Nova Science Publishers Inc Gaucher's Disease: From Diagnosis to Treatment
This book was designed as an educational resource for all professionals involved in the care of patients with Gaucher disease (GD). Chapter 1 is an introduction about GD. GD is the most common autosomal recessive lysosomal storage disease (LSD) due to the deficiency or absence of the activity of enzyme glucosylceramidase (GCase) or also known as acidï¢- glucosidase (GBA1) or uridine phosphate glucosylceramide synthase (UDP-GLC). Defects in these enzymes cause miss-sorting or loss function of lysosomal proteins leading to accumulation of glucocerebroside (GLC) in the tissue macrophages monocyte. Accumulation of GLC cause enlargement of the spleen, destruction of bone, anemia, thrombocytopenia, and abnormalities of the lungs. GD is classified into three types: type 1 GD (GD1) is a chronic and non-neuronopathic accounting for 95% of GD cases, and types 2 and 3 (GD2, GD3) involves nerve cell destruction causing acute brainstem dysfunction or progressive neuroleptic deterioration, respectively. Chapter 1 review disease classification, epidemiology, pathophysiology, and clinical manifestations of GD. Chapter 2 discusses GD diagnosis and clinical presentations from prenatal, new-born, first year of life, childhood, adolescence, and adulthood patients suspected to have GD. Several biomarkers, tools used to detect abnormal biomarkers such as chitotriosidase, pulmonary and activation-regulated chemokines, and other inflammatory markers are discussed, in addition to methods used to detect these biomarkers. Chapter 3 highlights available enzyme replacement therapy (ERT), clinical trials, dosing, and adverse drug reaction of each ERT, pregnancy information, and effect of ERT on endocrine and metabolic profiles. Chapter 4 discusses available substrate reduction therapy (SRT), clinical trials, dosing, adverse drug reaction, pregnancy information, drug-drug interaction and patients' polymorphism profiles that influence the dosing of SRT. Chapter 5 discusses type 2 and 3 GD which are the neuronopathic variants of the GD, genotype and phenotype of the patients, role of ERT or SRT or gene therapy, chaperone therapy, and hematopoietic stem cell transplantation. Chapter 6 discusses common complication of GD such as bone and hepatocellular abnormalities, increased risk of cancer and Parkinson Disease development. Chapter 7 highlights several novel therapies with their mechanisms of action, details regarding ongoing or completed clinical trials such as newer SRT lucerastat which is on clinical trials for possible use in GD3. At this time, there are no drugs available to treat GD2 or GD3. Promising novel therapy include pharmacology chaperone (PC), antioxidants, and gene therapy. There are 2 PCs currently being tested in clinical trial, ambroxol and arimoclomol. Ambroxol, an over-the-counter drugs has been used to treat various airway infections, showed promising result to cross blood brain barrier that provide promising options for GD3 treatment.

"Impossible Flying" is deeply felt writing that has an intensity and tautness which, if not new in Dawes' work, rises to new levels of eloquence. It is impossible to read this collection without feeling that one's consciousness of what it means to be human has been immeasurably deepened, or without wanting to constantly return to the poems.

Maintaining good metabolic profile plays a significant role in improving the quality of life at aging. Widely recommended physical and psychological strategies include exercise, calorie restriction (such as healthy diet), anti-aging neuroprotective and anti-inflammation therapy. Most occurring risks at middle age range (45-65 years old) are obesity, insulin resistance, inflammation, alteration in the hypothalamus-hypophysis suprarenal axis activity, stress and hypertension that could increase the prevalence of metabolic syndrome. Metabolic syndrome increases with age, particularly for women. Significant associations were seen between imaging measures and cardiovascular risk factors at both baseline and 18-month follow-ups. The formation of the Pathfinder Force in August 1942 produced a steady but certain change in the fortunes of Bomber Command. Its effectiveness against targets during the early years of the war had been very difficult to gauge. When examined in detail afterwards, aerial photographs showed that only one third of the aircraft were successfully reaching their target area and less than this were actually placing their bombs with target accuracy. It was known during the large-scale bombing of Coventry in the autumn of 1940 that the Germans had used an elite force of pathfinder aircraft, armed with incendiaries, who had acted as target finders for the main force of German bombers. What was now needed for the RAF were some similar specialist squadrons, with crews handpicked for their discipline, courage, high morale and, in particular, skills in a wider than normal range of flying jobs. Sidney Bufton, Deputy Director of Bomber Operations, developed the concept of a new Target Finding Force, and his tenacity in putting pressure upon senior Air Ministry staff to implement his ideas and bring Bomber Command out of the doldrums paid off. The new force was finally accepted and the choice of Don Bennett as its first Commander was inspired. Bennett was restless, imaginative, and receptive to change. He never accepted second best and he became a legend to all who served under him. Pathfinder Squadrons were equipped with the best available aircraft, which included the famous Lancaster bomber and later, increasingly, the Mosquito which was a hugely versatile and successful fighter bomber. Would be aircrew for the RAF came as volunteers from all parts of the world including Canada, America, Australia and New Zealand. To join a Pathfinder Squadron was a rare privilege but with it went a huge leap in the likelihood of being shot down. Pathfinder aircrew and aircraft had to lead the way for their following Bomber Force in hazardous raid after raid. They flew at night but it took a full 25 minutes to run the gauntlet of the Berlin defences from end to end at full stretch. They were highly vulnerable to the wall of flak thrown up by German city defenders, as well as to attacks by night-fighters. By the end of the war some 56,000 crewmen of Bomber Command had lost their lives. Martyn Chorlton has written a gripping account of the RAF's Pathfinder Squadrons, recalling the challenges faced in the smoke-filled skies over occupied Europe. It is also a tribute to the brave young men whose exploits, lives and, in all too many cases, deaths have left a powerful torch to bear for all who care about freedom. The infamous Nuremberg Doctors' Trials of 1946-47 revealed horrifying crimes -ranging from grotesque medical experiments on humans to mass murder-committed by physicians and other health care workers in Nazi Germany. But far more common, argue the authors of Cleansing the Fatherland, were the doctors who profited professionally and financially from the killings but were never called to task-and, indeed, were actively shielded by colleagues in postwar German medical organizations. The authors examine the role of German physicians in such infamous operations as the "T 4" euthanasia program (code-named for the Berlin address of its headquarters at Number 4 Tiergartenstrasse). They also reveal details of countless lesser known killings-all ordered by doctors and all in the name of public health. Maladjusted adolescents, the handicapped, foreign laborers too illto work, even German civilians who suffered mental breakdowns after air raids were "selected for treatment." (One physician who persisted in speaking of "killings" was officially reprimanded for his "negative attitude.") The book also includes original documents-never before published in English-that give unique and chilling insight into the everyday workings of Nazi medicine. Among them: * Minutes from a 1940 meeting of the Conference of German Mayors, at which a Nazi official gives the assembled politicians detailed instructions for the secret burial of murdered mental patients. * A pre-Nazi era questionnaire sent by the head of a state mental institution to parents of disabled children. (Sample question: "Would you agree to a painless shortening of your child's life after an expert had determined him incurably imbecilic?" Sample answer: "Yes, but I would prefer not to know.") * The diary of Dr. Hermann Voss, chief anatomist at the Reichs University of Posen (and later a highly respected physician in postwar Germany), who delights in the flowers blooming outside his window and worries that the overstock of Polish cadavers from his Gestapo suppliers might cause his crematory oven to break down. * Letters of Dr. Friedrich Mennecke, director of the notorious Eichberg Clinic, who writes with cloying sentimentality to the wife he calls "mommy" and comments offhandedly about visiting concentration camps to select "patients" for death. Today, as reports of mass death in Europe are once again cast in terms of public hygiene, and as euthanasia is advocated-even applauded-on U.S. television, the relevance of what Michael H.Kater here calls "the lessons of the Third Reich" is perhaps greater than ever.
