Over five million people die from stroke each year. Stroke is also the leading cause of adult disability, affecting over 20 million people world-wide, with two-thirds of stroke victims left permanently disabled. We now know that speed of intervention is critical and patient outcomes are determined by a very narrow therapeutic window, during which physicians need to be able to make their choice of treatment rapidly, based on the best evidence available. The purpose of this book is to review that evidence, for both medical and surgical therapies as well as preventative strategies, drawing on the expertise of leading international authorities to distinguish the issues for which there is a consensus on best practice from those for which the evidence remains inconclusive. Evidence-based medicine, or EBM which slips off the tongue more readily, seeks to offer proof that what we offer patients whenever we treat them, do a diagnostic test, or recommend anything from an operation to change in life style, is known to be effective. Fifty to 100 years ago, health care was delivered patchily, much less of it was effective, and was based on an image of a kindly doctor treating the sick, who were glad of whatever help could be offered. Now, medicine is effective and whether you look at it as major government expense or big business in private health care insurance, there is no sense in spending money on ineffective treatments, while we do not have enough to spend on what is of proven worth. Individual doctors cannot read everything and cannot possibly synthesise what they do read, so that they are aware of what is proven to be effective and what probably is not. Surgery remains a challenge for young learners. Perhaps the most difficult of all surgeries are those done on the liver, biliary tree and pancreas. These organs are difficult to expose, difficult to operate and the patients are often difficult to recover. There are significant consequences for any missteps on what is a narrow pathway to a successful outcome. The skills needed to master surgery on these organs are hard to acquire. This book is a simple case-by-case approach to the many different types of orthopaedic fractures. The book is being written for residents, registrars, newly qualified doctors and medical students, primarily targeted to those in the fields of diagnostic radiology, orthopaedic surgery and emergency medicine. There are a variety of texts that exist in these fields to assist in acquiring knowledge; however, there lacks a text that provides simple images and explanations from which a fundamental base of skeletal trauma can be formed. Some of the standard textbooks are still too advanced and lack the basic knowledge. This book is divided into sections by anatomic location, each containing a number of cases presented as unknowns. A standard template is utilised for each case, including a brief clinical description, physical examination, followed by radiological images. The reader is then asked to interpret the plain films given. Several questions are then asked, including the most likely diagnosis and followed by brief discussions on radiological findings (with appropriate annotated images, as necessary) and clinical management. Finally, the case is concluded with a summary of key points. With this format readers can test their knowledge on important orthopaedic fractures as seen on a plain film. Furthermore, they can integrate this information with the standard clinical management involved with that specific disease entity.

Foetal cardiology has developed dramatically into a subspecialty in the past 25 years. The majority of people examining the foetal heart are not 'experts' in foetal cardiology and therefore find interpreting images, particularly in case of abnormality, rather difficult. This book is designed as a practical guide, to be kept near the ultrasound machine, for all those performing foetal heart scans, but without the expertise of a foetal cardiologist. The aim is to allow the user to recognise the common forms of foetal cardiac abnormality and to appreciate the associated lesions and outcome. The book has a large number of illustrations to enable the reader to visualise the different types of problem and the various forms in which they may manifest. There are relatively few books published in the field of foetal cardiology. Whilst there is a place for large textbooks covering all aspects of foetal cardiology in detail, most people performing the foetal heart scan need an idiot's guide about the normal foetal heart and the common things that go wrong with it. This book is aimed at all sonographers, obstetricians and radiologists performing obstetric ultrasound scans and at paediatric cardiologists, both at consultant and trainee level. Thus, this book should fulfil this unmet need in the market, although it is not designed as a comprehensive reference textbook in foetal cardiology.

The study of heart disease -- cardiology -- is an immensely satisfying branch of medicine. The heart is just so tangible; we can see it, touch it, hear it, and when all else fails, we can always manage to put a tube inside and take X-ray pictures of it. Because we try to make sense of a three-dimensional organ by taking two dimensional snapshots, we need a variety of angles to get the complete image, and so often use 'an oblique view'. Day to day life in a cardiac unit is the same; often mundane, sometimes dramatic, but always amenable to being examined with a different slant. How do common cardiac drugs get their name? What sort of music do surgeons choose to listen to as they delve amongst our innards? Why can going to cardiac conferences be so damned irritating and just how close to reality are all those TV hospital dramas? What personalised number plates adorn doctors' cars, which accents best suit which specialties and what should you call a disease in order to ensure that it is your name that is immortalised in the medical textbooks for years to come? This collection of musings touches on these, as well as many other aspects of cardiology. They do not attempt to make sense of any of them; they aim only to broaden the mind a little by taking a sideways glance at medical life.

Back pain is the commonest form of rheumatism and it is estimated to affect between 65% and 80% of populations sampled. The costs of back pain-related medical care and disability compensation make it one of society's most significant non-lethal medical conditions. Whilst mortality is low, its morbidity is high, inconvenience great, and economic burden massive. The actual symptom of back pain is one of the commonest and most perplexing encountered in medical practice. A proper understanding of what it means is crucial for further medical advance. However, the field of back pain research probably has more experts than any other. Anybody who has back pain will, as often as not, expound views on all matters concerning causation and expectations. Indeed, it is not for a lack of ideas that this field of investigation has failed to make progress. A respected senior investigator advised me early on that patients could not even tell which side their pain was on. It was only many years later that I discovered why. Though the initial incident or accident would seem important to the patient and to the researcher, it is remarkable how little it helps distinguish patterns of back pain. Tests of inappropriate behaviour in the past have been widely disseminated and yet even the originators have said that they cannot be sure of the implications of positive non-organic test findings. This book will show that these 'indicators' are not what they seem to be. In many cases, it is difficult to tell which level in the lumbar spine the pain comes from, and thus treatments such as injections or operation, may be given at the wrong level, let alone the wrong side. Though the various forms of back pain treatment seem to help some patients, when these same treatments are subjected to randomised controlled trials, the evidence for efficacy seems to all but vanish. With all these difficulties and uncertainties, it can be seen why low back pain presents one of the most fallible fields of healthcare. This book indicates that there are ways of dealing with the majority of these problems. Many of the answers from the research conducted were most unexpected. The reader is also advised that the scientific approach cannot deliver hard and fast answers to all contemporary questions in such a wide field and much has depended on the choice of problems to study. Whilst the tests used to distinguish the patterns in this study have their limitations, and whilst there is, as yet, only limited evidence as to the underlying pathology for the patterns recognised, it is felt that some of the other patterns identified are descriptive of diagnoses that are generally accepted. If the answers that are put forward appear to change the direction of understanding, it is hoped that you, the reader, will be able to carry the enquiry further when you see patients, advise treatment or even experience back pain yourself!

Several studies have looked at populations of volunteers, women from the electoral roll and women attending general practices, and have reported the ultrasound appearance of polycystic ovaries in as many as one in three to one in five. If groups of women with irregular periods are looked at, up to 90% will have polycystic ovaries at ultrasound. Similarly, when women with increased facial and body hair are screened, about 90% will have polycystic ovaries, as will three out of four women with acne. It has also been suggested by some studies that women with recurrent miscarriage or early pregnancy loss after in-vitro fertilisation have an increased incidence of polycystic ovaries. However, not all studies support this hypothesis, and additional research is needed to evaluate this further. Medical interviews have changed a lot with time; in the last two decades, multiple mini interviews (MMIs) have revolutionized the assessment strategies for entrance to medical schools. MMIs usually consist of 6-10 stations of independent assessments done in a timed circuit so that the same examiner evaluates a specific skill for a station among the candidates. Many of these stations assess a candidates soft skills, including critical thinking, problem solving, team working, leadership and professional attitudes in addition to their knowledge, skills and motivation towards choosing medicine as a career. MMIs are not just a test of a candidates knowledge or skills, but their ability to use it appropriately at the right time. Aristotle said, we are what we repeatedly do. Excellence, then, is not an act, but a habit. Candidates wishing to succeed in medical MMI interviews need repetitive practice in the various stations. Modern science has shown that the widely held beliefs of clinicians about urinary tract infection (UTI) are wrong.
