Our lives increasingly take place in ever more complex and interconnected networks that blur the boundaries we have traditionally used to define our social and political spaces. Accordingly, the policy problems that governments are called upon to deal with have become less clear-cut and far messier. This is particularly the case with climate change, environmental policy, transport, health and ageing - all areas in which the tried-and-tested linear policy solutions are increasingly inadequate or failing. What makes messy policy problems particularly uncomfortable for policy makers is that science and scientific knowledge have themselves become sources of uncertainty and ambiguity. Indeed what is to count as a 'rational solution' is itself now the subject of considerable debate and controversy. This book focuses on the intractable conflict that characterises policy debate about messy issues. The author first develops a framework for analysing these conflicts and then applies the conceptual framework to four very different policy issues: the environment - focussing on climate change - as well as transport, ageing and health. Using evidence from Europe, North America and the Asia-Pacific, the book compares how policy actors construct contending narratives in order to make sense of, and deal with, messy challenges. In the final section the author discusses the implications of the analysis for collective learning and adaptation processes. The aim is to contribute to a more refined understanding of policy-making in the face of uncertainty and, most importantly, to provide practical methods for critical reflection on policy and to point to sustainable adaptation pathways and learning mechanisms for policy formulation.

Worldwide, over 75 million people are involuntarily childless, a devastating experience for many with significant consequences for the social and psychological well-being of women in particular. Despite greater levels of infertility and strong cultural meanings attached to having children, little attention has been paid politically or academically to the needs of minority ethnic women and men. This groundbreaking volume is the first to highlight the ways in which diverse ethnic, cultural and religious identities impact upon understandings of technological solutions for infertility and associated treatment experiences within Western societies. It offers a corrective to the dominance of the narratives of hegemonic groups in infertility research. The collection begins with a discussion of fertility prevalence and access to treatment for minorities in the West and considers some of the key methodological challenges for social research on ethnicity and infertility. Drawing on primary research from the US, the UK, Eire, Germany, the Netherlands and Australia, the book then turns the spotlight onto the ways in which minority status and cultural and religious mores might impact on the experience of infertility and assisted reproductive technologies. The EU emissions trading scheme is the largest emissions control scheme in the world, capping almost half of European CO2 emissions. As the scheme emerges from its pilot phase, this special issue of Climate Policy journal analyses the lessons learned from the last two years and their implications for phase II. The volume presents some of the key analyses that helped inform the European Commission's decisions on national allocation plans, with research ranging from detailed country-by-country comparisons to more generic analysis that puts forward the case for harmonization. Challenging calls to seperate electricity from other sectors, a macroeconomic study suggests that the biggest efficiency gains come from inter-sectoral trading, even more than international trading. Empirical papers, which look at the expected scarcity of allowances in the market and merge models for the power and non-power sectors to project emissions and contrast these to the aggregate allocation volume, are complemented by two numerical simulations of trade and distributional effects, estimating the efficiency gains of the EU ETS in phase I and assessing allocation and distribution effects in the RGGI context.

Despite decades of macroeconomic reforms, poverty reduction plans and rural growth strategies, poverty is persistent and environmental degradation is accelerating in the developing world. Though traditional economic indicators have improved in some countries, little has worked to open enduring economic and ecological opportunities to the rural poor. This unacceptable outcome grows, in large part, from the failure to place the needs of the poor at the centre of national development strategies and to link local level changes to urgently needed changes in national development policies. This book is designed to change all of that by showing how change must begin at the local level and, from there, push upwards to change policies and institutions at higher levels to remove political, economic and institutional impediments that stifle opportunities for the rural poor and improved environmental management. Global warming is the most severe environmental challenge faced by humanity today and the costs of responding effectively will be high. While Russia's ratification of the Kyoto Protocol ensures the treaty's entry into force, lack of capacity, or incentives to renege on their commitments, will impede mitigation efforts in many countries. An important prerequisite for the proper functioning of the Protocol is that its compliance system - which is spelled out by the Marrakesh Accords - proves effective. The growth of pollution that crosses national borders represents a significant threat to human health and ecological sustainability. Various international agreements exist between countries to reduce risks to their populations, however there is often a mismatch between national territories of state responsibility and transboundary hazards. The broad aim of this book is to provide a general basis for comparatively analysing and understanding the French riots of October/November 2005 and the corresponding Bristish disorders which occurred in the spring/summer of 2001. The first of the French riots broke out on 27 October in the north Parisian banlieue (suburb) of Clichy-sous-Bois when two teenage youths of Muslim heritage were electrocuted in a substation while fleeing from the police. The two youths had apparently become unwittingly involved, together with their friends, in a police investigation of a break-in. It is not clear whether they had actually been chased by police officers. Nevertheless, a rumor to this effect quickly circulated the locality, provoking violent confrontation between youths and police. Three more weeks of rioting then ensued in neighbouring Parisian suburbs and other major French cities with similar concentrations of ethnic minorities. The riots invariably involved thousands of youths from poorer areas who confronted the police, set fire to local buildings and ignited hundreds of motor vehicles. Further rioting - though not on the same scale as in 2005 - occurred subsequently in 2006 and 2007. England and Wales have had their own counterparts to the French riots. In the early and mid 1980s, there were a number of clashes between police and African-Caribbean youths in inner-city areas. Further, in 2001 rioting broke out in the northern mill towns and cities of Bradford, Burnley, Leeds and Oldham. All of these later instances involved youths from Pakistani or Bangladeshi descent. In contrast to the riots that occurred in France though, a contributing factor to 2001 riots was the activities of white neo-Fascists. Many official reports and academic studies followed each wave of disorder, each questioning the effectiveness of Britain's 'multicultural' society, in addition to other possible factors such as the marginalisation and 'criminalisation' of minority ethnic youth, and their relations with the police. Such issues were again on the agenda after more rioting occurred in the Lozells area of Birmingham in 2005. Unlike the previous disorders, this entailed conflict between South Asian and African-Caribbean youths, following a rumor that a young African girl had been gang-raped by South Asians. British attempts to analyse and remedy the underlying causes of the riots constitute a potentially valuable resource to French academics, practitioners and policy makers. In turn, the French experience provides a fertile basis for re-applying, testing and enhancing existing British theory and policy. The book consists of a highly coherent, theoretically rich and thematically comprehensive collection of papers which provide an unparalleled description and comparative analysis of the French and British riots, along with social policy recommendations to help to address the underlying issues.

Transcatheter Valve Repair discusses all aspects related to percutaneous and established valve repair methods. The book is divided into few major sections covering all four valves and other topics. Each section contains several chapters discussing everything related to that valve. Beginning with the pulmonary valve, since it was the first valve to be tackled in the catheterization laboratory, and then moving to the aortic, then the mitral and then finally end with the tricuspid valve. 1.5M US citizens alone have some degree of aortic valve stenosis, with half (750K) requiring aortic valve replacement. Aortic valve replacement, on the whole, is performed by surgeons, requiring bypass machines and technicians, as well as the usual operating team. The operation is expensive and occupies a considerable amount of operating room time. Mostly, the aortic valve is calcified and the usual option available to patients is valve replacement with a variety of choices, ranging from porcine valves to synthetic, for which there are many manufacturers. It should be noted that the aortic valve is the most problematic of valves. Percutaneous procedures are the answer.The bottom line is that given the growing elderly population, many more patients will require valve repair, thus increasing health care costs with not only surgical operations but also hospitalisation. Percutanous valve repair, whilst requiring a cath lab team, does not involve bypass machines nor extended hospitalisation. Like percutaneous transluminal coronary artery interventions (PTCA) has replaced coronary artery bypass grafts (once the golden standard), and now stenting having replaced PTCA and its balloons. We now see drug eluting stents replacing ordinary stents (though at a much higher cost. There will be a huge movement toward percutanous valve repair, which should presumably cut costs but also morbidity and mortality.
