, and the Century Association, the club of artists and writers of which, twenty years earlier, he had been a principal founder and which he would direct for the last decade of his life. Imagine you suddenly find yourself in the control room of a vast technological apparatus, sometime in the future, where you are told that science has satisfied all the needs of all living humans. Furthermore, you learn, the next generation of the species will not be produced in the usual way, but instead by this machine, provided only that somebody push a little red button. The catch: you have to give a reason for pushing it. You hesitate: what do you say? Our own world is more like this scenario than we at first may be inclined to admit, not least in the fact that, mutatis mutandis, we seem to be struggling to come up with a good answer. The problem, says Rémi Brague, is fundamentally a metaphysical one. Now, mention of ‘metaphysics’ in decent society these days is likely to elicit a smile or an unimpressed shrug. If there is a shelf with that label on it in your typical bookstore you are as likely to find guides to crystals, chakras, or hemp care there as you are treatises by Aristotle, Aquinas, or Kant. And, in spite of the ongoing revival of academic interest in metaphysics, it remains a rather specialist domain, a marginal sub-discipline in departments of philosophy, be they analytical or continental in cast. If you should take it too seriously, you’ll lose your bearings in the real world, and you’ll go adrift in some ethereal sea of dreams. It is, in a word, irrelevant – right? Wrong, Brague writes. Sustained reflection on the nature of being, undertaken in the hope that something can indeed be said about it, was for millennia considered to be among the most important of intellectual pursuits, and not without reason. With his characteristic combination of erudition and wit, Brague takes us on a sweeping tour of the discipline’s varying fortunes, from its early Athenian practitioners through its Jewish, Muslim, and Christian heirs, to the chorus of critics who in the last few centuries succeeded in putting an end to its dominance. But the questions that metaphysics was asking, Brague shows, did not disappear with its demise, and so, whether implicitly or explicitly, metaphysics itself has resisted relegation to the history books. For the nature of being, and especially our relationship to it, has continued to haunt its triumphant critics. One quintessentially metaphysical claim above all, as Brague suggests, seems to have horrified them: the doctrine that all that is, insofar as it is, is good. And yet, in rejecting the “convertibility” of the “transcendentals” of being and goodness, critics of the old metaphysics – Voltaire, Kant, Schopenhauer, Nietzsche, Heidegger, Carnap, and Levinas among them – in their own ways offered metaphysical counter-claims, even as they turned increasingly anthropological in their interests. They also raised the stakes. For, whether the denial of the goodness of being can legitimately be attributed some causal responsibility for a world in which our species could rapidly and deliberately ensure its own extinction, this is the world we live in, and that denial does form the basis of the intellectual background from which we tend to begin our speculations. If we need to be able to articulate reasons for our project not to end, then we also need to rethink the rejection that we have come to take for granted. What Brague offers us here is not a narrative of decline, not a Jeremiad, not a nostalgic lament for the thought-world of a bygone era, but a sympathetic outline of some of the major tensions in the philosophical underpinnings of the modernity that we all inhabit. As such, it forms a part of his ongoing effort take modernity “more seriously than it takes itself”, to expose its hidden foundations, and to push it to its logical conclusions. In so doing, he hopes to help clarify where it is that we are going as a species, and to ensure that wherever it is, there is room for us humans in it.

FROM THE PREFACESince Federal funding is scarce for massive upgrades, and/or complete new Wastewater Treatment Plants (WWTP) construction, treatment plant operators, superintendents, managers, city councils, boards, etc. must get more creative on funding and coordinating process equipment replacements. Contained herein you will find hints, tactics and procedures aimed at getting the "biggest bang for your public buck." During the 1970s and 1980s, through grants, the Federal Government paid 80% of costs to build new or expanded wastewater treatment plants, pumping stations and collection system renovations. The majority of the grants were to upgrade primary treatment facilities to secondary, and secondary to tertiary treatment status based on Clean Water Act regulations. If your facility was fortunate enough to receive grants, you were in good shape for approximately 20 to 30 years (depending on community growth rates). Since most wastewater treatment facilities are designed to last 20 years, many of the new or expanded facilities in the 70's and 80's are reaching the end of their service life. Some may have reached it sooner due to growth beyond the expected rate, inadequate preventive maintenance, or design inadequacies when built. Now you have identified problems with insufficient aeration capacity, equipment mechanical failure, insufficient pump station capacity, infrastructure deterioration, etc. and need to do something about it before you violate your NPDES permit (if you have not already). This equipment seems very costly to replace because you now must pay 100% opposed to 20% with the grants. Many WWTP are in need of replacement and/or upgraded equipment. The equipment itself is typically about 25% to 50% of the total project cost. This cannot be changed much. However, the remaining 50% to 75% (engineering, installation labor costs and project management) may be whittled down dependent on how active and creative the project coordinator (yourself) wants to be in the process. When EPA funded 80% of project costs in prior years, it was no big deal to have an "open pocketbook" attitude. Those days have changed forever and so have procurement procedures for projects. A Management Guide to Retrofitting Wastewater Treatment Plants is geared towards the managerial and administrative scope of a Lead Operator, Superintendent, Facility Manager type of Wastewater individual. All the junior college courses available, practical operator experience, and certification status will still not offer the opportunity to learn administrative and cost savings techniques (similar to operating a business). But soon, your job may demand these skills. This book is a handy reference for making the task of upgrading/retrofitting wastewater process equipment easier and less costly. It includes ideas for selling upgrade ideas to superiors, pre- and post-project activities, and certain management techniques useful for successful retrofitting or upgrading in past projects. This book should prove helpful to those who find themselves involved in retrofitting their facility, and need assistance on resolving facility problems, including treatment plant operators, superintendents, managers, city council members, and boards. It is also a valuable reference guide for municipal operations individuals who want to retain control of their facilities, but don't quite know how. It was written with the front line operator, superintendent, and manager in mind, in common operator language in order to allow easier understanding.
