Mel Lewis (1929–1990) was born Melvin Sokoloff to Jewish Russian immigrants in Buffalo, New York. For eight decades the Primer on the Rheumatic Diseases has been the standard text from which most medical students and house ofcers have learned rheumatology. I myself will never forget thumbing through an older edition of the Primer as a second-year resident, while waiting to review a perplexing patient with my tutor. Fortunately the tutor was r- ning late with his own patients, so I had time to fip through the book – then much thinner – a couple of times. While turning the pages, per- ing the features of those diseases whose names were still exotic to me, and considering my patient’s history of conductive hearing loss and p- monary nodules, a light went on when I stumbled eventually on a part- ular chapter. I still remember the jaw-dropping efect on my tutor of my announcement then that I had a patient with Wegener’s granulomatosis. I think I became a rheumatologist that very moment! Subsequent editions of the Primer have sufered from the inevitable “obesity creep,” making it an outstanding reference textbook but virtually impossible to fip through quickly while awaiting one’s tutor, and even more difcult to slip into the pocket of a white coat to carry on rounds. For this reason we have created the Pocket Primer, a mini version that cuts the larger book down to its essentials.

Having driven the British and Indian Forces out of Burma in 1942, General Mutaguchi, Commanding the 15th Japanese Army, was obsessed by the conquest of India. In 1944 the British 14th Army, under its commander General Slim, drew back to the Imphal Plain before Mutaguchi's impending offensive.However to the north, the entire Japanese 31st Division had crossed the Chindwin and, on April 5, arrived at the hill-station and road junction of Kohima, cutting off Imphal except by air from the supply point at Dimpapur. Kohima was initially manned by only 266 men of the Assam Regiment and a few hundred convalescents and administrative troops. They were joined, on April 5, by 440 men of the Fourth Battalion of the Royal West Kent Regiment, straight from the Battle of Arakan.In pouring rain, under continual bombardment, this tiny garrison held the assaults of thirteen thousand Japanese troops in hand-tohand combat for sixteen days, an action described by Mountbatten as 'probably one of the greatest battles in history n effect the Battle of Burma, naked, unparalled herosim, the British/Indian Thermopylae'.

The 128-year dynasty of the Komneni (1057 to 1185) was the last great epoch of Byzantium, when the empire had to fend off Turkish and Norman foes simultaneously. Starting with the extremely able Alexios I, and unable now to count on help from the West, the Komneni played their strategic cards very well. Though the dynasty ended in cruelty and incompetence under Andronikos I (the Terrible), it fought a valiant rear-guard action in keeping eastern Christendom alive. The Komnene dynasty saw several changes in Byzantine military practice, such as the adoption of heavy cavalry on the western model, the extensive use of foreign mercenaries and the neglect of the navy (both of which were to prove a huge and possibly fatal disadvantage). A chapter is devoted to the famous Varangian Guard, which included many Saxons in exile following the Norman conquest of England. The terrible defeat at Myriokephalon in 1176 sealed the doom of the dynasty, preparing the way for the conquest of Constantinople by the Fourth Crusaders.
