The Faster Care for Veterans Act of 2016 called for the Department of Veterans Affairs (VA) to conduct a pilot program under which veterans can use an Internet website or mobile application to schedule and confirm appointments at the department's medical facilities. Chapter 1 evaluates the VA's patient self-scheduling systems. Chapter 2 focuses on veterans' access to care and the status of VA's community care programs and on VA's scheduling of timely medical appointments. To help ensure that veterans are provided timely and accessible health care services, the Department of Veterans Affairs (VA) may purchase care from non-VA providers, known as community care. Chapter 3 describes trends in obligations for and utilization of VA's community care programs since fiscal year 2014, how VA develops its community care budget estimate and any subsequent changes made to this estimate, and how VA's actual obligations for community care compared with estimated obligations for fiscal years 2017 and 2018. The Department of Veterans Affairs' (VA) Veterans Choice Program (Choice Program) allows eligible veterans to obtain health care services from providers not directly employed by VA (community providers). Chapter 4 identifies challenges to the Choice Program that VA needs to address as it implements its new Veterans Community Care Program (VCCP). The Civilian Health and Medical Program of the Department of Veterans Affairs (CHAMPVA) was established by the Veterans Health Care Expansion Act. As reported in chapter 5, CHAMPVA is primarily a health insurance program where certain eligible dependents and survivors of veterans receive care from private sector health care providers. The Department of Veterans Affairs (VA) and the Department of Health and Human Services' (HHS) Indian Health Service (IHS) established a memorandum of understanding (MOU) to improve the health status of American Indian and Alaska Native (AI/AN) veterans through coordination and resource sharing among VA, IHS, and tribes. Chapter 6 examines VA and IHS oversight of MOU implementation since 2014, the use of reimbursement agreements to pay for AI/AN veterans' care since 2014, and key issues identified by selected VA, IHS, and tribal health program facilities related to coordinating AI/AN veterans' care. Traditionally, the Veterans Health Administration (VHA) of the Department of Veterans Affairs (VA) had predominantly provided gender-specific health care services to male veterans. Congress passed a number of laws to specifically increase women veterans' access to gender-specific health care services through the VHA as reported in chapter 7. Chapter 8 reports on a major rule promulgated by the Department of Veterans Affairs (VA) entitled "Reimbursement for Emergency Treatment."

This remarkable book details the shared experiences of Hans von Mend and his comrade in arms, Adolf Hitler, throughout almost the whole of the First World War. Mend writes of his call-up as a reservist in July 1914 and of joining the 16th Bavarian Reserve Infantry Regiment, more commonly known as List Regiment after its commander Colonel List. It was then that he first met the 25-year-old Hitler. Together, they marched out to the front, and to Flanders, where the regiment was involved in the struggle for Wytschaete, where few men survived unscathed. Hitler was one of those, being promoted to lance-corporal and assigned to the position of regimental runner. Over the course of the following years, the regiment participated in the battles of the Somme and Fromelles in 1916, and Arras and Passchendaele in 1917\. At Fromelles the messengers had to navigate along a particularly dangerous path, which, according to Mend, Hitler passed many times daily and, if he wanted to come through safely, had to more crawl than march. The slightest movement did not elude the English sharp shooters.' Mend states the Hitler's personal courage was acknowledged by those around him'. Mend wrote of Hitler's conversations during quieter periods in the trenches, of how the future Fuhrer spoke of his favourite topics, including art and painting. Mend claims that he listened to him willingly and was amazed how he knew about this field He could explain, like a professor, about German history of art.' But, intriguingly, according to Mend, Hitler's political views, which he was never shy in expounding, made enemies of some of his fellow soldiers. Perhaps inevitably, Hitler was wounded -in his left thigh -and he was decorated with the Iron Cross Second Class, as well as, unusually for a lowly corporal, the Iron Cross First Class. The latter award was for stumbling into a French-held trench while delivering one of his messengers. Reacting quickly, he pointed his rifle at the French soldiers and ordered them to surrender; Hitler delivered twelve prisoners to his commanding officer. Though _I Served With Hitler in the Trenches_ was written in a certain era, it provides much detail about the personal nature and actions of Adolf Hitler. In some ways it is perhaps more insightful than many of the accounts that were to follow when the man who became the German Chancellor was known to the world and a new image of him had been formed.

This is the history of Washington Duke and two of his sons, Benjamin Newton Duke and James Buchanan Duke. Although numerous other members of the family play their parts in the story it focuses primarily on the three men who were at the center of the economic and philanthropic activities which made the Dukes of Durham one of America's famous families. The Dukes operated closely and constantly as a family, and only in that context is their full story told.In the years after the Civil War, Washington Duke proved to be an unusually able industrialist and a conscientious, Methodist philanthropist. He was, in fact, a major Southern pioneer in both industry and philanthropy. His two sons by a second marriage were remarkably devoted to each other as well as to their father. Both sons also reflected traits of thier father. While Benjamin N. Duke and James B. Duke had life-long involvement with the business world—first in tobacco, then textiles, and finally electric power—as well as with philanthropy, they actually developed complementary specializations. Benjamin N. Duke, the older of the two, served as the family's primary agent for philanthropy from his early manhood in the late 1800's until he gradually became semi-invalid after 1915. James B. Duke, on the other hand, early displayed a marked talent, even a genius, for business. Toward the end of his life, with the establishment of The Duke Endowment late in 1924, he emerged as one of the nation's major philanthropists, ranking alongside Andrew Carnegie and John D. Rockefeller. A central theme of this book is, however, that the Endowment, despite its magnitude and far-reaching scope, was essentially the institutionalization and culmination of a pattern of family philanthropy that emerged in the 1890's and for which the older brother, Benjamin N. Duke, had always been the primary agent. Thus, the story of James B. Duke, who was and has remained much the more well-known of the two brothers, cannot properly be told out of the family context from which he emerged and in which occurred most of the important phases of his life.Washington Duke, as a small, land-owning yeoman farmer, was typical of the great majority class not only in antebellum North Carolina but in the South as a whole. Only after the war, when he and his sons emerged as large-scale industrialists and philanthropists, did the Dukes become atypical. Their story is, then, both agricultural and industrial, both Southern and national. Born North Carolinians, they moved onto a national, even global, stage. Yet all the while they kept deep roots, as well as vast investments of capital, in the Old North State, and they poured many millions into philanthropy, largely in the two Carolinas. Based largely on manuscript sources, many of them hitherto unused, this is the first study of the Duke family. The "New South," as recent historians have told us, may not have been so new—but it was certainly different in important ways, and the Dukes loomed large among those who helped to make it so.

Maintaining good metabolic profile plays a significant role in improving the quality of life at aging. Widely recommended physical and psychological strategies include exercise, calorie restriction (such as healthy diet), anti-aging neuroprotective and anti-inflammation therapy. Most occurring risks at middle age range (45-65 years old) are obesity, insulin resistance, inflammation, alteration in the hypothalamus-hypophysis suprarenal axis activity, stress and hypertension that could increase the prevalence of metabolic syndrome. Metabolic syndrome increases with age, particularly for women. Significant associations were seen between imaging measures and cardiovascular risk factors at both baseline and 18-month follow-ups. Both baseline and longitudinal imaging analysis and correlations with neurocogntive tests as well as cardiovascular risk factors could provide distinct and confirmative perspectives relating to the pathophysiology of aging-related diseases such as dementia and diabetes. Significant baseline and longitudinal effects of age, smoking and neuropathological burdens such as amyloid, tau and glucose metabolism provide a complete imaging, neurocognitive and cardiovascular profile for better staging and differentiating different diseases. Together with accurate imaging guidance, early detection and treatment could be achieved with the ultimate goal of improving quality of life at middle age and extending longevity. Henrik Lunde grew up in Norway and came to the United States with his parents as a young teenager. After completing high school, he attended the University of California at Berkeley, graduating in 1958 as the Honor Graduate in the History Department. He also received an appointment in the Regular Army.After the Basic Infantry Officer, Ranger and Airborne courses, and his first duty station with the 2nd Battle Group, 6th Infantry Regiment in Berlin, Hank spent 18 months with a covert Special Forces unit in Berlin. In 1963 he attended the Infantry Officer Career Course at Fort Benning and was designated an Honor Graduate. He then attended the elite Pathfinder Course before reporting to Fort Campbell, Kentucky for assignment to the elite 101st Airborne Division.He deployed to Vietnam with 1st Brigade, 101st Airborne Division, in 1965. For most of his tour he commanded a rifle company. On his return to the States Hank worked as Branch Chief at the Airborne Test Division at Fort Bragg. Still, at the end of 1967, he volunteered for the 9th Division in the Delta despite becoming disillusioned with the tactical/strategic conduct of the war. In the 9th Division, he served as Brigade S-3 and battalion executive officer. He then moved to the Vietnamese II Corps as deputy operations adviser.After graduating from the Command and General Staff College in 1970, in the upper 10% of the class, he moved on to Syracuse University to obtain a master’s degree. He then returned to Vietnam in 1973, serving as Chief of Negotiations of the U.S. Delegation to the FPJMT set up by the 1973 Paris Peace Treaty to account for the dead and missing. After a year at the Political/Military Division of the Army General Staff with southeast Asia as his responsibility area Hank attended the U.S. Army War College as the second youngest student in 1975–76. From 1976 to 1979, he served in the Plans of Policy branch of Supreme Headquarters Allied Powers Europe. His last assignment was as Director of National and International Security Studies for Europe at the Army War College. Colonel Lunde is highly decorated from his three tours in Vietnam.
