Nova Science Publishers Inc Spatial Distribution of Health Manpower in China: Exploring its Characteristics and Determinants
The misdistribution of health manpower is one of the major challenges faced by the Chinese health sector hindering people's access to healthcare. However, this subject remains underexplored, as spatial distribution characteristics have not been fully mapped out, nor have its causes been determined at the macro regional level. To fill this research void, this study aims to conduct sophisticated spatial analyses to examine the characteristics of health manpower distribution and test various determinants of health manpower densities. By introducing spatial techniques, this study sheds light on the distribution of health manpower in China, thus offering invaluable evidence for making national and regional policies to facilitate a more effective distribution of health manpower in China. On the one hand, this study used spatial statistics (space-time scan statistics and Moran's I statistics) to reveal, visualize, and compare the temporal trends, spatio-temporal dynamics, and spatial clustering patterns of different subtypes of health manpower (clinicians, doctors of traditional Chinese medicine, dentists, public health doctors, general practitioners, registered nurses, pharmacists, technologists, and interns) and their total. The results reveal that regional disparities and huge urban-rural imbalances exist in China's health manpower distribution, indicating that some provincial units have been left behind. In addition, spatial distribution characteristics varied across different subtypes of health manpower, which still held with respect to either the urban or rural health manpower distribution. On the other hand, this study theoretically modeled and empirically measured various determinants of health manpower distribution from both the supply and demand sides while taking the spillover effect between adjacent geographical units into consideration. Based on the results of the estimation, the health service demand was, as expected, found to forcefully drive the health manpower distribution across the nation. In light of this, the increase in health services demands in a province could significantly help one unit attract licensed doctors and registered nurses from adjacent units. Unexpectedly but intriguingly, the power of medical education capacity was relatively limited in increasing the health manpower density in local units compared to its spillover effect on neighboring units. In addition, government and social health expenditure played different roles in the health labor market, the former being more effective in increasing the stock of licensed doctors and registered nurses, the latter doing better in attracting dentists and general practitioners. This study has the potential to make profound contributions to the theory and practice of health manpower distribution in China. First, this study advances our understanding of characteristics of health manpower distribution by introducing spatial statistics. By doing so, this study visualizes the detailed spatial distribution characteristics with maps and identifies the priority areas for health manpower allocation in China. Second, this study provides a comprehensive research framework to understand the potential drivers of health manpower distribution, and more importantly, takes spillover effects into consideration and measures them for the first time. Third, this study systematically explores and compares the distribution characteristics and determinants of different subtypes of health manpower, which differ greatly. By exploring both the characteristics and determinants, this study gives a full picture of the health manpower distribution and provides new evidence for making health manpower allocation policies in China.

This Annals volume presents three scholarly meeting reports: (1) Dissecting signaling and functions of adhesion G protein–coupled receptors; (2) Scientific considerations for complex drugs in light of established and emerging regulatory guidance; ; and (3) Fetal programming and environmental exposures: implications for prenatal care and preterm birth. G protein–coupled receptors (GPCRs) comprise an expanded superfamily of receptors in the human genome. Adhesion class G protein–coupled receptors (adhesion-GPCRs) form the second largest class of GPCRs. Despite the abundance, size, molecular structure, and functions in facilitating cell and matrix contacts in a variety of organ systems, adhesion-GPCRs are by far the most poorly understood GPCR class. The “6th International Adhesion-GPCR Workshop,” held at the Institute of Physiology of the University of Würzburg on September 6–8, 2012, assembled a majority of the investigators currently actively pursuing research on adhesion-GPCRs, including scientists from laboratories in Europe, the United States, and Asia. The meeting featured the nascent mechanistic understanding of the molecular events driving the signal transduction of adhesion-GPCRs, novel models to evaluate their functions, and evidence for their involvement in human disease. On March 9, 2012, the New York Academy of Sciences brought together experts representing a variety of perspectives—including academic, industrial, regulatory, as well as those from physicians and consumers—to discuss considerations for the non-biological complex drug (NBCD) regulatory approval pathway, given the emerging regulatory guidelines for biosimilars (follow-on biological complex drugs). Plenary sessions addressed the most recent regulatory developments, experimental design, interchangeability, and immunogenicity issues for follow-on versions of complex drugs from the perspective of key audiences, including industry, regulatory agencies, physicians, and consumers. This report summarizes various perspectives on NBCDs and the scientific and regulatory considerations associated with complex drug categories. Sponsored by the New York Academy of Sciences and Cincinnati Children’s Hospital Medical Center, with support from the National Institute of Environmental Health Sciences (NIEHS), the National Institute on Drug Abuse (NIDA), and Life Technologies, “Fetal Programming and Environmental Exposures: Implications for Prenatal Care and Preterm Birth” was held on June 11–12, 2012 at the New York Academy of Sciences in New York City. The meeting, comprising individual talks and panel discussions, highlighted basic, clinical, and translational research approaches, and highlighted the need for specialized testing of drugs, consumer products, and industrial chemicals, with a view to the unique impacts these can have during gestation. Speakers also discussed many other factors that affect prenatal development, from genetics to parental diet, revealing the extraordinary sensitivity of the developing fetus. It’s a fair bet that most of what you think you know about oil prices is wrong. Stalingrad was not only the most-crucial battle on the Eastern Front, it was the main turning point of the whole Second World War in Europe. The Third Reich had suffered setbacks earlier, notably at El Alamein in North Africa in October 1942, but the scale of the fighting on the Eastern Front was incomparably larger than any of the other war fronts and it was the fate of the armies there that decided the outcome of the global conflict. After the demise of the German 6. Armee at Stalingrad in February 1943 it was clear that Nazi Germany would lose the war. This book brings together three After the Battle stories devoted to that historic struggle. It opens with a detailed account of the fight for the city of Voronezh. Lying on the great Don river, it was a prime initial objective of the German summer offensive towards the Caucasus launched on June 28, 1942. Possession of Voronezh would secure an eastern anchor point for a northern defensive line needed for the southward advance to Stalingrad. The city was taken with relative ease in early July but, when the Soviets launched a counter-offensive, the Heeresgruppe S d commander, Generalfeldmarschall Fedor von Bock, allowed his panzer and motorised divisions to be drawn into the protracted fight. This week-long delay which infuriated Hitler severely disrupted the timetable for the main offensive, and fatally contributed to the failure to seize Stalingrad in a surprise raid. The main part of the book is taken up by a comprehensive description of the gargantuan seven-month battle for Stalingrad itself. All stages are described in detail: the advance of the German armies to the city in August, the stubborn and heroic defence of the besieged Soviet 62nd Army against overwhelming German superiority in September-November; and the subsequent encirclement and annihilation of the doomed 6. Armee in the winter, ending in total capitulation on February 2, 1943. Due to the wholesale destruction of the embattled city, it was long thought impossible to apply After the Battle s then and now format to Stalingrad but with the help of a local expert and acknowledged student of the battle, Alexander Trofimov, we managed to match up numerous combat photos taken all over the city, giving full treatment to the months-long struggle for the city on the Volga. The same goes for Voronezh where we found another local expert, Sergey Popov, who achieved equally astounding comparisons. Without them, this book could not have been made. The German catastrophe at Stalingrad, with around 150,000 men killed or succumbing to the winter cold and around 100,000 taken prisoner (of whom only some 5,000 survived captivity), remained a national trauma in Germany. Coming to terms with the event proved difficult, the sorrow over the loss of so many German lives being surmounted by guilt over the fact that Germany had been the aggressor. In many ways, Stalingrad became a taboo, remembered in silence but avoided in public discussion. Illustrative of this is the fact that it took a full 50 years before a major feature film on Stalingrad could be produced in Germany. It was only in 1992 that the German film industry felt the time was ripe and produced and released Stalingrad, the first full-fledged war movie on the battle. We include the story of the making of this film as an epilogue to the main story.

The atheists Daniel Dennett in Breaking the Spell and Richard Dawkins in The God Delusion talk down to believers. Sam Harris in The End of Faith and Letter to a Christian Nation insults believers outright. All three assume that believers are not very bright. Their approach is not productive of much understanding. In The Philosopher’s Enigma, Richard Watson explains to believers in temperate and readable prose why he and many others are not believers. His discussion is based on strict Augustinianism, the foundation of seriously argued Christianity. God is hidden – that is, the concept of God is unintelligible – as discussed at length by Leszek Kolakowski in his Religion If There Is No God (St. Augustine’s Press) – in the sense that there are no known rational arguments for God’s existence. Moreover, Augustine argues that finite human beings cannot understand God’s infinite perfections. Augustine concludes that God has omniscient knowledge of every human being’s behavior, which after all, is predetermined by God prior to His creation of the world. Most difficult to accept, as Calvin later stresses, is the inference that because humans do not determine their own behavior, God predetermines who is saved and who is damned with no reference to this behavior. A foundation of Christianity is that because of the Fall of Man, we are all sinners, and thus there is no reason why God should pick this person for salvation and that one for damnation. But most Christians believe that faith, God’s grace, Jesus’ sacrifice, being born again, and in particular, good works, can earn one salvation. But Augustine and later Calvin see no evidence for these views. Even if, or perhaps even because, God gives a sinner the grace to be good – a person’s good works do not assure salvation. After all, even before God created the world, God predetermined the behavior of every human being. Thus because humans cannot determine their own behavior, they cannot be saved or damned with reference to this behavior.A major difficulty in understanding and accepting the story of the Creation, then, is that even though God determines Adam’s behavior, God punishes Adam for disobedience by decreeing that all Adam’s progeny will be born sinners. Watson begins his book with the steel-trap objections made by his daughter, when she was seven years old, as he read the Bible to her. To the story of the Garden, she objected: “But God made Adam! God made Adam sin! God is not fair!” She slid off his lap, and he had to bribe her to return.In The Philosopher’s Enigma, Watson also discusses in detail the concepts of the soul, angels, ghosts, mind, and body. He argues that the classic Cartesian mind/body problem of how an immaterial mind or soul and a material body can interact will eventually be superseded by a concept of a human being according to which, even though a person’s body/mind is bound by physical laws, it still makes its own considered decisions, and to that extent a human being is free. And because the mind/body is one entity, there is no problem about two different things – a mind and a body-interacting.Watson concludes that this means there is no such thing as a disembodied mind or soul, and so no such things as angels and ghosts that could help or harm you. Basing this discussion in the context of contemporary neurophilosophy, his conclusions about the relationships of mind/soul follow those of Kolakowski in being reminiscent of Spinoza.

The well-known Alzheimer's Disease Neuroimaging Initiative (ADNI) Center provides the most advanced, comprehensive, multiparametric and up-to-date biomarkers for mild cognitive impairment (MCI) and early Alzheimer's disease (AD) projects, including neuroimaging, clinical assessments, biospecimens and genetic data. Recent developments in imaging techniques, including new molecular tracers for imaging disease burden and systematic multi-modal integration, have emerged to overcome the limitations of each single modality and individual-dependent variability. The MRI-based high-resolution structural and morphological changes in the brain, such as atrophy, and the abnormal activity/connectivity patterns of the hippocampus subfields and default mode network (DMN) modulation, together with the amyloid and tau neuropathological quantification using PET molecular tracers, could be used to predict brain changes and cognitive performance declines in early AD, including transitional MCI. Finally, a generalized and integrative model with multiple biomarkers could be built to target disease progression and symptom prediction as well as to optimize patient management. Multiomics investigates metabolomic, lipidomic, genomic, transcriptomic and proteomic perspectives by presenting an accurate biochemical profile of the organism in health and disease. The Alzheimer's Disease Metabolomics Consortium (ADMC) in partnership with ADNI is creating a comprehensive biochemical database for patients in the ADNI1 cohort, consisting of eight metabolomics datasets. The vast majorities of biospecimen data provide rich biological information to the human brain at normal and dementia status. One of the purposes is to reveal the connections between disease and multiomics such as obesity, hypertension, cholesterol imbalance and inflammation risks that might lead to neurodegenerative disease. Multiomic biomarker developments in the dementia field have provided earlier clues to novel treatments that help correct metabolic dysfunction and delay disease progression. Furthermore, the assembling of multiomics-based biomarkers including metabolites and lipids, cholesterol biosynthesis, purine metabolism, lipoprotein, bile acids, and genetics as well as their relation to the pathological amyloid and tau network could improve disease diagnosis sensitivity and reveal more diverse and complementary molecular pathways to allow for the advancement of early AD diagnosis and therapeutic prevention. In this book, we report on the significant differences of multiple biomarkers from the ADNI database including neuroimaging, clinical assessments and multiomic biospecimen/genetic data in MCI and early probable AD (pAD), and elucidate the interconnections among different metrics at various domains. Classification results with high accuracies (0.95-1) for each early dementia subtype including early MCI (EMCI), late MCI (LMCI) and pAD, and better prediction of clinical symptoms is achieved with these comprehensive biomarkers. Further longitudinal changes of imaging and neuropsychological biomarkers, and inter-correlations with baseline parameters are examined for a better illustration of disease progression association. Additionally, an analysis of the post-traumatic stress disorder biomarkers is performed with high classification accuracy. With illustrative and rigorous data analyses and confirmative results, this book provides readers with a full spectrum of biomarker research for early dementia diagnosis and treatment, and helps convey the technical development and data evaluation perspectives in advanced medical imaging and various disease application fields.

Nova Science Publishers Inc Surgical Treatment of Femoral Neck Fractures (CD Included)
While arthroplasty is the preferred treatment for most elderly patients with displaced femoral neck fractures, internal fixation is the treatment of choice in the majority of patients below the age of 65 as a joint-preserving procedure. The osteosynthesis of fractures of the femoral neck in the elderly has been partly abandoned during the last years due to the poor clinical outcomes following the conventional fixation with parallel screws or DHS. Based on clinical evidence and laboratory testing, the novel method of biplane double-supported screw fixation (BDSF) offers much better fixation stability, reflecting in excellent clinical outcomes. With its innovative biomechanic principle, the BDSF method provides supreme stability for cannulated screw fixation, achieving up to 44% higher axial fixation strength in vitro, and a rate of bone union reaching up to 96.6% in clinical practice, which is much higher than the conventional parallel screw fixation data. The method of BDSF provides supreme stability by buttressing two out of three medially diverging cannulated screws on the inferior femoral neck cortex and supporting the steeper inferior screw on the posterior femoral neck cortex. The two calcar screws are oriented in different coronal inclinations intended to provide constant fixation strength during different patient activities and load directions. Biomechanically, the most effective component is the inferior screw placed at an obtuse angle and supported on a large area along the inferior and posterior cortex of the femoral neck following its spiral anterior curve. Given the clinical outcomes, BDSF is the perfect technique for femoral neck fracture fixation, as the fracture healing rate is high at 96% with this approach. Therefore, BDSF is not only a treatment alternative to conventional fixation, but also a much better procedure. Thus, BDSF should be routinely applied, and conventional fixation gradually abandoned in clinical practice (this has been the approach in our institution over the last ten years). This book describes the full surgical technique of the method of BDSF for femoral neck fracture osteosynthesis; quality criteria and surgical recommendations for successful BDSF implementation, according to the vast clinical experience of ten years with this highly effective method. A novel surgical approach for hip arthroplasty is described in this book. The current trends aimed at decreasing operative trauma and blood loss have been not entirely satisfied with respect to most of the standard approaches for hip arthroplasty. These surgeries are often associated with considerable blood loss and the necessity for restricting patients' activities in the postoperative period due to impaired joint stability and risk of dislocations. This book describes the full surgical technique of the novel anatomical direct lateral approach for hip arthroplasty, aimed at decreasing blood loss, minimizing operative trauma, and optimizing joint stability. This technique is associated with minimal blood loss and high joint stability. Patients are allowed to perform activities within the normal range of motion in the early postoperative period. This book describes also the history of internal fixation in femoral neck fractures, as well as the biomechanics of femoral neck fracture osteosynthesis and the role of the implants.

A rich examination of the securitization of the everyday lives of the citizens of Cairo and how to build a more equitable urban orderUntil the year 2000, Cairo had been a model megacity, relatively crime free, safe, and public facing. It featured a thriving public culture and vibrant street life. In recent decades, however, the Egyptian state has accelerated a wholesale dismantlement of public education and public sector jobs and reversed the modest land reforms of the Nasser era. St Augustine's Press Homo Americanus – The Rise of Totalitarian Democracy in America
What is the man who cannot be known apart from his socio-political environment? As Zbigniew Janowski asserts, one does not ask who this man is, for he does not even know himself. This man is suppressed and separated, and not by Fascism or Communism. In present-day America this has been accomplished by democracy. “Only someone shortsighted, or someone who values equality more than freedom, would deny that today’s citizens enjoy little or no freedom, particularly freedom of speech, and even less the ability to express openly or publicly the opinions that are not in conformity with what the majority considers acceptable at a given moment. It may sound paradoxical to contemporary ears, but a fight against totalitarianism must also mean a fight against the expansion of democracy.” Janowski all at once brazen and out of bounds states what he calls the obvious and unthinkable truth: In the United States, we are already living in a totalitarian reality. The American citizen, the Homo Americanus, is an ideological being who is no longer good or bad, reasonable or irrational, proper or improper except when measured against the objectives of the dominating egalitarian mentality that American democracy has successfully incubated. American democracy has done what other despotic regimes have likewise achieved––namely, taken hold of the individual and forced him to renounce (or forget) his greatness, pursuit of virtue and his orientation toward history and Tradition. Homo Americanus, Janowski argues, has no mind or soul and he cannot tolerate diversity and indeed he now censors himself. Democracy is not benign, and we should fear its principles come by and applied ad hoc. It is deeply troublesome that in the way democracy moves today it gives critics no real insight into any trajectory of reason behind its motion, which is erratic and unmappable. The Homo Americanus is an ideological entity whose thought and even morality are forbidden from universal abstraction. Janowski mounts the offensive against what the American holds most sacred, and he does so in order to save him. After exposing the danger and the damage done, Janowski makes another startling proposal. It is a “diseased collective mind” that is the source of this ideology, the liberal anti-perspective that presses man into the image of the Homo Americanus, and its grip can only be broken through the recovery of instinct. Homo Americanus cannot be free again until he is himself again. That is, until the shadow that belongs only to him is restored, and he is thereby no longer alienated from others. Despite the condemnation Janowski seems to be levying on the citizen of the United States, he betrays a great hope and confidence that the means to shake ourselves awake from the bad dream are nevertheless in hand. Janowski’s work is the next title in St. Augustine’s Press Dissident American Thought Today Series. It occupies a controversial overlapping terrain between the philosophical descriptions of liberalism as a tradition, psychology and the fundamentally influential critiques of democracy offered by Thucydides, Jefferson, Franklin, Tocqueville, Mill, Burke and more. More anecdotal than analytical, Janowski offers the contemporary proof that the reader is right to be scandalized by democracy and his or her own likeness of the Homo Americanus. Once upon a time it was the despicable Homo Sovieticus fruit of tyranny, but now we fear democratic society too might fall and all its citizens never be found again.
