Female scientists, technologists, engineers, and mathematicians worldwide are making historic contributions to their fields. The modern workforce is closer to gender-equal than it has ever been, and many efforts are in place to support further progress. However there's significant work being done to shift this tide. My footsteps thud as they hit the ground. I’m trying to pace my breathing; slow, quick breaths – in through the nose, out through the mouth. The terrain zips past me, obscured by the shadowy darkness as we make our descent of Kilimanjaro from Kibo Hut. Gugs is lying unconscious on a single-wheel bicycle stretcher carried by four guides. I look down to check my watch – only 15 minutes since we left Kibo. Suddenly my foot strikes a rock and I’m flung forward, unable to stop myself. I tuck in and somehow manage a soft landing on the rough gravel path. Luckily, I’m wearing my summit-night gear, which is well padded. Richard, the founder of Imbumba Foundation, stops and gives me a hand. As he hoists me up, he reaches for my shoulders and removes my backpack. He wears it on his chest and, now carrying two backpacks, instructs me to continue running. I quickly dust myself off but, as I look up, I realise that the team carrying Gugs a short distance ahead has also stopped. Panicked, I rush up to ask what’s wrong. The leading guide, Frank, says, “The drip’s not flowing.” My first thought is that there’s a blockage, but the problem is I don’t know how to fix it. In the dark, with the help of my head torch, I peer through the drip window and I realise it’s filled with the liquid. I don’t know anything about drips! Desperate for a miracle, I stare into the four faces. Nothing. “It looks blocked,” I blurt out. “What should we do?” The guide who’s been trying to hold up the drip while running, responds: “Well, if it’s not working, then I think I should stop carrying it. I’ll put it next to him so I can run properly.” No one responds so he does exactly that, securing the drip neatly between Gugs and the stretcher. Frank and I exchange a worried glance. He places a hand on Gugs’s neck to check his pulse. He looks back up at me. I read concern on his face so, shakily, I place two fingers just below Gugs’s chin. Nothing. Terrified, I pull my hand away almost immediately. I decide to check his wrist pulse and, as my fingers search for a beat, I’m hit with an Aha! moment. “Fitbit!” I find myself shouting. Gugs has two different Fitbit heart-rate monitors, one on each wrist. I had given one to him for his birthday two months earlier and the other he received as a gift from the Fitbit team two weeks ago, before we set off on our adventure. I click the one on his left wrist – it gives me a heart-rate reading of 185bpm. “Jeez!” I scream. My husband is clearly in deep trouble … My only consolation is that his heart is still beating. I show Frank how to check the heart rate and he nods. Our brief rest period has come to an end. We have roughly 28 kilometres to cover before we get to the bottom of the mountain, to an ambulance, which I pray will be waiting for us at KINAPA headquarters, the main gate to the Marangu Route. Their circumstances are a cut above those of Warda and her husband, yet, 'living on the old goats and chickens and a disability grant was never enough', and Alletjie who 'does everything' thinks it isn't fair, 'the mine owned her this future for herself'. Resurrecting again exerts a certain surreal appeal. A father takes to his bed because of a crushed pigeon or is it a metaphor for a crushed soul in the office? His son is told to pray but is there going to be a resurrection?

The misdistribution of health manpower is one of the major challenges faced by the Chinese health sector hindering people's access to healthcare. However, this subject remains underexplored, as spatial distribution characteristics have not been fully mapped out, nor have its causes been determined at the macro regional level. To fill this research void, this study aims to conduct sophisticated spatial analyses to examine the characteristics of health manpower distribution and test various determinants of health manpower densities. By introducing spatial techniques, this study sheds light on the distribution of health manpower in China, thus offering invaluable evidence for making national and regional policies to facilitate a more effective distribution of health manpower in China. On the one hand, this study used spatial statistics (space-time scan statistics and Moran's I statistics) to reveal, visualize, and compare the temporal trends, spatio-temporal dynamics, and spatial clustering patterns of different subtypes of health manpower (clinicians, doctors of traditional Chinese medicine, dentists, public health doctors, general practitioners, registered nurses, pharmacists, technologists, and interns) and their total. The results reveal that regional disparities and huge urban-rural imbalances exist in China's health manpower distribution, indicating that some provincial units have been left behind. In addition, spatial distribution characteristics varied across different subtypes of health manpower, which still held with respect to either the urban or rural health manpower distribution. On the other hand, this study theoretically modeled and empirically measured various determinants of health manpower distribution from both the supply and demand sides while taking the spillover effect between adjacent geographical units into consideration. Based on the results of the estimation, the health service demand was, as expected, found to forcefully drive the health manpower distribution across the nation. In light of this, the increase in health services demands in a province could significantly help one unit attract licensed doctors and registered nurses from adjacent units. Unexpectedly but intriguingly, the power of medical education capacity was relatively limited in increasing the health manpower density in local units compared to its spillover effect on neighboring units. In addition, government and social health expenditure played different roles in the health labor market, the former being more effective in increasing the stock of licensed doctors and registered nurses, the latter doing better in attracting dentists and general practitioners. This study has the potential to make profound contributions to the theory and practice of health manpower distribution in China. First, this study advances our understanding of characteristics of health manpower distribution by introducing spatial statistics. By doing so, this study visualizes the detailed spatial distribution characteristics with maps and identifies the priority areas for health manpower allocation in China. Second, this study provides a comprehensive research framework to understand the potential drivers of health manpower distribution, and more importantly, takes spillover effects into consideration and measures them for the first time. Third, this study systematically explores and compares the distribution characteristics and determinants of different subtypes of health manpower, which differ greatly. By exploring both the characteristics and determinants, this study gives a full picture of the health manpower distribution and provides new evidence for making health manpower allocation policies in China.

FROM THE PREFACESince Federal funding is scarce for massive upgrades, and/or complete new Wastewater Treatment Plants (WWTP) construction, treatment plant operators, superintendents, managers, city councils, boards, etc. must get more creative on funding and coordinating process equipment replacements. Contained herein you will find hints, tactics and procedures aimed at getting the "biggest bang for your public buck." During the 1970s and 1980s, through grants, the Federal Government paid 80% of costs to build new or expanded wastewater treatment plants, pumping stations and collection system renovations. The majority of the grants were to upgrade primary treatment facilities to secondary, and secondary to tertiary treatment status based on Clean Water Act regulations. If your facility was fortunate enough to receive grants, you were in good shape for approximately 20 to 30 years (depending on community growth rates). Since most wastewater treatment facilities are designed to last 20 years, many of the new or expanded facilities in the 70's and 80's are reaching the end of their service life. Some may have reached it sooner due to growth beyond the expected rate, inadequate preventive maintenance, or design inadequacies when built. Now you have identified problems with insufficient aeration capacity, equipment mechanical failure, insufficient pump station capacity, infrastructure deterioration, etc. and need to do something about it before you violate your NPDES permit (if you have not already). This equipment seems very costly to replace because you now must pay 100% opposed to 20% with the grants. Many WWTP are in need of replacement and/or upgraded equipment. The equipment itself is typically about 25% to 50% of the total project cost. This cannot be changed much. However, the remaining 50% to 75% (engineering, installation labor costs and project management) may be whittled down dependent on how active and creative the project coordinator (yourself) wants to be in the process. When EPA funded 80% of project costs in prior years, it was no big deal to have an "open pocketbook" attitude. Those days have changed forever and so have procurement procedures for projects. A Management Guide to Retrofitting Wastewater Treatment Plants is geared towards the managerial and administrative scope of a Lead Operator, Superintendent, Facility Manager type of Wastewater individual. All the junior college courses available, practical operator experience, and certification status will still not offer the opportunity to learn administrative and cost savings techniques (similar to operating a business). But soon, your job may demand these skills. This book is a handy reference for making the task of upgrading/retrofitting wastewater process equipment easier and less costly. It includes ideas for selling upgrade ideas to superiors, pre- and post-project activities, and certain management techniques useful for successful retrofitting or upgrading in past projects. This book should prove helpful to those who find themselves involved in retrofitting their facility, and need assistance on resolving facility problems, including treatment plant operators, superintendents, managers, city council members, and boards. It is also a valuable reference guide for municipal operations individuals who want to retain control of their facilities, but don't quite know how. It was written with the front line operator, superintendent, and manager in mind, in common operator language in order to allow easier understanding. It contains many tips and techniques which operators can implement immediately.

Excavations at Dryslwyn between 1980 and 1995 uncovered a masonry castle, founded in the late 1220s by Rhys Gryg for his son Maredudd ap Rhys, the first Lord of Dryslwyn. The first castle was a simple round tower and polygonal walled enclosure, within which were constructed a kitchen, prison and wood-framed, clay-floored great chamber beside a great hall. In the mid 13th century a second ward was added and the great chamber rebuilt in stone. This castle was greatly expanded in the period 1283-87 by Rhys ap Maredudd, the second and final Lord of Dryslwyn, who built an Outer Ward and gatehouse. He also rebuilt much of the Inner Ward, adding an extra storey to the great hall and great chamber, apartments and a chapel. At the end of the 13th century a large three-ward castle stretched along the eastern and southern edge of the hill while the rest of the hilltop was occupied by a settlement defended by a wall and substantial ditch with access through a gatehouse. This castle and its associated settlement were besieged and captured in 1287 by an English royal army of over 11,000 men following damage inflicted by a trebuchet and mining of the walls. Throughout the 14th century the English Crown garrisoned and repaired the castle, supervised by an appointed constable, before it was surrendered to Owain Glyn Dwr in 1403. During the early to mid 15th century the castle was deliberately walled up to deny its use to a potential enemy and it was subsequently looted and demolished. By the late 13th century, the castle had a white rendered and lime-washed appearance, creating a very dramatic and highly visible symbol of lordship. Internally, the lord's and guest apartments had decorative wall paintings and glazed windows. Evidence from charred beams still in situ, the sizes, shapes and distribution of nails, sheet lead, slates and postholes recovered during excavation has enabled some of the wooden as well as masonry buildings to be reconstructed. Waterlogged deposits had preserAt just 132 hectares (325 acres) the parish of Caldecote is one of the smallest parishes in Hertfordshire. Today the settlement comprises the manor house, until recently surrounded by a range of traditional farm buildings, together with six labourer's cottages and the church. To the north lies the site of the old rectory and the earthworks of a medieval settlement. In 1973 the Department of Environment and the Deserted Medieval Village Research Group arranged a rescue excavation to examine the earthworks of the medieval village before they were levelled and ploughed. Five crofts, the old rectory site and much of the moated enclosure were investigated in one of the largest excavations ever conducted on a later medieval rural site in Britain. Though the excavations did recover a Bronze Age beaker burial and small quantities of Roman and Iron Age pottery, the medieval settlement at Caldecote was probably founded in the 10th century, and by the time of the Domesday Survey there was a church, a priest and nine villeins. A moated site was added in the 13th century. A century later, Caldecote was granted to the abbots of the Benedictine monastery in St Albans, at a time when there were seventeen householders. Early in the second half of the 14th century, the estate and demesne were subdivided into six farms, each complete with a hall-house and two or more barns. Following the dissolution of the monastery in 1539, the manor was again held by an absentee lord and the farms continued to prosper. However, the late 16th and early 17th centuries, for which there are several surviving wills and inventories, saw their gradual abandonment.After the desertion of Caldecote Marish in 1698, Caldecote was farmed as a single unit until 1970, when the estate was attached to that adjoining the manor of Newnham. Nahuatl was the primary native language of central Mexico both before and after the Spanish conquest. It is the Latin of the indigenous languages of the New World. Its tradition of alphabetic writing goes back to the middle years of the sixteenth century and embraces not only grammars, dictionaries, collections of preconquest lore, and works of religious instruction, but also, above all, a great mass of mundane writing by the Nahuas themselves for their own purposes. Though the past quarter century has seen a flourishing of ethnohistorical, philological, and grammatical studies based on this corpus, those interested in the world of Nahuatl texts still find access to it difficult. James Lockhart, an eminent historian of early Latin America, is also perhaps the leading interpreter of this large body of work. He has translated and edited a wide range of texts, analyzed their cultural and linguistic implications, and over the years trained a large number of students, several of whom have gone on to become well known scholars of Nahuatl and other indigenous languages. Lockhart's main tools of instruction were: (1) a gradually growing set of lessons consisting primarily of examples culled from many sources of the sixteenth through eighteenth centuries (or concocted in the spirit of that time), and (2) the grammar or Arte of Nahuatl published in Spanish by the Florentine Jesuit Horacio de Carochi in 1645. In small groups of students, with a maximum of personal instruction and discussion, these materials accomplished their purpose, but the lessons were only in skeletal form, and the Carochi grammar, too, in the Spanish editions available, needed extensive explanation. Now, Lockhart has organized and expanded these materials into volumes that can be understood by students working alone or used in organized Nahuatl classes. The two books together will allow any seriously interested person to master Nahuatl sufficiently to begin reading the texts, and they will provide essential reference works as one progresses. They are geared primarily to the older form of the language and its written texts, but they can also be extremely useful to those studying the spoken Nahuatl of later times. Nahuatl as Written presumes no previous knowledge of the language. Treating all essential features of Nahuatl, it is organized on purely pedagogical principles, using techniques developed over many years of practical teaching experience. The book is in large format, almost like a workbook, with a great abundance of examples that serve as exercises; the examples are also available separately for the student's convenience. The orthography and vocabulary are those found in texts of the time, and the last several of the twenty lessons give the student training in working with texts as they were actually written. Some of the lessons deal with syntax in a way not found elsewhere and develop notions of anticipation and crossreference that are basic to Nahuatl grammar. In line with Lockhart's wish to bring more people into the Nahuatl documentary world, an Epilogue surveys many of the published Nahuatl texts and an Appendix presents substantial selections from ten different texts. Carochi's 1645 Grammar is the most influential work ever published on Nahuatl grammar and remains an essential work of reference. The best recent grammars of Nahuatl are based on it, but they have not exhausted it. It includes an extensive discussion of adverbial expressions and particles that is found nowhere else, as well as an irreplaceable fund of authentic examples from the time, translated by a contemporary. Though a facsimile edition is available, the original is very difficult to read, and only a few experts can fully understand the seventeenth-century Spanish and Latinate grammatical terms. This new edition presents the original Spanish and an English translation on facing pages. Helpful footnotes provide explanatory commentary and more literal translations of some of Carochi's examples. The volume is at once an indispensable pedagogical tool and the first critical edition of the premier monument of the Nahuatl grammatical literature. More is known about Nikita Khrushchev than about many former Soviet leaders, partly because of his own efforts to communicate through speeches, interviews, and memoirs. (A partial version of his memoirs was published in three volumes in 1970, 1974, and 1990, and a complete version was published in Russia in 1999 and will appear in an English translation to be published by Penn State Press.) But even with the opening of party and state archives in 1991, as William Taubman points out in his Foreword, many questions remain unanswered. "How did Khrushchev manage not only to survive Stalin but to succeed him? What led him to denounce his former master [an event that some interpreters herald as the first act in the drama that led to the end of the USSR]? How could a man of minimal formal education direct the affairs of a vast intercontinental empire in the nuclear age? Why did Khrushchev’s attempt to ease East-West tensions result in two of the worst crises of the Cold War in Berlin and Cuba? To resolve these and other contradictions, we need more than policy documents from archives and memoirs from associates. London's suburbs are home to many thousands of people who travel into the centre every day to work, but they also house many thousands who rarely find a reason to do so. They contain all the essential infrastructure for the city, too, including airports, offices, shopping centre, factories and warehouses. Outer London is therefore both metropolitan and suburban at the same time - it is Metroburbia. In this book Paul Knox examines the architectural history and development of London's suburbs, and celebrates their surprising variety and organized structure, refuting the common claim that they are monotonous or amorphous. The first chapter, The Foundations of Metroburbia, explains the foundation and development of Metroburbia and looks at how topography and geology influenced the siting of the villages that would become part of Greater London. The River Thames, of course, is one of London's most important and well-known structural elements, and in this chapter Knox examines how its meanders and bends have produced distinct patterns of settlement and development. He also describes in detail the seven distinctive sectors of London, which are (running clockwise from the west) the Thames Valley, Northwest London, North London, the Lea Valley, Northeast London, the Thames Estuary and South London. Finally, he looks at how early settlements, country estates and royal palaces shaped Metroburbia, and how the increase in roads and industry consolidated the development of what would become suburbia. Chapter 2, Pattern-book London, looks at Victorian and Edwardian suburbs - the first developments to be given that name. The building booms and their effect on employment in the city, and the difference in style and purpose between the various suburbs, are discussed, and Knox also examines the effects of immigration and industrialization on the city's housing requirements. He also describes the genesis of the parks, cemeteries and garden villages that now provide such valuable green space for Londoners, and the creation of the impressive industrial, civic and institutional buildings that are still striking parts of the city's infrastructure. Chapter 3, Inter-war Suburbia: Metro-Land and the Universal Plan, describes the acceleration of building projects between the wars and the beginning of the transition from Edwardian society to the modern welfare state. The term 'Metro-Land', introduced by the Metropolitan Railway Company in the early twentieth century, gives the chapter its title, and describes the expansion of residential London along the route of the Underground lines into Buckinghamshire. The effect of widespread car ownership is discussed, and the various housing styles - Stockbroker Tudor, Suburban Moderne, the mansion block, and so on - are described. The fourth chapter, Secular Reformation and Modernism, covers the thirty years from the end of the Second World War, during which time the welfare state brought about radical changes to life in London and the architecture of the city. Chapter 5, Counter-Reformation, describes the changes wrought on the country by the new neo-liberal agenda, as the welfare state was overtaken by a market-driven economy that fostered free-for-all development. By this time Metroburbia had spread outwards to incorporate Chelmsford, Southend-on-Sea, Maidstone, Guildford, Reading and Luton. This was an era of radical new infrastructure projects - from the rise of the suburban shopping centre to the construction of the new Thames Barrier - and huge increases in house prices. The regeneration of the Isle of Dogs into the Docklands commercial area is one of the most high-profile developments of the era, but infill house-building and small-scale environmental developments were also produced, and social housing regenerated. Finally, the last chapter, Megapolitan Futures, explores the various theories about the capital's future and conjectures about the shape of the city in the twenty-first century.
