Treatment-resistant Depress Successful management of patients with treatment-resistant depression requires a thorough understanding of the biological basis for both the depression and its failure to respond to standard treatments. This book clearly and succinctly summarizes the latest scientific research and its applications in clinical practice. A first step is a clear definition of what constitutes treatment-resistant depression so that clinical trials and other studies are using common criteria, enabling comparison and meta-analysis of their outcomes. The opening chapter reviews definitions and predictors of treatment-resistant depression originating from different fields and discusses their usefulness in clinical practice and clinical research. The next chapter proposes a new definition, adapting terminology from medicine. Biological classification requires identification of genetic risk factors and gene variants have been identified as accounting for 50% of the variance in the clinical outcomes of antidepressant treatments. Chapter 3 describes several genes already associated with treatment-resistant depression and, while further work is needed to translate findings into clinical recommendations, suggests that genetic prediction of treatment resistance could become a widespread clinical reality within a few years. Most patients with treatment-resistant depression will be treated pharmacologically, so three chapters review the latest evidence for pharmacological best practice in switching strategies for antidepressants, the role of antipsychotics and augmentation strategies to complement lithium. There are two major alternatives to pharmacotherapy: neuromodulation and psychotherapy. The brain intervention chapter summarizes clinical research and experience with electroconvulsive therapy, transcranial magnetic stimulation, vagus nerve stimulation, deep brain stimulation and magnetic seizure therapy. The final chapter reviews the literature pertaining to the effectiveness of various forms of psychotherapy in patients who have not responded to antidepressant pharmacotherapy, explaining that patients who have not responded to one or two trials of antidepressant medication have a 30%-50% chance of responding to a focused psychotherapy. It proposes indications for psychotherapy in treatment-resistant depression and summarizes general therapeutic principles. Defending society against natural hazards is a high-stakes game of chance against nature, involving tough decisions. How should a developing nation allocate its budget between building schools for towns without ones or making existing schools earthquake-resistant? Does it make more sense to build levees to protect against floods, or to prevent development in the areas at risk? Would more lives be saved by making hospitals earthquake-resistant, or using the funds for patient care? What should scientists tell the public when – as occurred in L’Aquila, Italy and Mammoth Lakes, California – there is a real but small risk of an upcoming earthquake or volcanic eruption? Recent hurricanes, earthquakes, and tsunamis show that society often handles such choices poorly. Sometimes nature surprises us, when an earthquake, hurricane, or flood is bigger or has greater effects than expected from detailed hazard assessments. In other cases, nature outsmarts us, doing great damage despite expensive mitigation measures or causing us to divert limited resources to mitigate hazards that are overestimated. Much of the problem comes from the fact that formulating effective natural hazard policy involves combining science, economics, and risk analysis to analyze a problem and explore the costs and benefits of different options, in situations where the future is very uncertain. Because mitigation policies are typically chosen without such analysis, the results are often disappointing. This book uses general principles and case studies to explore how we can do better by taking an integrated view of natural hazards issues, rather than treating the relevant geoscience, engineering, economics, and policy formulation separately. George Perle takes us into the composer's workshop as he reevaluates what we call 'twentieth-century music' - a term used to refer to new or modern or contemporary music that represents a radical break from the tonal tradition, or 'common practice', of the preceding three centuries. He proposes that this music, in the course of breaking with the tonal tradition, presents coherent and definable elements of a new tradition. In spite of the disparity in their styles, idioms, and compositional methods, he argues, what unites Scriabin, Stravinsky, Bartok, and the Viennese circle (Schoenberg, Berg, and Webern) is more important than what separates them. If we are to understand the connections among these mainstream composers, we also have to understand their connections with the past. Through an extraordinarily comprehensive analysis of a single piece by Varese, Density 21.5 for unaccompanied flute, Perle shows how these composers refer not only to their contemporaries but also to Wagner, Debussy, and Beethoven. Perle isolates the years 1909-10 as the moment of revolutionary transformation in the foundational premises of our musical language. Before you lies the second volume in a trilogy about the Sicilian Defense. The first volume dealt with the Najdorf and Scheveningen variations, and it is now time to pay attention to three other extremely popular systems: the Taimanov, Kan and Richter-Rauzer variations. After careful consideration within the Thinkers Publis­-hing team, we decided that it made sense to group these variations together. In particular, the first two are closely related and share the feature that, in both cases, Black plays …e7-e6 and …a7-a6 at an early stage. They typically have the idea of retaining more options for their king’s bishop by postponing …d7-d6 (or even omitting it entirely.) The bishop may go to b4 or c5 in different lines. The Richter-Rauzer is, in theory, just one of the possible developments from a Classical Sicilian. We have already dealt with a few games that started with the Classical and where Black shortly played …e7-e6; and 6.Bc4 (the Sozin variation) was rightly treated within the Scheveningen pages. However, it is clear that White’s most po­pular counter, the Richter-Rauzer variation (6.Bg5) deserves separate attention. While looking at the variation structure for the Kan and Taimanov and deciding on which model games to use, I noticed a lot of possible transitions to the ‘Hedgehog’ structure, shown on the right. The key features are white pawns on e4 and c4, and at least four black pawns on a6, b6, d6 and e6. This structure is ideally suited to the task of playing for a win as Black, because of the very complicated middlegames that arise. (And one often needs complicated middlegames to have a better chance of ‘converting’ a rating advantage!) The ‘Hedgehog’ is definitely a structure rather than a variation, but it has such a distinctive character of its own that I chose to examine it first in chapter 2. This simplifies later discussion of the Taimanov and Kan variations by removing the need to discuss every possible way of entering a Hedgehog structure.

Marc Woodworth's book covers the album's long and unorthodox period of writing, recording, sequencing, and editing. It includes interviews with members of the band, manager Pete Jamison, web-master and GBV historian Rich Turiel and Robert Griffin of Scat Records. At least sixty-five songs were recorded and considered for the album and five distinct concepts were rejected before the band hit upon the records final form. One late version, very nearly released, contained only a few of Bee Thousand's definitive songs. The rest were left out and nearly ended up in the boxes of cassette out-takes cluttering up Robert Pollard's basement. The story of Guided By Voices transformation from an occasional and revolving group of complete unknowns to indie-rock heroes is very much part of the story behind the making of "Bee Thousand". In addition to providing a central account of how the record was made, Woodworth devotes another substantial chapter to the album's lyrics. Robert Pollard's lyrics are described by critics, when they're described at all, as a brand of tossed-off surrealism, as if his verbal sensibility is somehow incidental to the songs themselves. Nothing could be further from the truth. I want to offer a sustained discussion of Pollard's work as a writer of often sublime, beautiful, and very human lyrics. I won't miss the chance to consider the pleasures of the absurd in this context and, of course, the contributions to "Bee Thousand" of Tobin Sprout. The third key section of the book covers aesthetics. Woodworth considers the great appeal of the do-it-yourself nature of "Bee Thousand" and reflects on the larger importance of the strain of alternative rock for which this record is a touchstone. The focus is on "Bee Thousand", of course, but also includes discussion on the rise of a lo-fi aesthetic and its continuing influence. This chapter would also provide the context for an account of rock that exists because of passion, love, and compulsive need rather than formula, money, and marketing.

If ever a man has earned his place in the annals of military history, that man is Francis “Gabby” Gabreski. His exploits as a fighter pilot in World War II and Korea are legendary; his rise from humble beginnings to success in military and business careers is inspiring. Born to Polish immigrant parents in 1919, he nearly washed out of Notre Dame and then flight school. He was down to his last chance, and he made the most of it. A witness to the Japanese attack on Pearl Harbor, Gabby had his own first taste of air combat flying with a Polish RAF squadron. Shortly thereafter he joined the 56th Fighter Group of the U.S. 8th Air Force, and in seventeen months he shot down twenty-eight German planes, the highest total of any 8th Air Force pilot in Europe. He became a hero whose name was splashed across newspaper headlines from coast to coast. And then, on the very day he was to fly home to his fiancee and a hero’s welcome, he took one last combat mission, crashed and, after a daring attempt to avoid capture, finished the war in a POW camp. Gabreski returned to combat in 1951, flying F-86 Sabrejets over Korea. He scored 6.5 more victories there, making him one of the few pilots ever to achieve ace status in two wars and in both propeller and jet aircraft. He retired from the Air Force as a colonel in 1967 and spent the next twenty years working in the aviation industry, sustained, as always, by his devout religious faith and his deep love for his family. Now, drawing on his private documents and photographs, Gabby, along with writer Carl Molesworth, tells his thrilling eyewitness story with a candor and a vivid style that should earn this brave pilot a whole new generation of admirers.
