Back pain is the commonest form of rheumatism and it is estimated to affect between 65% and 80% of populations sampled. The costs of back pain-related medical care and disability compensation make it one of society's most significant non-lethal medical conditions. Whilst mortality is low, its morbidity is high, inconvenience great, and economic burden massive. The actual symptom of back pain is one of the commonest and most perplexing encountered in medical practice. A proper understanding of what it means is crucial for further medical advance. However, the field of back pain research probably has more experts than any other. Anybody who has back pain will, as often as not, expound views on all matters concerning causation and expectations. Indeed, it is not for a lack of ideas that this field of investigation has failed to make progress. A respected senior investigator advised me early on that patients could not even tell which side their pain was on. It was only many years later that I discovered why. Though the initial incident or accident would seem important to the patient and to the researcher, it is remarkable how little it helps distinguish patterns of back pain. Tests of inappropriate behaviour in the past have been widely disseminated and yet even the originators have said that they cannot be sure of the implications of positive non-organic test findings. This book will show that these 'indicators' are not what they seem to be. In many cases, it is difficult to tell which level in the lumbar spine the pain comes from, and thus treatments such as injections or operation, may be given at the wrong level, let alone the wrong side. Though the various forms of back pain treatment seem to help some patients, when these same treatments are subjected to randomised controlled trials, the evidence for efficacy seems to all but vanish. With all these difficulties and uncertainties, it can be seen why low back pain presents one of the most fallible fields of healthcare. This book indicates that there are ways of dealing with the majority of these problems. Many of the answers from the research conducted were most unexpected. The reader is also advised that the scientific approach cannot deliver hard and fast answers to all contemporary questions in such a wide field and much has depended on the choice of problems to study. Whilst the tests used to distinguish the patterns in this study have their limitations, and whilst there is, as yet, only limited evidence as to the underlying pathology for the patterns recognised, it is felt that some of the other patterns identified are descriptive of diagnoses that are generally accepted. If the answers that are put forward appear to change the direction of understanding, it is hoped that you, the reader, will be able to carry the enquiry further when you see patients, advise treatment or even experience back pain yourself!

The Original Structuring model will be bifurcated into a Sources of Funds model, the assets of the collateral pool, and a Uses of Funds model, the liabilities waterfall model. This will give the reader experience working with an existing model as a base departure point of development rather than the less common “start from scratch” approach. Once the Base Asset Model (BAM) and the Base Liabilities Models (BLM) are complete, each model will serve as the platform for future development. From the BAM and BLM a pair of much more robust models, the Collateral Cash Flow Generator (CCFG) and the Liabilities Waterfall Model (LWM), respectively, are created. The CCFG is designed to process a significantly greater quantity, variety, and complexity of collateral types. It also introduces the analyst to Geographic based prepayment methods as well as those using Financial/Demographic factor approaches. The LWM, operating independently of the CCFG, reads the collateral cash flow scenarios and applies them across a two tiered, six tranche bond structure. The liabilities waterfall contains provision for an interest rate swap, performance triggers and a variety of other credit enhancement features. With the CCFG and LWM now complete Access is introduced. Access will replace various Excel/VBA code modules in the CCFG such as the collateral data management, data scrubbing and reporting, data sufficiency testing, and eligibility screening. Access is also employed to export and the various collateral cash flows. In the LWM Access is used to import the cash flows, store the liability structure specifications, and export/save the resultant structure performance. PowerPoint is then introduced in the CCFG to allow for the fully automated production of presentation report packages. Last Outlook is added to the models to inform the analyst of model performance and to automatically distribute the various report packages to pre-designated mailing lists. The book contains over 750 exhibits of reports, menus, calculation, and algorithmic examples and most of all code. The complete Excel, VBA, Access, PowerPoint, and Outlook code for all of the models is included on the Web site. The Web site also includes Web chapters containing detailed instructions on how to create and populate a directory environment to store this code, run the models, and organize the results. Lastly there are other Web chapters containing supplemental information explaining the structure and function of the original model, bond and mortgage math (with calculation examples), representative line generator programs, and a tutorial on the construction and integration of UserForms. The theoretical framework is informed by human needs understood as cognitive mechanisms of neural processes that regulate human behaviour and bio-values. In Strategic Coupling, Henry Wai-chung Yeung examines economic development and state-firm relations in East Asia, focusing in particular on South Korea, Taiwan, and Singapore. As a result of the massive changes of the last twenty-five years, new explanations must be found for the economic success and industrial transformation in the region. State-assisted startups and incubator firms in East Asia have become major players in the manufacture of products with a global reach: Taiwan's Hon Hai Precision has assembled more than 500 million iPhones, for instance, and South Korea’s Samsung provides the iPhone’s semiconductor chips and retina displays.Drawing on extensive interviews with top executives and senior government officials, Yeung argues that since the late 1980s, many East Asian firms have outgrown their home states, and are no longer dependent on state support; as a result the developmental state has lost much of its capacity to steer and direct industrialization. We cannot read the performance of national firms as a direct outcome of state action. Yeung calls for a thorough renovation of the still-dominant view that states are the primary engine of industrial transformation. He stresses action by national firms and traces various global production networks to incorporate both firm-specific activities and the international political economy. He identifies two sets of dynamics in these national-global articulations known as strategic coupling: coevolution in the confluence of state, firm, and global production networks, and the various strategies pursued by East Asian firms to attain competitive positions in the global marketplace.

L.T.C. Rolt’s fame was born from his unique ability to produce works of literature from subject matter seemingly ill suited to such treatment – engineering, canals, railways, steam engines, agricultural machinery, vintage cars – such as in his classic biographies of Brunel, Telford, Trevithick and the Stephensons, and in his superbly written volumes of autobiography. In Landscape with Machines Rolt told the story of his youth and his subsequent training as an engineer. That book ended with the fulfilment of his dream to convert the narrow boat Cressy into a floating home in which he could travel the then neglected waterways of England and, he hoped, earn his living as a writer. Landscape with Canals takes up the story at this point. It tells of voyages through the secret green water-lanes of England and Wales, and of the beginning of his writing career with the publication of his celebrated first book, Narrow Boat. The underlying theme of Landscape with Machines was the conflict between Rolt’s love for the English landscape and his life-long fascination with machines. In this sequel the same conflict is apparent yet we see how it was at least partly resolved. This is the testament of a man who has given literary shape to the history of the Industrial Revolution and who had a unique gift for imparting to others his knowledge, his enthusiasm and his love of life.

The Never-Taken Images documents a unique long-term project that Swiss photographers Françoise and Daniel Cartier have been pursuing since 1998. They have put together a vast collection of unfixed photographic papers, glass negatives, and films, mostly dating from 1880 to 1990. Samples of these are mounted and displayed, and, exposed to light over the course of several exhibitions, evolve towards colour saturation. Instead of looking at still images, the Cartiers’ installations, titled Wait and See, allow the viewers to perceive a kind of reality for themselves. The book features on around 100 pages the entire test catalogue that the Cartiers have put together to date, showing some 900 different papers and photosensitive supports. These facsimiles offer an almost real impression of their formats, colours, and materiality. Essays by Kathrin Schönegg, photo historian and curator, Thilo Koenig, scholar of art history and critic, and Christophe Brandt, former director of the Institute for the Conservation of Photographs at the University of Neuchâtel, supplement the images and place the Wait and See project in the art historical and technological context of abstract media art. Phenomenology of Civilization explores the philosophy of Edmund Husserl and R.G. Collingwood, two of the most influential philosophers of the twentieth century. Husserl founded phenomenology, which has had a direct effect on contemporary philosophy, and Collingwood, though less formally known, is still one of the most commonly read twentieth century philosophers. Maurice Eisenstein examines their work in relation to recent philosophy, particularly focusing on existentialism, Heideggerian phenomenology, and postmodernism. He brings these two philosophers together because they were contemporaries of each other, addressed the same audience, and, therefore, had similar issues to influence them. This discussion of Husserl and Collingwood's work moves beyond Husserl's phenomenology, and Collingwood's typical association with Hegel or Kant, to a new understanding of their ideas through an association with each other in regard to contemporary philosophy and political theory. Eisenstein's discoveries place Husserl and Collingwood into the main Western liberal political tradition with Dewey and James, rather than the more radical critique of that tradition with Sartre and Heidegger.

Evangelical Christians and members of the global LGBTQI human rights movement have vied for influence in Haiti since the 2010 earthquake. Each side accuses the other of serving foreign interests. Yet each proposes future foreign interventions on behalf of their respective causes despite the country’s traumatic past with European colonialism and American imperialism. As Erin L. Durban shows, two discourses dominate discussions of intervention. One maintains imperialist notions of a backward Haiti so riddled with cultural deficiencies that foreign supervision is necessary to overcome Haitians’ resistance to progress. The other sees Haiti as a modern but failed state that exists only through its capacity for violence, including homophobia. In the context of these competing claims, Durban explores the creative ways that same-sex desiring and gender creative Haitians contend with anti-LGBTQI violence and ongoing foreign intervention.
