Dramatic changes to the structure of health systems since the 1980s has seen the development of large, integrated health organizations designed to provide scale and scope advantages, improve the quality of care and health outcomes, and provide greater bargaining power relative to payers and large employers. This transition has led to greater interest in understanding hospitals and health systems as complex systems. Two important themes emerged from this effort: 1. Creation and organization of physician-health organizations, and alignment of these organizations with hospital or system structure; and 2. Viewing health care organizations as complex systems, leading to new perspectives on design and management of these organizations. In Volume 15, Reuben McDaniel and L. Robert Burns, authors of two influential articles on these themes from earlier volumes of AHCM, revisit the evolution of health systems organization in light of regulatory and organizational evolution in health care, including the Patient Protection and Accountable Care Act of 2010, and increasing consolidation of health systems. The aerial clashes between the iconic Corsair and Zero-sen translated into a contest of speed and altitude for the former, versus the latter’s outstanding agility and range. Whilst the F4U Corsair eventually proved to be a superior fighter in Pacific operations, its introduction into combat in this theatre initially demonstrated its weaknesses. Indeed, the ‘Saint Valentine's Day Massacre’ debacle showcased exemplary Zero-sen fighter tactics, and American losses were of sufficient magnitude that further daylight missions toward Bougainville were discontinued until Allied fighter tactics could be improved. As a result, for the next two months the Corsair’s combat results were much subdued. Indeed, the F4U only became a superb fighter when both its pilots and their commanders worked out how to deploy the gull-wing design effectively. Optimum circumstances for effective engagement did not always occur, and the Zero-sen remained effective against the Corsair until February 1944 in the South Pacific, after which all IJNAF fighter units vacated Rabaul. This book closely examines these two different fighters in the Solomons/Rabaul theatre, and the unique geographic conditions which shaped their deployment and effectiveness. It contains rare photographs and digital artwork that accurately showcases and aligns combats of both types in-theatre with unprecedented accuracy. Both sides vastly over-claimed. With full access to IJNAF and US Navy/US Marine Corps records, these numbers will be presented accurately.

This book details the exploits of the highly skilled Naval Aviators charged with achieving air supremacy over New Guinea in their A6M2/3 Zero-sens. The combat record of the Zero-sen in New Guinea has mostly been overstated, with little due being given to the constraining conditions under which the fighter operated. The air combats fought over New Guinea in 1942 between Imperial Japanese Naval Air Force (IJNAF) pilots and their Allied counterparts in P-39 Airacobras and P-40 Warhawks were often ‘trial and error’ affairs, with both belligerents being caught out by weather. This study covers the key role played by governing factors including geography and climatic conditions, and examines the modified tactics employed by IJNAF Zero-sen pilots to help them cope in-theatre through the comprehensive analysis of RAAF, USAAF and Japanese operational after action reports. Using first-hand accounts from both famous aviators and previously unknown RAAF and Japanese pilots, and specially commissioned artwork, leading South Pacific historian and author Michael John Claringbould sheds new light on the air war fought over the wilds of New Guinea during the course of 1942.

Despite their regularity, such accounts treat each case as unprecedented and unique. This book proposes a radical rethinking of the history, present and future of our relations with the digital, spatial technologies that increasingly mediate our everyday lives. From smartphones to surveillance cameras, to navigational satellites, these new technologies offer visions of integrated, smooth and efficient societies, even as they directly conflict with the ways users experience them. Recognising the potential for both control and liberation, the authors argue against both acquiescence to and rejection of these technologies. After the failed April 1972 invasion of South Vietnam and the heavy US tactical bombing raids in the Hanoi area, the North Vietnamese agreed to return to the Paris peace talks, yet very quickly these negotiations stalled. In an attempt to end the war quickly and 'persuade' the North Vietnamese to return to the negotiating table, President Nixon ordered the Air Force to send the US’ ultimate conventional weapon, the B-52 bomber, against their capital, Hanoi. Bristling with the latest Soviet air defence missiles, it was the most heavily defended target in Vietnam. Taking place in late December, this campaign was soon dubbed the ‘Christmas Bombings’. The Vought Corsair was the first American single-engined fighter to exceed 400 mph and establish dominance over the legendary Mitsubishi Type Zero-sen. The Ki-84 Hayate was introduced by the Japanese specifically to counter this growing American dominance of the skies over the Pacific. Built in greater numbers than any other late war Japanese fighter, nearly 3000 were completed between 1944 and 1945. This volume examines the clashes between the Corsair and Ki-84 in the closing stages of the war, revealing how Corsair pilots had to adapt their techniques and combat strategies to adapt to these newer types. It also reveals how the kill rate was largely driven by the reduced quality of fighter pilots after the high casualty rates inflicted on the Japanese air force during the air battles over the Solomon Islands.

'A la Recherche du Temps Perdu' by Marcel Proust is a magnificent and towering achievement of French literature, published in English translation in twelve volumes between 1922 and 1931, and totalling nearly one and a half million words. It is difficult for many readers to find the time and motivation needed to keep going through to the end as Proust has a rich and sometimes discursive style. However, a powerful structure underlies the whole work which is not revealed till the last chapter. This selection, which is less than a quarter of the original, includes all the crucial characters, places and themes needed to understand this and omits everything else. It is not intended to improve the original novels by editing, as many wonderful passages and descriptions have been left out. The style of the book in its vocabulary and sentence structure is unaltered. Nothing is paraphrased or condensed. The text reads as a continuous lively narrative with much of Proust's wit and humour and follows the sequence of the original, showing the development of all the major characters and including all the incidents which are referred to in the closing passages which resolve the whole novel. Proust himself encouraged the publication of "selected passages showing a coherent whole which is not diffused and will make one want to read the whole book" and this is the aim of this selection. It is intended to inspire the reader to go to the full text and enjoy individual sections in the knowledge of where they fit into the whole.

With first-hand insight into the key role of the US Air Force's fighter-bomber from the Vietnam War through to Operation Desert Storm during the First Gulf War, this book is an unmissable account of some of the most dangerous and demanding missions in the two wars. The advent of the surface-to-air missile (SAM) in the early 1950s threatened the whole concept of aerial bombing from medium and high altitude. Countermeasures were developed during the Korean War, but with little initial success. It was only in the closing stages of the Vietnam War, with the F-4Cww Phantom II (Wild Weasel 4), that this equipment started to become successful enough to allow a substantial investment in converting 116 F-4E Phantom IIs into dedicated SEAD aircraft. This move introduced a new generation of anti-radar missiles which became invaluable in later operations including operations Desert Shield, Desert Storm and Northern Watch over Iraq. This volume features dynamic archival photography from crews who flew the jet, alongside mission accounts and technical details of the development and fielding of the F-4 Wild Weasel in its various iterations. The volume opens with an essay on the power of naming in creating early modern subjectivities, followed by a pair of provocative discussions of Shakespeare's plays:the first addresses temporal gaps in A Winter's Tale; the second is a reading of misogyny in The Taming of the Shrew in which Petruchio is no longer seen as "the true tamer."
