Chapters in Section II are formatted similarly, with an introduction to a treatment approach, followed by details and clinically useable information, such as recommended candidates, treatment procedures, ways to individualize the approach, and data collection techniques. There is no denying that time runs out for both capitalism and liberal markets in representative democracies. On the other hand, authoritarian or populist regimes are intensely trying other political and economic roads by putting on impressive performances as the one China has claimed for the last decades. This book, however, sets forth another standpoint, asserting that capitalism and economic liberalism have fostered and empowered a triad comprising organized crime, corruption and ecocide to an extent not witnessed before in human history. This partnership stemmed from the internal dynamics of capitalism and economic liberalism that failed in strengthening the institutions of representative democracies and neglected the most basic social issues. To add insult to injury, the triad took advantage of the same tool kit of organizational skills and technologies that allowed praiseworthy companies to thrive and develop, but without displaying any moral constraint and with dire contempt of the law instead. In contrast with the conventional approach, this book connects the dots between the bad politics and worse governance that organizations in the triad extensively have carried out so far. For the first time in the literature, the threads of organized crime, corruption and ecocide are borne to light by showing how rotten politics and criminal governances have been intertwined in the end. If capitalism and the liberal economics are doomed to failure, the trappings of the triad put both ideologies on the slippery slope and will bring them to account at last. It is the author's contention that to redress the balance in representative democracies, their underlying political systems must work their way through post capitalism by means of social democracy and social markets, which amounts to a new governance and a better politics. Although iliopsoas disorders are one of the most common causes of anterior hip pain in young athletes and active people, tendinopathy may also be described in elderly patients after total hip replacement. The authors go on to discuss, Clostridium difficile infection (CDI), a significant enteric pathogen of public health concern that causes toxin-mediated diarrhea in humans. CDI is commonly reported among hospital in-patients undergoing prolonged antibiotic therapy, which disrupts normal gut microflora. Additionally, schistosomiasis, a parasitic disease caused by trematodes of the genus Schistosoma, is examined. After malaria, schistosomiasis remains the most prevalent tropical disease in the world. Despite significant efforts to control schistosomiasis, the disease remains of considerable public health and economic importance in many developing countries. The authors describe the clinical and pathological manifestations of Highly Pathogenic Avian Influenza as experienced firsthand in over a decade of infection in West Africa, and emphasize effective control measures. A concise summary of pediatric post-laminectomy kyphosis is provided via a discussion of the following areas: the background and pathophysiology of how post-laminectomy kyphosis occurs in a pediatric population, risk factors for kyphosis, methods of prevention and eventual treatment if the kyphotic deformity progresses, and relevant studies of post-laminectomy kyphosis in post-tumor resection and post-spinal cord injury. A clinical and histopathological overview of subependymal giant cell astrocytomas, including symptoms, diagnosis, treatment, prognosis, and genetic, is also provided. Later, the authors explore mucositis, inflammatory and/or ulcerative lesions of the oral and/or gastrointestinal system. It is one of the side effects of radiotherapy and chemotherapy used in cancer treatment. Similarly explored is inflammatory bowel disease (IBD), is a chronic inflammatory condition of the intestines with an incompletely understood pathophysiology. IBD encompasses gut inflammation which leads to changes in neutrally-controlled functions of the gut including abdominal pain, cramps, urgency to defecate, and diarrhea. The penultimate chapter focuses on flat foot, a common postural disorder of lower extremity in children. It may occur during the growth and developmental period of children, and has been found to normally reduce with age. The aim of the closing study is to investigate the occurrence of differences in flexible flatfoot in adolescents who have trained various sports (football, swimming, basketball, team handball, karate) in comparison to the untrained adolescents.

The premise of this book is twofold: First, that the imaginary is real. Where it differs from what we commonly take to be reality is in structure and in form. The imaginary of art, for example, is not illusionary for it is phenomenologically describable and even depictable, as demonstrated by the self-reflexive efforts of modernist painters and writers. No less real than the imaginary of art, and thus fantasy, is the imaginary of delusion, ascertainable in the very function it serves. Though fundamentally different, fantasy and delusion do share a significant feature: a preoccupation with agency. Second is that change, the enhancement of self through an increase in agency, is facilitated by the biology of reward: The pleasure of increased self-cohesion—the efficacy acquired through knowledge of, and the attribution of meaning to, the world—is ultimately the sine qua non of imaginative thought.Oppenheim emphasizes the idea that imagination generates knowledge. Our sensory systems, like our higher cognitive functions, give the human brain knowledge to maintain the homeostatic balance required for survival and to enrich the sense of self required for agency. And, she suggests, imagination is a function of their doing so. Moreover, she explores the construct by which we apprehend the workings of imagination—fantasy—and considers in what the mental imagery that endows it consists, how fantasy may be transmitted transgenerationally, and how delusion can be an impediment to imagination while also being a product of it. Additionally, she likens psychoanalysis to the making of art as a process of acquiring knowledge and looks at creativity itself as a coming-to-know.Throughout this book, there run several opposing threads. The first is that of the intra- and interpsychic psychoanalytic paradigms. This theoretical contrast bears on our understanding of aesthetic experience as sublimatory versus object relational and on our understanding of the construction of meaning. A second opposition resides in the notion of agency (with its implication of self-cohesion) which has everything to do with ego function and, seemingly, the usefulness of "unconscious fantasy," a cornerstone of psychoanalysis now thrown into question by the postmodern favoring of dissociation over repression and other mechanisms of defense. Last, but no less significant, is the contrast interwoven between the empiricism of neuroscience and the metaphysics of philosophical thought. This new book tells the story of the Ocean class of standard cargo ships, their design, building and careers, and the author places them firmly in the context of the battle of the Atlantic which was raging at the time of the first launchings. They entered the vanguard of the Allied shipping effort at a time when the German U-boat threat was at its most dangerous, and British shipping resources were stretched to the limit. They were deployed in the North Atlantic, on the long supply routes around Africa to the Middle East, in the Russian convoys, in operations in support of the invasions of North Africa and Italy and the land campaigns which followed, in the D-Day landings and later amphibious operations on the south coast of France. Finally, some of the class joined an invasion force making its way towards Malaya when Japan surrendered in August 1945\. The Oceans paid a heavy price for these accomplishments, one third of the class being lost to torpedoes, bombs or mines in places as far apart as the Florida coast, the Norwegian Sea, the Bay of Algiers and the Gulf of Oman. While these achievements alone would merit an important place in histories of the war at sea, the impact of the Oceans stretched far beyond the direct contribution of the ships themselves. The yards where they were built also served as models for a series of new American shipyards, designed to mass produce cargo vessels with such speed and in such volume as to completely reverse the mathematics of attrition, which had run so badly against the Allies into 1942\. Even more important, the Oceans' blueprints were used as the basis for the American Liberty ship, the 2,700-strong fleet which finally tilted the balance of the war at sea decisively in the Allies' favour and went on to underpin the post-war renewal of the world merchant fleet. This comprehensive new history, based on extensive archival research and lavishly illustrated with contemporary photographs, restores the Oceans to their rightful place in history. The ships' design antecedents are explained, and their ordering, financing and construction analysed in full. Wartime operations are covered in depth, by theatre and with full details of war losses and other casualties. This book, however, sets forth another standpoint, asserting that capitalism and economic liberalism have fostered and empowered a triad comprising organized crime, corruption and ecocide to an extent not witnessed before in human history. This partnership stemmed from the internal dynamics of capitalism and economic liberalism that failed in strengthening the institutions of representative democracies and neglected the most basic social issues. To add insult to injury, the triad took advantage of the same tool kit of organizational skills and technologies that allowed praiseworthy companies to thrive and develop, but without displaying any moral constraint and with dire contempt of the law instead. In contrast with the conventional approach, this book connects the dots between the bad politics and worse governance that organizations in the triad extensively have carried out so far. For the first time in the literature, the threads of organized crime, corruption and ecocide are borne to light by showing how rotten politics and criminal governances have been intertwined in the end. If capitalism and the liberal economics are doomed to failure, the trappings of the triad put both ideologies on the slippery slope and will bring them to account at last. Kiftsgate Court, perched on the northern edge of the Cotswolds Hills in Gloucestershire, is a garden composed of many different scenes. Some elements - the bluebell wood, the clipped hedging and the rose border, with its famously huge Kiftsgate rose - are traditionally English, but there are also areas of Italianate planting and terracing, and others where a mixture of perennials, roses and rare and exotic shrubs thrive side by side. This is largely because the garden has belonged to the same family since its creation 100 years ago. Three women have tended Kiftsgate, each one its driving force for a third of a century, and each building on the legacy of the previous generation. Heather gave Kiftsgate its structure, laying out the semi-formal gardens by the house, planting the tapestry hedge and rose garden, and terracing the banks. In 1954 Heather was succeeded by her daughter, Diany Binny, who extended and developed her mother's planting, made more borders and paths, and refashioned the White Sunk Garden. Since the late 1980s Diany's daughter, Anne Chambers, has been at the helm, further modernizing the garden and its planting, creating new areas of interest, and opening more often to the public. As Robin Lane Fox, who has written the foreword, comments: `There is nowhere else in Britain that has such a family tradition of planting and dedication ... Questel established himself as one of the finest new Caribbean poets in the 1970s with three collections, all published in his native Trinidad: Score (1972) published jointly with his friend Anson Gonzalez, Near Mourning Ground (1979) and his posthumous Hard Stares (1982). Sadly, Victor Questel died too young at 33 in 1982 – and who knows how his writing would have further developed. What is evident is that his poetry developed rapidly in the ten years between first and last publications, and that he left many fine poems that continue to speak to the present. The poems in this collection move from the orality and bitter punning of Prelude (his section of Score) that deal with the fall-out from the Black Power revolution of 1970; to his sceptical investigations of faith, particularly the family resonances of Spiritual Baptist ritual in Near Mourning Ground, and the severe and stoical poems of Hard Stares that look at himself, domesticity and political corruption. Questel, as Gordon Rohlehr’s exceptional tribute and close reading of the poems shows, was an unsparing observer of his own and his region’s failings. His world is frequently a dark one, but the poems are intense with life and bracingly free from sentimentality or self-pity. His scepticism centred most rigorously on himself as a poet, and drove him to the continuing refinement of the language and forms of his verse.Gordon Rohlehr was Questel’s tutor at the University of the West Indies (St Augustine), mentor and friend. His afterword is a record of the man, the development of the poetry and the times. But it is so much more.
