Every year, 10 million people fall ill with tuberculosis (TB) in the world, and of those, 1.5 million people die even though it is a preventable and curable disease. As a result, TB remains the world’s leading infectious cause of death. On the other hand, in industrialized countries, the TB incidence has fallen to a historic low, and clinicians’ experience with TB diminished significantly in recent years. Additional challenges for clinicians include atypical presentation of TB in the immunocompromised, especially among the elderly, and an increasing number of patients with drug-resistant TB. Delayed diagnosis of TB leads to the spread of TB including nosocomial transmission because TB is an air-borne infectious disease, and suboptimal treatment can result in development of drug resistance. Furthermore, treatment of latent TB infection (LTBI) can prevent future TB cases but it has been under-utilized despite recent innovations in diagnosis and treatment regimens of LTBI. This book focuses on advances in diagnostic tools and treatment for both TB disease and latent TB infection. Each chapter/topic is written by one of the top TB experts in the field and the authors are from Japan and the US. After the Napoleonic wars many wealthy British women and men settled along the coast in Liguria and travelled in Piedmont and Valle d'Aosta in search of warmth and health. They established English-speaking colonies of retired clerics, colonial officials, aristocrats and industrialists at places such as Alassio, Bordighera, Sanremo and Portofino. Many were keen artists. This book assesses hundreds of topographical drawings, paintings and photographs of north-west Italy produced by these British visitors and residents in the nineteenth and early twentieth century. Through the identification and analysis of these works, scattered today in private and public collections in Italy and Britain, it provides insights into the way Italian landscapes were understood and appreciated. Considered in conjunction with historical photography, maps, archives and fieldwork, they deepen our knowledge of past land management traditions and recover how the contemporary landscape looked. Cluster analysis finds groups in data automatically. Most methods have been heuristic and leave open such central questions as: how many clusters are there? Which method should I use? How should I handle outliers? Classification assigns new observations to groups given previously classified observations, and also has open questions about parameter tuning, robustness and uncertainty assessment. This book frames cluster analysis and classification in terms of statistical models, thus yielding principled estimation, testing and prediction methods, and sound answers to the central questions. Small business has captured the imagination of both the popular press and politicians. The tradition that has created sympathy for the small entrepreneur has been strengthened in recent years by images of small firms as dynamic, growing, and flexible and of large firms as struggling, outdated, and intractable in the face of changing competitive environments. There is, it appears, an added fervor for America’s support of “the little guy.”Employers Large and Small draws on existing data and new research to create a more complete picture of the roles of large and small employers, challenging much of the conventional wisdom. It argues that the oft-cited achievement of small firms in generating new jobs is primarily a reflection of the fact that industries in which the typical firm is small have grown rapidly in recent years.The authors show that there are striking differences between large and small employers—that in fact large employers pay higher wages, offer better fringe benefits, and on average offer a more attractive package of working conditions and compensation. These differences reflect real challenges faced by small firms: they pay more for their nonlabor inputs and for many fringe benefits if they choose to offer them.Employers Large and Small also goes beyond the workplace, examining the role of large and small employers in politics. Despite the typical portrayal of small business as the underdog in policy disputes, the political resources of small employers are substantial. Janzen describes patterns of healing among the BaKongo of Lower Zaire in Africa, who, like many people elsewhere, utilize cosmopolitan medicine alongside traditional healing practices. What criteria, he asks, determine the choice of the alternative therapies? And what is their institutional interrelationship? In seeking answers, he analyzes case histories and cultural contexts to explore what social transactions, decision making, illness and therapy classifications, and resource allocations are used in the choice of therapy by the ill, their kinfolk, friends, associates, and specialized practitioners. From the Preface: this book presents an 'on the ground' ethnographic account of how medical clients of one region of Lower Zaire diagnose illness, select therapies, and evaluate treatments, a process we call 'therapy management'. The book is intended to clarify a phenomenon of which central African clients have long been cognizant, namely, that medical systems are used in combination. Our study is aimed primarily at readers interested in the practical issues of medical decision-making in an African country, the cultural content of symptoms, and the dynamics of medical pluralism, that is, the existence in a single society of differently designed and conceived medical systems.
