The City at Stake tells the dramatic story of how the nation's second-largest city completed a major reform of its government in the face of a deeply threatening movement for secession by the San Fernando Valley. How did Los Angeles, a diverse city with an image of unstructured politics and fragmented government, find a way to unify itself around a controversial set of reforms? Los Angeles government nearly collapsed in political bickering over charter reform, which generated the remarkable phenomenon of two competing charter reform commissions. Out of this nearly impossible tangle, reformers managed to knit a new city charter that greatly expanded institutions for citizen participation and addressed long-standing weaknesses in the role of the mayor. The new charter, pursued by a Republican mayor, won its greatest support from liberal whites who had long favored reform measures. Among the standard oncology modalities, surgical oncology is singular in that it lacks a separate board certification or even an added qualification mecha­ nism. 'Card-carrying' surgical oncologists are certified by the American Board of Surgery, as are all other general surgeons. What distinguishes the surgical oncologist is a set of cognitive skills rather than a specific armamentarium of surgical techniques. This different conceptual framework is derived from ex­ tensive additional training that leads to an in-depth understanding of the natural history and biologic behavior of the various solid tumor systems. Equipped with this perspective, the surgical oncologist is particularly well positioned to integrate the various available therapeutic modalities into a coherent care program for the solid tumor patient. As a central theme, the chapters of this book demonstrate that increasingly sophisticated diagnostic and staging approaches are helping to move chemo­ therapy and radiotherapy into the preoperative neoadjuvant setting. This fundamental alteration is based on the awareness that even early-stage solid tumor disease is frequently systemic at the time of presentation, at least on a subclinical level. And although the primary tumor may be controllable by surgery with radiotherapy, the uncontrolled (and initially clinically unappar­ ent) distant disease ultimately determines patient survival. The other perspec­ tive driving the neoadjuvant approach is an emerging awareness that for most solid tumor systems, neoadjuvant treatment responses can facilitate less muti­ lating surgery with comparable levels of local disease control.

The first monographic publication in English on German Expressionist artist and architect Paul Goesch, who long struggled with—and was persecuted and ultimately murdered for—his schizophrenia Paul Goesch (1885–1940) produced one of the most inventive, peculiar, and poignant bodies of work to emerge from Weimar Germany. An artist and architect, he made both fanciful figurative drawings and visionary architectural designs. The latter, from the extensive holdings of the Centre for Canadian Architecture in Montreal, are the focus of this publication, the first in English dedicated to Goesch. Amid the aftermath of First World War, a generation of young architects sketched their visions for utopia. Goesch stands out among them for his formal range, his kaleidoscopic color sense, and his playful and pluralistic embrace of architectural history, as well as for his long struggles with schizophrenia, a condition for which he was institutionalized and ultimately murdered by the Nazis. This publication highlights the decorative portals and archways that predominate in Goesch’s work. These represent the artist’s metaphysical passages, as a spiritualist steeped in diverse religious and esoteric beliefs, and his altered psychological states. They also suggest Goesch’s liminal status between art and architecture, “sanity” and “madness,” the trained insider and the institutionalized “outsider.” Celebrated in his time and since forgotten, Goesch is presented here in the context of period discussions on art, architecture, and mental health. Much of Mark Klett's (born 1952) work as a photographer has entailed conversations with historical images. For this project, Klett worked only with the account of a young mining engineer named Raphael Pumpelly who wrote of his perilous journey through Arizona and Mexico in 1861 on the lawless Camino del Diablo or "road of the devil." More than 150 years later, Klett traversed the same route, making photographs in response to Pumpelly's words. Today, most of the Camino is located on the Barry M. Goldwater Bombing Range and the border is a militarized zone patrolled by government agents and crisscrossed by air and ground forces practicing for war. Unable to trace the engineer's exact steps, Klett created images that are not literal references to specific places or events; rather, he sought to produce a more poetic narrative of their shared experience of the Arizona desert.
