Human reliability and error have become a very important issue in health care, owing to the vast number of associated deaths each year. For example, according to the findings of the Institute of Medicine in 1999, around 100000 Americans die each year because of human error. This makes human error in health care the eighth leading cause of deaths in the US. Moreover, the total annual national cost of the medical errors is estimated at between $17 billion and $37.6 billion.There are very few books on this subject, and none of them covers it at a significant depth. The need for a book presenting the basics of human reliability, human factors and comprehensive information on error in medical systems is essential. This book meets that need.

Over the last forty years, Basil Robinson has established a reputation as a leading authority on the art of Persia. His work on Persian manuscript illumination represents one of the most important contributions made in this century to the study of the development of this pivotal branch of Islamic art, which absorbed the influence of Arab and Chinese painting, and influenced in turn the miniature painting of Mughal India. This first volume concentrates on Persian painting. Seven papers examine the general evolution of painting in Persia from the fourteenth to the nineteenth centuries, "mostly preserved in manuscript illumination, with emphasis on that most characteristic of Persian manuscripts, "the Shah-Nameh, the national epic. Most of the contributions published in this volume were prepared within the framework of the research project "Dynamics of Contacts between Populations and Languages - Lexical and Structural Diffusion and the Genealogies in the Sahel" co-ordinated at the Faculty of Humanities, Charles University in Prague, and in a workshop dedicated to the project at the occasion of the XVIIth International Congress of Linguists (Prague 2003).The purpose of the project was to examine the linguistic situation in the Sahel in view of the recent developments in the study of language contacts, assessing the reality of the Sahel "Sprachbund". The concept of "Sprachbund", or 'linguistic area' as a bundle of features common to languages over a geographical area has experienced considerable extention: from phonology and morphology, its domain has been extended to morphosyntax and lexicon. By doing so, the concepts of calquing, typological convergence, reflexification and metatypy are being used to characterize the process witnessed in the study of language contacts.
