Medicine itself is sick. We hardly understand any disease and therefore need to chronically treat symptoms but not the causes. Lehigh University Press Raising More Hell and Fewer Dahlias: The Public Life of Charlotte Smith, 1840-1917
This book is the first biography of nineteenth-century magazine editor and reformer Charlotte Smith. Based on years of research, and previously untapped sources, it shows both why she should be remembered and why she was forgotten. Her story is quintessentially American: this daughter of Irish immigrants, despite having only a grade-school education and supporting two children alone, became a force to be reckoned with, first in journalism and then in reform. Her first periodical, the Inland Monthly, was doubly rare: edited by a woman but not a women's magazine; and a profitable venture, bringing a large sum when sold. If Smith had stopped there, she wold still deserve recognition, but she was just beginning. Not satisfied with just regional fame, she moved to Washington, D.C. in 1878. There she soon gained the ear of a powerful senator and the respect of many others in Congress through her unfailing attendance at its sessions and hearings, her undercover investigations of working conditions, and, after 1882, her all-female union. The Librarian of Congress praised her pioneering expertise in labor statistics. Smith became a tireless advocate for working women, women inventors, prostitutes, the poor or less fortunate in general, and many individual underdogs as well. She also worked for public health and safety, demanding things we take for granted today, such as bans on adulteration, and ingredient lists on product labels. By 1891 she was already credited for fifty bills passed by Congress. By the 1880s Charlotte Smith was nationally famous, with frequent coverage in the New York Times, the Washington Post, and newspapers as distant as Montana and Hawaii. In 1886 she founded her second periodical, the Working Woman. Again, if her resume contained only her successful lobbying and her reform campaigns in Washington, Charlotte Smith would deserve mention in American history. But in the late 1880s she engineered the compilation of a list of American women patentees, and in 1891 founded her third periodical, the Woman Inventor. She continued her crusades until her death in 1917. These achievements (and others too numerous to mention here) make Smith's omission from American history mysterious indeed. This book suggests several explanations, including historians' traditional adeptness at "Vanishing the Lady." Perhaps most persuasive is the suggestion that Charlotte Smith's ideas were forgotten because they were too far ahead of their time. For example, her prostitution reforms—including regulation, medical care, and retraining programs—have yet to be adapted in much of the world. All those who like a good mystery, as well as those who delight in seeing long overdue credit given at last, should enjoy this book.

A tiny African-American baby lies in a hospital incubator, tubes protruding from his nostrils, head, and limbs. \u0022He couldn't take the hit,\u0022 the caption warns. \u0022If you're pregnant, don't take drugs.\u0022 Ten years earlier, this billboard would have been largely unintelligible to many of us. But when it appeared in 1991, it immediately conjured up several powerful images: the helpless infant himself; his unseen environment, a newborn intensive care unit filled with babies crying inconsolably; and the mother who did this -- crack-addicted and unrepentant. Misconceiving Mothers is a case study of how public policy about reproduction and crime is made. Laura E. Gomez uses secondary research and first-hand interviews with legislators and prosecutors to examine attitudes toward the criminalization and/or medicalization of drug use during pregnancy by the legislature and criminal justice systems in California. She traces how an initial tendency toward criminalization gave way to a trend toward seeing the problem of \u0022crack babies\u0022 as an issue of social welfare and public health. It is no surprise that in an atmosphere of mother-blaming, particularly targeted at poor women and women of color, \u0022crack babies\u0022 so easily captured the American popular imagination in the late 1980s. What is surprising is the was prenatal drug exposure came to be institutionalized in the state apparatus. Gomez attributes this circumstance to four interrelated cause: the gendered nature of the social problem; the recasting of the problem as fundamentally \u0022medical\u0022 rather than \u0022criminal\u0022; the dynamic nature of t he process of institutionalization; and the specific feature of the legal institutions -- that is, the legislature and prosecutors' offices -- the became prominent in the case. At one level Misconceiving Mothers tells the story of a particular problem at a particular time and place -- how the California legislature and district attorneys grappled with pregnant women's drug use in the late 1980s and early 1990s. At another level, the book tells a more general story about the political nature of contemporary social problems. The story it tells is political not just because it deals with the character of political institutions but because the process itself and the nature of the claims-making concern the power to control the allocation of state resources. A number of studies have looked at how the initial criminalization of social problems takes place. Misconceiving Mothers looks at the process by which a criminalized social problem is institutionalized through the attitudes and policies of elite decision-makers.

This dictionary deals with the little words which everyone uses every day without even noticing them and which are so liable to escape the grasp of a speaker of another language - clipped words, obtained by back-clipping or apocope (the dropping of one or more final syllables), by front-clipping or aphesis (the dropping of one or more final syllables), by the combination of these two processes and sometimes further transformed, especially in slang, by the addition of a new ending. The aim is not of course merely to list all these words and say from which longer words they were obtained; an attempt is made here to retrace the history of each of them, its stylistic, semantic and often morphological evolution, to illustrate this with authentic and often pungent or humorous quotations and also to show how each can be translated into the other language. Indeed this is an English-French bilingual dictionary, whose aim is to translate clipped words according to priciples of historical and register fidelity which bilingual dictionaries do not ordinarily set for this type of headword. French Provincial Cooking should be approached and read as a series of short stories, as well written and evocative as the best literature. The voice is highly personal and opinionated, sometimes sharp but always true and always entertaining. Here is a long essay on French cuisine, offering background stories and sketches of recipes more than the slavishly didactic type of recipes that most modern readers might be used to today. For many Elizabeth David was the first to introduce us to the French notion of la cuisine terroir, sometimes interpreted as `what grows together goes together'. For David, this is the heart of regional cooking, and the thing which most distinguishes it from cooking in haute cuisine restaurants where diners arrive at any time or any season and expect to be able to order any well known French speciality. One of the passages which best characterizes David's approach to a lot of cooking is her opening statement on the perfect omelette: `As everybody knows, there is only one infallible recipe for the perfect omelette: your own.' The book starts with a short essay on each of the major culinary regions of France, starting perhaps not surprisingly with Provence which is blessed with an abundance of produce. The largest portion of the book consists of chapters on cuisine by type of dish: Sauces, Hors-D'oeuvres and Salads, Soups, Eggs and Cheese, Pates and Terrines, Vegetables, Fish, Shellfish, Meat, Composite Meat Dishes, Poultry and Game and Sweet dishes. The book is all the more valuable in that it paints a picture of a cooking style which existed before modern equipment such as the food processor. Most importantly, the recipes work if your aim is to produce the most excellent food imaginable. What initially may seem to be annoying details (e.g., for omelettes, eggs should not really be beaten at all, but stirred, whereas for scrambled eggs, they should be very well beaten) are actually secrets to be treasured, that elevate a good dish to a superb one. The lesson is that good food should be done simply, but it takes care, attention to detail, and frequently, time. A hardback edition of French Provincial Cooking has been unavailable for many years and Grub Street is re-issuing it because of overwhelming demand. It should become as popular an edition as the best-selling Elizabeth David Classics.

This volume, the original version of which was published in 1988, brings to a close the autobiographical writings of a modern Christian philosopher who lived through the two World Wars and the ecclesiastical upheaval in the Catholic Church in the context of the Second Vatican Council. What stamps this philosopher throughout the course of his life – with all its social and political uncertainties – is his constant dedication to truth and his manifest unswerving integrity. Themes with which the reader of his previous works would be well acquainted recur in this volume. The dedicated Catholic philosopher, who preferred his independence as a trainer of teachers to the less independent role of a professor in a Catholic university, was quite prepared to criticize developments in the Church which resulted from Vatican II. In his defense of the sacred, which he deemed threatened by popularizing trends in the Church, he criticized what he saw as the watered down language in modern German translations of Church liturgical texts; the growing preference for secular garb; and the compromising developments which saw the sacramental signs – surrounding baptism, for instance – being reduced to such an extent that they no longer had the power to signify their sacred meaning even to a well-intentioned congregation. A great lover of the philosophy of Plato, Augustine, and Aquinas – among many others –, Pieper highlighted the need for living a life of truth. He did not consider truth to be merely something abstract but as something to be lived existentially. While he could explain his philosophy in clear rational terms, something which especially stood to him in his post-war lectures to eager students who were hungry for intellectual guidance and leadership, the great interest of his philosophy was, possibly, his preoccupation with mystery – that which impinges on our inner lives but frustrates all our attempts to account for it in purely rational terms. As a philosopher – one might say a Christian philosopher – Pieper seems to have observed the traditional boundaries drawn between philosophy and theology. His generation was exposed to the modernist debates in the Church. It would have been deemed heretical to say that the Divine could be grasped by our purely human thought processes – access to the Divine being only possible through faith and grace. Pieper was no heretic. But he was also not altogether conservative. In fact, his philosophy, closely allied to existentialism – despite his care, for instance, to distance himself from the negative existentialism of Sartre – focused on the individual’s inner existential grasp of the most profound reality. Truth is to be found within us, even if it remains a mystery. What lies beyond death is, for the individual, the ultimate mystery.

Tricuspid valve disease has long been ignored as a surrogate for the left atrioventricular valve (the mitral valve) and mainly a benign, indolent disease. This concept is currently outdated, as we know that the pathology of the tricuspid valve, primitive or secondary, sometimes isolated, brings with it a poor prognosis if not treated. To date, the complexity is recognized not only of the valve itself, but also of its pathophysiology and the environment of the right heart and its interaction with the left heart. In order to better define the therapeutic path of a patient suffering from tricuspid valve disease, we have tried to offer a comprehensive overview to the reader, starting from historical considerations (Chapter 1) about the vision of the circulatory system and from the evidence accumulated over the centuries until the recognition of the continuum between signs and symptoms related to the valve. As the basis of a modern approach to the understanding of the valve itself, we have traced the embryological and morphogenetic stages towards the anatomo-clinical correlates (Chapter 2). Subsequently, the epidemiology and clinical approach to tricuspid valve disease (Chapter 3) were illustrated, followed by the diagnostic procedure, state-of-the-art imaging techniques (Chapter 4) and more advanced techniques (Chapter 5). In particular, I thought it was necessary to dedicate a chapter to the pathology of the tricuspid valve secondary (functional) to left heart disease (Chapter 6), probably the most frequent form encountered by the clinicians. We have not neglected to discuss about tricuspid valve infections (Chapter 7), with particular reference to the microbiology, diagnosis and treatment. We considered it necessary that the role of the valve in congenital adult disease should also be addressed (Chapter 8), whether in natural history, modified by surgery or as an acquired disease. Neoplastic pathology may also involve the valve and the pathologist provides (Chapter 9) the knowledge and systematization of the most frequent primary and secondary tumors. Coming to the therapeutic act, after dealing with special pathology of the valve, (Chapter 10) we present the pharmacological approach, a necessary act, sometimes as a sole therapeutic option and sometimes propedeutic to surgery. Tricuspid valve plays an absolutely primary role in two aspects of the advanced treatment of refractory heart failure, namely its clinical and prognostic impact on ventricular care systems (LVAD) and cardiac transplantation (Chapter 11) has been discussed. The last two chapters are dedicated to non-medical treatment, with a particular focus on indications, risk selection and stratification as well as adequate surgical techniques (Chapter 12) and new perspectives of intervention through transcatheter techniques (Chapter 13). Overall, there is a need for an integrated and multidisciplinary approach to a complex nosological entity that is itself interdependent on pathophysiological systems, which the clinician, cardiologist, cardiac surgeon, internist and researcher cannot ignore.
