Chapter 1 examines the actions of drug companies in raising prescription drug prices in the United States, as well as the effects of these actions on the Federal and state budgets, and on American families. Chapter 2 addresses frequently asked questions about government and private-sector policies that affect drug prices and availability. Among the prescription drug topics covered are federally funded research and development, regulation of direct-to-consumer advertising, legal restrictions on reimportation, and federal price negotiation. The 340B Drug Pricing Program requires drug manufacturers to sell outpatient drugs at discounted prices to covered entities-eligible clinics, hospitals, and others-in order to have their drugs covered by Medicaid. Covered entities are only allowed to provide 340B drugs to certain eligible patients. Chapter 3 reviews the Health Resources and Services Administration's (HRSA) oversight of the 340B Program to ensure compliance with program rules. In 2017, nearly 60% of U.S. adults aged 18-64 reported being prescribed medication in the past 12 months. Approximately 70% of prescription medications carry out-of-pocket costs. Strategies to reduce prescription drug costs at the individual level are discussed in chapters 4 and 5. Each year, Americans pay more for prescription drugs, and rising drug prices have a disproportionate impact on older Americans. Chapter 6 examines the history of rising drug prices for the brand-name drugs most commonly prescribed for seniors. Generic drugs-copies of brand-name drugs-lead to significant cost savings. Before a generic drug can be marketed, FDA must approve the generic drug application. According to FDA, applications go through an average of three cycles of review before being approved, which may take years. Chapter 7 examines 1) the first review cycle approval rate of generic drug applications in recent years and factors that may have contributed to whether applications were approved; and 2) changes FDA has made to increase the first review cycle approval rate.

Geoffrey le Baker's chronicle covers the reigns of Edward II and Edward III up to the English victory at Poitiers. It starts in a low key, copying an earlier chronicle, but by the end of Edward II's reign he offers a much more vivid account. Baker's description of Edward II's last days is partly based on the eyewitness account of his patron, Sir Thomas de la More, who was present at one critical interview. This story of Edward's death, like many other details from his chronicle, was picked up by Tudor historians, particularly by Holinshed, who was the source for Shakespeare's history plays. The reign of Edward III is dominated, not by Edward III himself, but by Baker's real hero, Edward prince of Wales. His bravery aged sixteen at Crécy is presented as a prelude to his victory at Poitiers, a battle which Baker is able to describe in great detail, apparently from what he was told by the prince's commanders. It is a rarity among medieval battles, because - in sharp contrast to the total anarchy at Crécy - the prince and his staff were able to see the enemy's manoeuvres. Throughout the chronicle there are sharply defined vignettes which stay in the mind - the killing of the Scottish champion on Halidon Hill, the drowning of Sir Edward Bohun, the earls of Salisbury and Suffolk as prisoners carried in a cart, the death of Sir Walter Selby and his two sons, the bravery of Sir Thomas Dagworth against a cobbler's son, the duel between Otho and the duke of Lancaster, John Dancaster and the lewd washerwoman. Baker writes in a complex Latin which even scholars find problematic,and David Preest's new translation will be widely welcomed by anyone interested in the fourteenth century. There are extensive notes and an introduction by Richard Barber. The composition of the fourth book is in no way inferior to that of the other four books already analysed. Its three major sections, each containing five psalms, are organised in a concentric fashion; they are linked by two short sections which constitute the focal points of the ellipse that the whole book draws. All are subject to the law of human affliction. They are like the grass that flourishes in the morning and withers and dries up in the evening; and sin accompanies frailty. As the refuge of humanity from generation to generation, the Lord saves people by satisfying those who fear him for many days and by assuring his faithfulness to the children of their children, from age to age. Despite their repeated unfaithfulness, God does not grow weary of forgiving them. The central section (Ps 96–100) stands out from the rest of the book, which it overshadows. All peoples are called to come to Jerusalem to worship the King of the universe and to bless him who has saved the chosen people from among all the families of the earth. Carried by the promise of the one God and by the hope of the one people, this invitation is situated on the unattained, but always desired eschatological horizon. The focal points of the ellipsis (Ps 95 and Ps 101) are the key to reading the book. The first one focuses on a wish addressed to humans: “Today if you would only listen to his voice” (Ps 95:7). The second one poses a question to God: “When will you come to me?” (Ps 101:2). Therefore, the whole fourth book is captured in this tension between God’s desire and human desire, each going to meet the other, in a “today” whose fragility places every human being between the morning when the grass flourishes and the evening when it dries up, between the time of the fathers, which the last section evokes at length, the time of sin and forgiveness, and the time of salvation and of the end, which the central section contemplates.

Two months after D-Day, just as the battle of Normandy was reaching its climax, with all eyes on the Falaise Pocket, the Allies unleashed the second invasion of France not in the Pas de Calais but the French Riviera. Immaculately planned, effectively undertaken, the Allies quickly broke out of their bridgehead, drove 400 miles into France in three weeks, and liberated 10,000 square miles of French territory while inflicting 143,250 German casualties. On September 10 they linked up with Patton’s Third Army and advanced into the Vosges Mountains, taking Strasbourg and holding the area against the Germans’ final big attack in the west: Operation Nordwind in January 1945. US Seventh Army and 6th Army Group undertook a successful campaign placing a third Allied army group with its own independent supply lines, in northeastern France at a time when the two northern Allied army groups were stretched to the limit. Without this force the Allies would have struggled to hold the frontage to Switzerland and Third Army would have been exposed to attack in its southern flank—something that could have had disastrous repercussions particularly during the Ardennes offensive of December 1944.The images of palm trees and azure seas obscure our view of this campaign. It was no cakewalk. The Germans knew the Allies were coming and had strong defences in the area. A shortage of landing craft, vehicles, and matériel meant that the US Seventh and French First armies were restricted in the assault. The heavy fog and anti-glider defences made for a difficult airborne assault, but it was carried out effectively, the amphibious assault was textbook in execution and the invasion of southern France ended up as a significant victory. But the story of 6th Army Group wasn’t finished. Taking up a position on the east flank of Third Army it fought its way through the Vosges and withstood the Germans’ last throw: Operation Nordwind—the vain attempt to relieve pressure on the Ardennes assault by attacking in the Vosges. Heavy fighting pressed hard towards Strasbourg but the Allies were ultimately victorious, inflicting severe losses on the Germans.

The Story of the Negro is a history of Americans of African descent before and after slavery. Originally produced in two volumes, and published here for the first time in one paperback volume, the first part covers Africa and the history of slavery in the United States while the second part carries the history from the Civil War to the first part of the twentieth century. Booker T. Washington was born into slavery, worked menial jobs in order to acquire an education, and became the most important voice of African American interests beginning in the latter part of the nineteenth century. The Story of the Negro is valuable in part because it is full of significant information taken from hundreds of obscure sources that would be nearly impossible to assemble today. For instance, Washington discusses the rise of African American comedy with names, places, and dates; elsewhere he traces the growth and spread of African American home ownership and independent businesses in the United States; and his discussion of slavery is informed by his own life. Washington wanted African Americans to understand and embrace their heritage, not be ashamed of it. He explains, as an example, the role of music in the lives of the slaves and then notes how, nearly a generation later, many African Americans were "embarrassed" by this music and did not want to learn traditional songs. Washington is able to reflect on the first fifty years of his life embracing a range of experiences from share-cropping to dinner at the White House. It is just this autobiographical element that makes the volume compelling. Washington, with his indefatigable optimism, worked his entire life to achieve equality for African Americans through practical means. Founder of the first business association (the National Negro Business League), leader of the Tuskeegee Institute, where George Washington Carver conducted research, and supporter of numerous social programs designed to improve the welfare of African Americans, Washington was considered during his lifetime the spokesperson for African Americans by white society, particularly those in positions of power. This led to criticism from within the African American community, most notably from W. E. B. Du Bois, who considered Washington too accommodating of the white majority, but it took Washington's farsightedness to recognize that the immediate concerns of education, employment, and self-reflection were necessary to achieve the ultimate goal of racial equality.

Clara D. True and Clinton J. Crandall, teacher and superintendent for the Indian Day School of the Santa Clara Pueblo, were typical agents in the campaign waged by the federal government to assimilate Native Americans into mainstream American society. As the primary Office of Indian Affairs officials for the Pueblo, True and Crandall administered the school and also served as de facto health officials, demographers, arbiters, and legal consultants as well as the eyes and ears of the government. Drawing upon an extensive correspondence between True and Crandall from 1902 to 1907, Adrea Lawrence provides an intimate look at the daily lives and challenges that the two educators faced as they worked with a diverse community of Tewa Indians and Hispanos. Through this long-overlooked correspondence, Lawrence introduces us to two fascinating characters-flawed but intent individuals charged with the task of carrying out the government's colonialist Indian education policy. Through descriptions of such episodes as their disdain for older Indians' suspicion of vaccination, True and Crandall provide clear examples of the inherent contradictions in the federal government's culturally insensitive approach toward its Indian population. Yet they were also great advocates for the Indians, often stepping in to mediate in matters involving land and taxation. On 7 April 1944 a Slovakian Jew, Rudolf Vrba (born Walter Rosenberg), and a fellow prisoner, Alfred Wetzler, succeeded in escaping from Auschwitz-Birkenau. As block registrars both men had been allowed relative (though always risky) freedom of movement in the camp and thus had been able to observe the massive preparations underway at Birkenau of the entire killing machine for the eradication of Europe's last remaining Jewish community, the 800,000 Jews of Hungary. The two men somehow made their way back to Slovakia where they sought out the Jewish Council (Judenrat) to warn them of the impending disaster. The Vrba-Wetzler report was the first document about the Auschwitz death camp to reach the free world and to be accepted as credible. Its authenticity broke the barrier of skepticism and apathy that had existed up to that point. However, though their critical and alarming assessment was in the hands of Hungarian Jewish leaders by April 28 or early May 1944, it is doubtful that the information it contained reached more than just a small part of the prospective victims—during May and June 1944, about 437,000 Hungarian Jews boarded, in good faith, the "resettlement" trains that were to carry them off to Auschwitz, where most of them were gassed on arrival. Vrba, who emigrated to Canada at war's end, published his autobiography in England nearly forty years ago. Yet his and Wetzler's story has been carefully kept from Israel's Hebrew-reading public and appears nowhere in any of the history texts that are part of the official curriculum. As Ruth Linn writes, "Israeli Holocaust historiography was to follow the spirit of the court's policy at the Eichmann trial: silencing and removing challenging survivors from the gallery, and muting questions about the role of the Jewish Council in the deportations." In 1998 Linn arranged for publication of the first Hebrew edition of Vrba's memoirs. In Escaping Auschwitz she establishes the chronology of Vrba's disappearance not only from Auschwitz but also from the Israeli Holocaust narrative, skillfully exposing how the official Israeli historiography of the Holocaust has sought to suppress the story.

These Potatoes Look Like Humans offers a unique understanding of the intersection between land, labour, dispossession and violence experienced by Black South Africans from the apartheid period to the present. In this ground-breaking book, Mbuso Nkosi criticises the historical framing of this debate within narrow materialist and legalistic arguments. His assertion is that, for most Black South Africans, the meaning of land cannot be separated from one’s spiritual and ancestral connection to it, and this results in him seeing the dispossession of land in South Africa with a perspective not yet explored.Nkosi takes as his starting point the historic 1959 potato boycott in South Africa, which came about as a result of startling rumours that potatoes dug out of the soil from the farms in the Bethal district of Mpumalanga were in fact human heads. Journalists such as Ruth First and Henry Nxumalo went to Bethal to uncover these stories and revealed horrific accounts of abuse and routine killings of farmworkers by white Afrikaners. The workers were disenfranchised Black people who were forced to work on these farms for alleged ‘crimes’ against National Party state laws, such as the failure to carry passbooks. In reading this violence from the perspectives of both the Black worker and the white farmer, Nkosi deploys the device of the eye to look at his research subjects and make sense of how the past informs the present. His argument is that the violence against Black farmworkers was not only on the exploitation of cheap labour, but also an anxiety white farmers felt about their settler-colonial appropriation of land. This anxiety, Nkosi argues, is pervasive in current heated public debates on the land question and calls for ‘land expropriation without compensation’. Furthermore, the dispossession of Black people from their land cannot be overcome until there is a recognition of the dead and restless spirits of the land, and a spiritual return to home for Black people’s ancestors. Until such time, the cycles of violence will persist.This book will be of interest to academics and scholars working in the area of land and workers’ struggles but also to the general reader who wants to gain a deeper understanding of redress and social justice on multiple levels.

On the outbreak of war in 1914, the Royal Navy found it required more small craft than it possessed to carry out minesweeping, anti-submarine patrols and coastal defence. This led to the formation of an auxiliary force of civilian vessels, including motor pleasure boats and yachts, relatively new types of craft powered by the internal combustion engine. The inclusion of these vessels came about when a group of motor boat owners suggested to the Admiralty that their vessels could play an important role in the defence of Britain. The result was the formation of the Royal Naval Motor Boat Reserve (RNMBR) in 1912. By mid-1915, the demands of naval duty had proved too much for these quirky craft. A meeting in the USA led to their replacement by American-designed Elco motor launches (MLs), of which 550 were purchased, and these Elco launches gave great service for the rest of the war, usually officered by RNVR personnel who won three Victoria Crosses. In addition to the Elcos, in 1915 some naval officers developed the hydroplaning coastal motor boats (CMBs), which served with equal distinction in the latter part of the war. Post-war, both types saw valuable service in the occupation of the Rhineland and in the Baltic campaign, where three more VCs were won. Other countries adopted similar craft. In Italy, the MAS torpedo motor craft achieved fame and success. And in France, MLs supplied by Britain, and by Elco, played their part. Germany too utilised small motor vessels, including the torpedo-armed Luftschiffmotorboote and Fernlenkboot remotely controlled designs. And when America entered the war, she built a fleet of so-called 'sub chasers', wooden-built and designed to counter U-boats along her East Coast. The Petrol Navy tells the stirring story of these motor-driven boats at war, of their development and operations and of the many colourful characters who were their captains and crew. It will acquaint historians and enthusiasts with an important and previously untold aspect of the naval war, and will engross those with a more general interest in the First World War.

Though the last two centuries have seen an inversion in the portion of people living on farms to those in cities, the land still beckons, whether traversed in a car or train, scanned from far above, or as the locus of our food supply or leisure. The Rural Landscape provides a deceptively simple method for approaching the often complex and variegated shape of the land. Hart divides it into its mineral, vegetable, and animal components and shows how each are interdependent, using examples from across Europe and America. Looking at the land forms of southern England, for instance, he comments on the use of hedgerows to divide fields, the mineral or geomorphological features of the land determining where hedgerows will grow in service of the human animal's needs. Hart reveals the impact on the land of human culture and the basic imperative of making a living as well as the evolution of technical skills toward that end (as seen in the advance of barbed wire as a function of modern transportation). Hart describes with equal clarity the erosion of land to form river basins and the workings of a coal mine. This book takes a carefully considered look at the history of many lesser known but nevertheless advanced British flying wing, delta and tailless aircraft that stretch across much of the last century. The emphasis is on classified projects considered for research or military purposes, but also includes those aircraft that were built, flown and entered service. The first commercially successful British flying wing biplane was designed by John Dunne and undertook limited military duties during the First World War. Soon, the early flying wing designs gave way to sleeker boomerangs that looked impressive, but often suffered with aerodynamic shortcomings. These were followed by the arrival of advanced wartime German jet powered delta projects initiated by Dr Alexander Lippisch. They were massively influential, with most post-war scientists and engineers immediately recognised the potential for a new generation of high performance warplanes. By the late 1940s, the UK required advanced jet powered bombers capable of carrying atomic bombs over long distances. This created many unusual, often German influenced designs that finally resulted in the V-Bombers. They were followed by short lived concepts for even larger aircraft that were not only capable of delivering nuclear weapons, but powered by nuclear propulsion. At the same time, interest in variable geometry wings gathered momentum, leading to the supersonic Swallow with its impressive science-fiction appearance. Sadly this Barnes Wallace concept was too far ahead of its time to overcome a number of technical issues, but the influence on American combat aircraft was considerable. The British also pushed ahead with ideas for vertical take-off fighters and the delta wing was often the first choice for many proposals. By the 1960s, Britain was attempting to gain a foothold in the space race. Plans were drawn up for rocket launching sites within the UK and the development of exotic delta winged space fighters. The ideas were impressive, although the technology was well beyond the UK's ability to fund and develop. These were glorious times for British aircraft designers who often pushed their ideas to the boundaries of what was possible, with design studies that remain influential today.

Today, 139 of these booklets survive, covering the years 1818 up to the composer's death in 1827 and including such topics as music, history, politics, art, literature, theatre, religion, and education as perceived on a day-to-day basis in post-Napoleonic Europe. An East German edition, begun in the 1960s and essentially complete by 2001, represents a diplomatic transcription of these documents. It is a masterpiece of pure scholarship but is difficult to use for anyone who is not a specialist. Moreover, Beethoven scholarship has moved on significantly since the long-ranging genesis of the German edition. These important booklets are here translated into English in their entirety for the first time. The volumes in this series include an updated editorial apparatus, with revised and expanded notes and many new footnotes exclusive to this edition, and brand new introductions, which together place many of the quickly changing conversational topics into context. In 1791, General Arthur St. Clair led the United States army in a campaign to destroy a complex of Indian villages at the Maumee River in northwestern Ohio. Almost within reach of their objective, St. Clair's 1,400 men were attacked by about one thousand Indians. The U.S. force was decimated, suffering nearly one thousand casualties in killed and wounded, while Indian casualties numbered only a few dozen. But despite the lopsided result, it wouldn't appear to carry much significance; it involved only a few thousand people, lasted less than three hours, and the outcome, which was never in doubt, was permanently reversed a mere three years later. Neither an epic struggle nor a clash that changed the course of history, the battle doesn't even have a name. Yet, as renowned Native American historian Colin Calloway demonstrates here, St. Clair's Defeat--as it came to be known-- was hugely important for its time. Systems' Verification Validation and Testing (VVT) are carried out throughout systems' lifetimes. Notably, quality-cost expended on performing VVT activities and correcting system defects consumes about half of the overall engineering cost. Verification, Validation and Testing of Engineered Systems provides a comprehensive compendium of VVT activities and corresponding VVT methods for implementation throughout the entire lifecycle of an engineered system.
