Many healthcare professionals that I've encountered refer to a patient's cancer journey; indeed I had an article published describing a patient's journey through biological therapy. This particular case history described my involvement with a medical doctor's experience of renal cell carcinoma and self-administration of immunotherapy. He was very keen to share his experience to educate and inform other health professionals and I now find myself in a similar situation.'I have 'journeyed' from nurse to patient when diagnosed with grade 3-breast cancer. Of course, I've experienced various health problems before this time, but I never spent a night in hospital, unlike members of my family who have experienced serious illness and hospitalisation on several occasions. Consequently I've always regarded myself in the caring role amongst family members and over the years I've used my nursing knowledge and skills to help them deal with both the physical and psychological issues of their various disease processes. However, being diagnosed with breast cancer has now changed me from nurse and carer into a vulnerable ill person with doubts and fears for my future. To restore some sense of purpose while on sick leave and help me to get through each treatment stage, I decided to keep a daily journal of my experience.

Narrative medicine has developed an identity already. Clinicians of many disciplines are being summoned to a practice that recognizes patients by receiving their accounts of self. Starting from different positions, the four authors have converged in a strong and shared commitment to narrative health care. They conceptualize narrative health care practices within frameworks derived from the social sciences and psychology, and, to a lesser degree, phenomenology and autobiographical theory. An area of forest the size of England is cut down in the tropics each year. Forestry is responsible for a fifth of global carbon emissions - more than the entire world transport sector. Urgent action to tackle the loss of global forests needs to be a central part of any new international agreement on climate change. Climate Change: Financing Global Forests is an independent report commissioned by the UK Prime Minister to address this vitally important issue. It assesses the impact of global forest loss on climate change and explores the future role of forests in the international climate change framework, with particular emphasis on the role of international finance. It also looks at the economic and policy drivers of deforestation and describes the incentives required to ensure more sustainable production of agriculture and timber in order to meet global demand while reducing carbon emissions. The report draws on a wide range of international expertise and will have significant national, EU and international interest and influence. It includes new modelling and analysis of the global economic impact of continued deforestation and provides a comprehensive assessment of the opportunity and capacity-building costs of addressing the problem. It shows that the benefits of halving deforestation could amount to $3.7 trillion over the long term. However, if the international community does not act, the global economic cost of climate change caused by deforestation could amount to $12 trillion. The aim of this book is to present the potential benefits as well as the challenges of introducing a more formal economic regulatory process into the urban water sector arena in lower-income countries. There is a particular focus upon the impact this may have on the poorest, the informal, slum and shanty dwellers of the rapidly growing cities. Economic regulation, usually introduced in the context of private operation of monopoly water supply, can deliver objectivity and transparency in the price-setting process for public as well as private providers. The book describes and analyses these issues through a consideration of ten country case studies. As a starting point, the current situation for the provision of water and sanitation services for the poorest through non-regulated public providers in India and Uganda is reviewed. Comparative chapters are then presented on Ghana, Philippines, Bolivia, Jordan, Zambia and Indonesia, all with varying degrees of private sector involvement and regulation. Finally the experiences of two richer countries are considered - Chile and England, countries with the longest experience of economic regulation and the 'most privatized' suppliers. In all cases there is a focus on the very necessary role of customer involvement in price-setting and service monitoring and on the role of alternative (private) service providers.

Low external-input technology (or LEIT) is an increasingly prominent subject in discussions of sustainable agriculture. There are growing calls for self-sufficient agriculture in an era experiencing diminishing returns from reliance upon expensive synthetic pesticides and fertilizers. There are many reasons to support strategies for low external input farming, including a concern for environmental sustainability, increased attention to resource-poor farmers and marginal environments, and the conviction that a better use of local resources in small-scale agriculture can improve farm productivity and innovation. But despite the increased attention to self-sufficient agriculture, there is little evidence available on the performance and impact of LEIT. This book examines the contributions and limitations of low external input technology for addressing the needs of resource-poor farmers.
