Disaster medicine has occupied an increasingly important niche within the specialty of emergency medicine over the latter half of the 20th century. Regardless of whether an event was natural, anthropogenic, or a combination of both, emergency medicine was and is the ideal discipline to develop the human resources, the strategies, the tactics, and the evidence- based research to elevate the field of disaster medicine. It began, organizationally speaking, with Hurricane Hugo in 1989 and it is continuing through the 2019 COVID- 19 pandemic and beyond. Now that we are well into the 21st century, we must steady our footing as a specialty on the 20th century's foundation so that we can wrestle with the more intricate challenges of the future. In the past two decades our global populations have experienced sectarian violence, wars, genocide, migration, terrorism, emerging infectious diseases, and pandemics. These natural and anthropogenic events will only worsen exponentially as we become intrinsically trapped by the effects of climate change. The United Nation's Intergovernmental Panel on Climate Change has recently concluded that regardless of how we try to reverse the effects of global warming, what we are experiencing now in terms of hurricanes, floods, and droughts will continue to worsen over the next 30 years. Should we, as a species, continue to equivocate and as global temperatures climb, these events will become more frequent and more catastrophic. "Catastrophic" may even be too mild a term. It is possible that over the next few years, the academic arm of emergency medicine may need to consider developing a curriculum devoted to "Cataclysmic Medicine." To confront these possibilities, there first needs to be a knowledge of disaster medicine and disaster management at its most basic level. It begins with crisis leadership, command and control, communications, and coordination. When all these pieces of the jigsaw puzzle are fitted together properly and enclosed by a proper ethical frame, then the best and most ethical and moral disaster medical can will be provided to the patient and to the community. The intent of this book is to introduce these concepts using diverse viewpoints and scenarios. Readers are challenged to cogitate, create, and layer their own set of building blocks upon the preexisting foundation, thereby reinforcing and sustaining their own capacity to prepare for and respond to any adverse eventuality, mass casualty or otherwise. I hope that the experiential and evidence- based contents of this book will inspire readers to delve deeper into the nuances of disaster medicine and will serve as an impetus for a more profound quest for knowledge and a desire to serve those who will be experiencing the worst moments of their lives. When the Japanese attacked Pearl Harbor on 7 December 1941, the U.S. Navy had a total of 111 submarines. However, this fleet was not nearly as impressive as the number suggests. It was mostly a collection of ageing boats from the late teens and early twenties, with only a few of the newer, more modern Gato-class boats. Fortunately, with the war in Europe already two years old and friction with Japan ever-increasing, help from what would become known as the Silent Service in the Pacific was on the way: there were 73 of the new fleet submarines under construction. The Silent Service in World War II tells the story of America’s intrepid underwater warriors in the words of the men who lived the war in the Pacific against Japan. The enemy had already begun to deploy advanced boats, but the U.S. was soon able to match them. By 1943 the new Gato-class boats were making a difference, carrying the war not just to the Japanese Imperial Navy, but to the vital merchant fleet that carried the vast array of material needed to keep the land of the Rising Sun afloat. As the war progressed, American success in the Solomons, starting with Guadalcanal, began to constrict the Japanese sea lanes, and operating singly or in wolfpacks they were able to press their attacks on convoys operating beyond the range of U.S. airpower, making daring forays even into the home waters of Japan itself in the quest for ever more elusive targets. Also taking on Japanese warships, as well as rescuing downed airmen (such as the grateful first President Bush), U.S. submarines made an enormous contribution to our war against Japan. The town of Maidstone became indelibly associated with revolution and riot between the 14th and 16th centuries when it experienced the Peasants' Revolt, Cade's Rebellion and the Wyatt Revolt. The townsfolk seem to have enjoyed an independant spirit ever since. Following the Battle of Maidstone, fought between Parliamentarians and Royalists in 1648, the Mayor was Clerk to the court which tried Charles I and it was probably he who pronounced the death sentence. Town meetings voiced opposition to the Revolutionary War with France, and urged the King to dismiss Pitt the Younger and his ministers, but the local population was patriotic. When the threat to the country became apparent the people speedily raised a regiment of Volunteers to defend the homeland. Maidstone artisans were prominent in the Swing Riots of 1830. The town was a centre for the fulling process, essential to cloth production, and for a long period enjoyed a virtual monopoly in threadmaking. It also provided local ragstone for the Tower of London and Windsor Castle Chapel and became, in time, a major source of hops and fruit. Brewers such as Fremlins and Style and Winch were situated here, as were producers of Hollands gin and cherry brandy.Papermaking was an industry of local importance. Sharps toffee, Fosters Clarks grocery lines, Tilling-Stevents buses and Rootes, who built the first Singer car in 1912, were among national names that began life in the town. Penenden Heath has been the county meeting place and the spot from which justice was dispensed since before Domesday, so it was appropriate that Maidstone should eventually become the county town for Kent. Nowadays the county administration and public services are centred here and they have largely replaced the former industries, which have all but disappeared. But the town is proud of its long and rich history and this fully illustrated account helps explain precisely why Maidstone has been such an important town in south-east England for over a thousand years.

In 2015, Bob Dylan said, "I learned lyrics and how to write them from listening to folk songs. And I played them, and I met other people that played them, back when nobody was doing it. Sang nothing but these folk songs, and they gave me the code for everything that's fair game, that everything belongs to everyone." In Hear My Sad Story, Richard Polenberg describes the historical events that led to the writing of many famous American folk songs that served as touchstones for generations of American musicians, lyricists, and folklorists. Those events, which took place from the early nineteenth to the mid-twentieth centuries, often involved tragic occurrences: murders, sometimes resulting from love affairs gone wrong; desperate acts borne out of poverty and unbearable working conditions; and calamities such as railroad crashes, shipwrecks, and natural disasters. All of Polenberg’s account of the songs in the book are grounded in historical fact and illuminate the social history of the times. Reading these tales of sorrow, misfortune, and regret puts us in touch with the dark but terribly familiar side of American history. On Christmas 1895 in St. Louis, an African American man named Lee Shelton, whose nickname was "Stack Lee," shot and killed William Lyons in a dispute over seventy-five cents and a hat. Shelton was sent to prison until 1911, committed another murder upon his release, and died in a prison hospital in 1912. Even during his lifetime, songs were being written about Shelton, and eventually 450 versions of his story would be recorded. As the song—you may know Shelton as Stagolee or Stagger Lee—was shared and adapted, the emotions of the time were preserved, but the fact that the songs described real people, real lives, often fell by the wayside. Polenberg returns us to the men and women who, in song, became legends. The lyrics serve as valuable historical sources, providing important information about what had happened, why, and what it all meant. More important, they reflect the character of American life and the pathos elicited by the musical memory of these common and troubled lives.

The book is the end product of more than five years' worth of investigation, prosecutions, and research by the New York State Organized Crime Task Force, a unique agency that has set a national example for marrying law enforcement initiatives with comprehensive and exhausting analysis of the causes and dynamics of industrial racketeering. This is a sobering analysis of the construction industry , one of New York City's largest industries, and in effect, one of the city's most significant economic sectors. In any given year during the 1980s, billions of dollars of construction were being carried out at any one time. The industry regularly employs more than 100,000 people in the city, involving some one hundred union locals and many hundreds of general and specialty contractors as well as a large number of architects, engineers, and materials suppliers. The book shows—in great and provocative detail—how organized extortion, bribery illegal cartels, and bid rigging characterize construction in the city. The basis for much of this crim is labor racketeering, controlled or orchestrated by organized crime. It reveals how this world of corruption affects not only the private sector but the city's vast public works program, and it spells out the ways in which both organized crime and official corruption each sustain the dynamics of ongoing criminality. Wrong-doing on a massive scale is documented at length. But this book is more than a recitation of extensive and systematic criminality. The book recommends a number of plausible options for genuine reform. Necessarily these are profound and radical solutions, but everyone who reads this book will conclude that only profound and radical solutions could hope to solve such an entrenched and intractable crime problem.

Depression is one of the most common issues that people bring to therapy. It is also a mental health condition with several well-known and readily available medications to treat it. That said, every clinician knows that medications do not work for all clients, and even if they do work they can often come with unwelcome side effects that are difficult and hard to bear. In short, medications are not foolproof. Fortunately today, with rising interest in non-drug approaches, effective and easy-to-implement alternative strategies exist for dealing with depression in your clients, either in conjunction with medication treatments or on their own. The advantage of “our” Benoni is based on a waiting approach. Black would like to choose a perfect moment to play …e6xd5, waiting for White to adopt some piece setup that turns out to be inconvenient for him after this exchange. At the same time, we would like to avoid some dangerous or deeply explored variations like the Flick-Knife (a.k.a Taimanov) or systems where White can place his bishop on the optimal f4-square. A lot of variations in this book can also be useful for King’s Indian players, as a main or alternative way to play. My own journey in the world of the Delayed Benoni started when I was a King’s Indian kind of guy! As every rose has its thorn, so White can annoy us by answering our …e6xd5 not with the routine c4xd5 but rather with e4xd5. This produces a completely different pawn formation, with an open e-file. White enjoys a space advantage but Black has his chances. And just as White can depart from the well-trodden path with e4xd5, so Black can dispense with the almost automatic …e6xd5 and instead play …e6-e5, producing a sort of King’s Indian formation. In the Main Line, covered in Chapter 4, White has already played h2-h3 and this pawn can become a target when Black gets his kingside attack moving. Our opponent can force us into a Modern Benoni, by playing f3 (Sämisch) or f4 (Four Pawns). I think that I have succeeded defending Black’s case, even finding some important nuances improving on the existing theory. From my personal experience, the only way for White to achieve some advantage is the h3 and Bd3 variation with e4xd5, the Main Variation covered in Chapter 4. The problem for White, though, is that Black can answer that line in many different ways. So White must be thoroughly prepared and acquainted with all the nuances of our system. And even then, White’s advantage is just a “normal” one. I give a different approach to this variation, three (!) different ways for Black to respond. The reader can also, through the game commentaries, see the development of the variation in my practice. In my opinion, the Delayed Benoni is a kind of mystery for White also, since it has not been covered deeply enough in chess publications. So I think that this work could be useful for White players, too.

For more than forty years William Watson has occupied a unique place in the study and teaching of Chinese art in Great Britain. When the Japanese attacked Pearl Harbor on 7 December 1941, the U.S. Navy had a total of 111 submarines. However, this fleet was not nearly as impressive as the number suggests. It was mostly a collection of ageing boats from the late teens and early twenties, with only a few of the newer, more modern Gato-class boats. Fortunately, with the war in Europe already two years old and friction with Japan ever-increasing, help from what would become known as the Silent Service in the Pacific was on the way: there were 73 of the new fleet submarines under construction.The Silent Service in World War II tells the story of America's intrepid underwater warriors in the words of the men who lived the war in the Pacific against Japan. The enemy had already begun to deploy advanced boats, but the U.S. was soon able to match them. By 1943 the new Gato-class boats were making a difference, carrying the war not just to the Japanese Imperial Navy, but to the vital merchant fleet that carried the vast array of material needed to keep the land of the Rising Sun afloat.As the war progressed, American success in the Solomons, starting with Guadalcanal, began to constrict the Japanese sea lanes, and operating singly or in wolfpacks they were able to press their attacks on convoys operating beyond the range of U.S. airpower, making daring forays even into the home waters of Japan itself in the quest for ever more elusive targets. Two months after D-Day, just as the battle of Normandy was reaching its climax, with all eyes on the Falaise Pocket, the Allies unleashed the second invasion of France not in the Pas de Calais but the French Riviera. Immaculately planned, effectively undertaken, the Allies quickly broke out of their bridgehead, drove 400 miles into France in three weeks, and liberated 10,000 square miles of French territory while inflicting 143,250 German casualties. On September 10 they linked up with Patton’s Third Army and advanced into the Vosges Mountains, taking Strasbourg and holding the area against the Germans’ final big attack in the west: Operation Nordwind in January 1945. US Seventh Army and 6th Army Group undertook a successful campaign placing a third Allied army group with its own independent supply lines, in northeastern France at a time when the two northern Allied army groups were stretched to the limit. Without this force the Allies would have struggled to hold the frontage to Switzerland and Third Army would have been exposed to attack in its southern flank—something that could have had disastrous repercussions particularly during the Ardennes offensive of December 1944.The images of palm trees and azure seas obscure our view of this campaign. It was no cakewalk. The Germans knew the Allies were coming and had strong defences in the area. A shortage of landing craft, vehicles, and matériel meant that the US Seventh and French First armies were restricted in the assault. The heavy fog and anti-glider defences made for a difficult airborne assault, but it was carried out effectively, the amphibious assault was textbook in execution and the invasion of southern France ended up as a significant victory. But the story of 6th Army Group wasn’t finished. Taking up a position on the east flank of Third Army it fought its way through the Vosges and withstood the Germans’ last throw: Operation Nordwind—the vain attempt to relieve pressure on the Ardennes assault by attacking in the Vosges. Heavy fighting pressed hard towards Strasbourg but the Allies were ultimately victorious, inflicting severe losses on the Germans.
